The basic dilemma here: trust HbA1c or glucose tolerance test?
Unfortunately, at this encounter, PCP did not really ask about diet like
Rubyheart did. PCP's plan included:
1) started pt with metformin 500mg, and told pt to cut down carbohydrate
intake (without asking pt first what pt was eating),
2) ordered vit B12, folic acid, blood smear tests, which all came back as
normal.
3) RTC in some months.

标准？ 而不是普通人的标准？看起来普通人二型糖尿病的标准要松很多呢。。。打算

短期回北京。因为又有人邀请回去工作，自己因为父母的原因也在考虑，所以比以前更
“认真”地考查了一下。时间很多，其实只是管中窥豹。
1)食。一般的居民区超市，食品大约是美国的1/3(主食、低质量水果、饮料)到4/3(高
级水果)，这是我粗略的估计，因为我对美国超市价格其实不太注意。饭馆的花销大概
是美国的1/2。综合来看，吃上的花销应该是美国的1/2左右。
2)衣。很便宜的有，大概是美国的1/3到1/2。但是如果想看不上去不老土，最好买跟美
国价格仿佛的衣物。如果去酒吧之类的地方，还是最好买点品牌的潮货，那个价格就没
谱了。综合来看，对我而言，衣服的花销应该是和美国差不多。
3)行。北京的公交车跟以前比不算很挤，但是还是不舒服，冬冷夏闷。地铁不错，就是
线路少，但是在修。出租车非常便宜，但是一旦有点雨、上下班高峰，也就是你想打车

我的个人看法是：首先要搞清楚高血压的原因。是家族遗传问题，还是伴随血糖产生的
，和胰岛素抗性是否有联系，心电图结果如何等。然后才能确定跑步是否有效。假如没
有什么特殊的原因，就是大多数人那样，活动少，饮食没有注意（比如米面特别多），
血脂也高一些，心脏机能弱，那么跑步应该会有一些效果。
有人专门做过这方面的研究，就是跑步是否可以改善高血压：

Match的结果终于尘埃落定，周1等电邮的时候非常紧张，因为成败就在此了，虽然一直
告诉自己，今年只是热热身，成败都可以接受，但到真的揭晓的时候，还是很紧张，毕
竟一旦卷入其中就身不由己，何况自己也还是有所付出。到周4等match program的时候
虽然也有点紧张，但程度已经大大减少，1个原因是毕竟match上才是头等大事，去哪里
尚在其次，还有1个原因是心理还是有所准备了，虽然希望留在home school，但直觉告
诉我恐怕是非常困难了，在我rank list上第二位的program应该就是我要去的地方，果
不其然。经过几天的等待，发现果然是要搬家了。

during pregnancy.
1) Your diabetes only gets worse during pregnancy. This is a natural
response of pregnancy for the fetus to get glucose. A special hormone
is produced by the placenta to slightly increase sugar level in the
blood. But if you have diabetes, your sugar will go out of control.
There is no way you can cure your diabetes during pregnancy.
2) You absolutely should get your sugar in control during pregnancy.

1. Glucose tolerance test should be done properly before we can rely on its
result. It is very important to make sure for days or even weeks before the
test, the patient has not been restricting carbohydrate. Secondly the
patient should be fasted properly (8hrs rule). You would be surprised how
people understand fasting.lol I have had a patient (back in china) who
thought fasting means no meals but fruits doesn't count so she had some
grape before bed...

简单再说一下三个减肥药, 算是最后的总结: 我认为在FDA决定前三个股票都可以trade
, as far as you buy low and sell high. The only reason I recommended VVUS in
the past is from speculative FA speaking, it is in relative stronger
position. And market do agree with me.
我以前没怎么提,但是减肥药费用不能保险公司支付, insurance companies don't
reimburse obesity drugs这点很重要. 也许这个市场并非象好多人想象的那么大, 另
外有效性不高的药(ARNA 3.6%, OREX 4.5%)即使批准也很难说服病人放弃高效的药(
VVUS 10%)或是便宜的药(metformin, sibutramine, phentermine, bupropion, and

diabetes is chronic disease that will not go away, peri-operatively diabetic
patients can be put on insulin drip for better glycemic control,
poorly controlled diabetes surely would interfere with healing and post-op
infections as such but should not delay surgeries, especially such a
cholecystectomy which nowadays often is performed laprascopically,
if it's symptomatic cholelithiasis yes you can electively have the
gallbladder removed after you have diabetes better controlled(which will not

今年我妈住了两次院。现在我看， 好像都不是非住院不可的毛病。但医院从利益出发
，轻易建议住院, 过度治疗， 病人结果适得其反。
第一次是年初。一天早晨我妈起床上厕所，不知怎么就摔倒在床边，短暂昏迷。我爸随
后扶她去了医院。诊断是脑梗，收住院，点滴稀释血液的药， 同时点滴所谓有营养功
能的中药制剂。最多的时候，一天里输液有七种药品。我妈本来血压还不大高，输了几
天液，血压就高上去了， 腿走路都没劲了。她以前做过核磁共振， 当时大夫说，那结
果，不要说是六十多岁的人的，就算是四十多岁的人，都算正常。这次住院又作了核磁
共振，和一两年前比较，并无大变化，但脑梗的诊断不变。每次我妈抱怨不舒服，大夫
就再给加一种药输液。后来我妈实在受不了了，说，“走着进的医院， 住了几天，反
倒不能走路了”。终于在两个多星期后坚持出院。回家后， 不用那些药， 血压又正常
