，有三个电话，主要问你对某某方面熟不熟，我说没做过，马上没戏了（譬如说该公司
想开发的药与diabete的蛋白降解有关，本人是做蛋白降解的，但对方问我是否做过
diabete，我说没有，马上就说你不match）有个公司我回答说都做过，给了我on-site
机会，但面试时问得非常 detail，包括一些实验的溶液配方，我有些技术只是看本实
验室的人做过，不熟，所以对方就很不满意。
感觉是应聘的人太多了，他们慢慢挑。 |

比如同样obese的两个人，具体到相同大小的omental和subcutaneous fat pad，
可能就有两种phenotypes，一种是细胞数量少，但是单个fat细胞很大，
另外一种细胞数量多，单个细胞小。
前者，往往伴随严重的insulin resistance，type 2 diabetes，
后者就很健康，跟没事人一样。
这里面就涉及到不同的adipogenesis program，为什么每个人不一样，
细节现在应该还不是太清楚。
另外adipocyte store fat到一定阶段，到底什么原因就induce apoptosis/cell death
了，
这个也还不清楚。有人说是hypoxia, lipid toxicity啥的，
也有人扯别的，现在还没common sense,
但是这个问题对于diabetes，obesity来说还是很重要的。

向楼主这种情况我觉得应该是induce type2 diabetes，所以我也觉得high fat diet比
较好
喂个几个月WT老鼠应该都变得超级胖，如果gene A真的在metabolism里面起作用的话，
那么highfat diet会更突出它的作用
至于preadipocyte的话，可以搞primary culture的preadipocyte，然后让他们分化，
看他们differentiation的能力 |

top10 医学院做了2.5年糖尿病临床trial的postdoc，文章大概15片，一作大概7，8片
，有diabetes care（》9分），endocrinology（5，6分），etc，常年为diabetes等杂
志审稿。目前有3年Gap。我一点儿也不想当faculty，只想找份稳定的工作，其实进工
业界是我最大的心愿，是不是3年的Gap，工业界就不要想了？因为location限制，想重
做postdoc好像也很难（只能找当地的一所学校），又不太可能因为postdoc的职位（如
果有的话）两地。目前很迷茫，不知自己今后能做些什么。lg到是无所谓，随便我。可
读了那么多年书，做一辈子家庭主妇好像自己又不太心甘。请板上的牛人们指点一下，
我的人生之路该怎么走？其实2年前就想工作，一直没有什么机会。做了这个专业，我

内科第一年intern
从去年开始，traditional call system（每3、4、5天call一次）不符合现在的要求，
现在新规定出来后intern连续工作不能超过16or24小时（不确定）
自己所在program，有night float，从前天晚上（偶不确定是晚上还是早上。。。）六点到第二天早上七点半，每人每年轮一个月
不值night float的时候，就值白班
一、Intern之前的准备：
（一）Limited license：
根据所在州的决定，工作一、两年后，可以申请full license，正式去program上岗前，必须拿到limited license，最好在7月1日前， 有了这个之后，才能有以下的两个号：NPI（你的全国的编号）、DEA（你签名开药需要）
tele所在州board of medicine一个月开一次会，review一次你所交的资料
tele是七年制：毕业证书是五年的时候拿到，但在五年之前没有实习，是七年的后两年实习，类似这种情况，任何时候有discrepency，就要自己所在的医学院出具文件说明

的拥趸，于是就买了本新版MTB3. 拿到手先翻了翻，很是困惑。虽然我有心理准备，
这本书可能比较简练，但是还是吃了一惊。随便看了看type 1 diabetes DKA的处理，
居然只有补液加胰岛素和补钾。这也太简单了吧。再随便找了找，楞是没有找到
diabetes insipidus，和SIADH。话说这两diseases也是挺重要的，这是怎么回事？我
科研背景偏内分泌，所以可能有
bias？请教用MTB3复习step3的童鞋梦有否发现同样的感觉？
今天去书店看了看2010版的step3 FA，发现好像东西还多一些。大家有没有比较？

成绩出来了，涉险挤入了９９队伍。总的来说，准备的不是很充份，详见以前贴的＂我
的step ３经历＂，主要是在吃CK的老本；其他乏善可陈，成绩单里唯一的一个星号是
给了CCS，这里就说一说CCS好了。
以前总把CCS和CS混淆，其实这两个有一个共同点就是open choices和uncertainty。在
这总场合，最重要的可能就是要有一个严格的protocol，这样可以尽量避免遗漏。考过
CS的同学都知道LIQOR AAA和PAMHUGSFOSS的重要性。CCS也一样。网上比较流行的是
Neeraj的protocol，用google一搜就能找到。非常有用。我把他的protocol稍微整理了
一下，自以为是更加简洁有条理一些。给几个朋友用了用，有的朋友觉的没有时间应用
，有的朋友用了觉的还成，让我贴出来，最后的决定是如果考出来还行就贴，要不就不
出来吓人了。

48. Diabetic ketoacidosis is maily caused by type 1 DM, but can also be
caused by type II. Thus, lack of insulin or not enough insulin. As for HHNS,
only type II causes it, not type I. Diabetes insipidus is caused by lack of

机会：这样的机会很难得。以前有个女的到我们科临时加班做tech，中午吃饭就跟她聊
了，结果发现她是我们system里的research institute的research coordinator。她最
早以前是tech，后来熟人介绍她去做research coordinator。tech只是她额外的收入。
我跟她说了我的情况，我以前申请了几次，都在hr就被拒了或者说职位被取消了，后来
就放弃了。而且HR的申请上说必须有一年的research 经验，我每次都硬者头皮投，希
望我以前在国内的research经验能算上，但都失败告终。我给她留了我的名字，电话和
work email。她说如果有位置会替我操个心的，还要我email我的resume给她。回家后
想了想，这只是客套话，而且我不想把自己的resume发给一个几乎是素不相识的人。所
以我就没有发。
惊喜：上个星期突然收到她的email说有个位置，看我是否感兴趣，并给我她的manager
的联系方式。我当天立即send resume。
