背景：
11岁的孩子，目前体育活动只有游泳。
以前小时候（四年前）练过体操，大概半年吧，怕受伤，不让她练了。
前两年练过跳舞，曾经崴脚，很长时间才好。
病情：
三个月前，抱怨右脚脚踝疼，以为崴脚了，过了一个月去看大夫。照MRI，诊断是
Osteochondral Lesion。上cast一个月，又续用boot两周。不疼了。上周恢复游泳训练
，昨天在游泳训练中，又开始疼痛。
大夫的建议是做手术，在骨头上面打洞，刺激骨头自我修复。
我的担心是：
1）手术中损伤growth plate的可能性大不大？
2）手术的预后怎么样？是否彻底解决问题？
还有，一直搞不懂这个到底怎么引起的。难道同以前崴脚有关吗？还是食品营养？孩子
的身体很健康，比同龄人高些，平时很爱活动，从来不疲劳。

，发现是可能是视网膜肿瘤。昨天医院CT和MRI的结果出来，确诊是Trilateral
retinoblastoma。小孩的父母非常着急，希望能够保住小孩的眼睛。请大家给出宝贵意

1. her dad's oncologist doesn't seem to have a good knowledge of the
mutations of the tumor, the complexity of the pathways and the mechanisms of
these inhibitors.
2. his intracranial metastasis is a real big concern. if you think those
more conventional drugs are bad in getting into the brain (and the tumor),

7月份b超是这样的，大家看看像不像恶性? 甲状腺双侧叶及峡部大小形体正常，被膜光
滑，腺体组织回声欠均匀，稍增粗，甲状腺左叶内见多处大小不等低等回声结节，较大
者位于腺体浅层，大小2.4*1.0cm,边界清晰。右叶中部深面似见0.7*0.4等回声结节，
边界欠清晰,CDFI上述结节周边可采集散在血流信号。左颌下见1.7*0.8淋巴结回声，左
颌下见2.1*0.9淋巴结回声。
8月份在国内另一家做的，想看一下那个大的变大没，没想到不同医院仪器不一样，没
法比较：甲状腺右叶1.79cm,峡部厚0.32cm,左叶厚1.89cm,实质回声增粗欠均，甲状腺
左叶可见中等回声结节，大小约2.36*1.15cm,边界尚清CDFI:期内可见血流信号显示，

further characterize the adrenal lesion as well as hepatic lesions. Rads are
very comfortable calling TCCs and RCCs on CT. For adrenal adenomas, you
need three phase CTs or MR. Also for the liver lesion, CT liver protocol or
MR are better to tell most of the lesions.

小46x35mm，说比较第一次32mm变大了。可能第一次看片医生那天喝多了？肿瘤位置在
Fundus，非常接近GOJ。
AFP好久没有查了，因为肝上的情况一直很好，肝移植前AFP 100多，移植后AFP一直小
于2。上一次至少是2，3年前。也是疏忽了。现在AFP 254. 显然胃上这个要么是转移，
要么是HAC。
下周二医生还要安排一次PETCT，周三见外科医生，估计排除转移外就安排切除了。 求
这里的专家教授指点，这个情况如何是好？下面我贴上最近几个报告。

膝关节的关节炎和跑步没有必然的联系，有文章做过专门的研究。但是最近又有文章说
鞋会影响膝关
节受的冲击，从而影响膝关节的受伤，这就间接证明跑步和膝关节关节炎还是有关联的
，但是那个研
究认为光脚跑最好。所以暂时没有一个统一的说法。
我感觉overuse肯定会增加关节炎的可能性。理由是lesion(初级阶段)一般都出现在受
载严重的地
方，外科治疗也经常去不受力的部位挖块好的软骨来补到烂洞里面。成年人软骨里面没
有神经没有血
管，只有稀稀拉拉分开的一些细胞，然后就是水，胶原蛋白和一些粘乎乎的鼻涕一样的
糖。受力，润
滑，缓冲全靠他们，而且要靠一辈子。细胞产生新胶原蛋白和糖的能力很差，速度也慢
，所以细胞外
的结构一旦因为长期过载磨损了，再生就困难了。细胞的营养主要靠关节液，关节液来
源于淋巴系
统。这个过程也不是很给力的，所以细胞自己活着就不容易，更别说生产新的细胞外组
织了。跑步的
时候，软骨受到长时间带有冲击的循环加载，温度的升高，水被挤出软骨，都会慢慢增
加摩擦系数，

我最近忽悠了几个local runner来做高精度骨结构扫描，看到一些有意思的结果。样本
很小，就4个（还加上我自己）。3个的trabecular bone (松质骨)结构和正常人有差异
，主要体现在有变粗变大的骨小梁，并且成片连接在一起。初步估计，在开始过长时间
，过大负荷的给予骨头Loading,unloading，造成了非常小的microfracture,
microdamage，恢复后就形成了microcallus(骨痂）。唯一一个跑完骨头一点事没有的
，当当当当，就是我们的兽人tonyprince同学。
下面按顺序是：1.vivil：我的只显示有很小的一点异常，在接近左下cortical bone（
硬质骨）的地方。

1. "If cancer strictly follows evolution model, should one imagine certain
genetic lesions gain dominance?" 某些肿瘤可能的确是这样的,比如一些EGFR突变
的NSCLC,表现出oncogene addictive,这种genetic lesion似乎dominate肿瘤的生长,因
为对突变的EGFR进行target therapy（erlotinib）具有疗效．当然oncogene
addictive也并不排斥CSC.
2. 关于癌基因会诱导正常细胞senescence or apoptosis.这是事实，但一种可能的解
释是，这种或这些癌基因并非是tumor initiator,它们只是promoter或maintainer;也
就是说在它们异常之前，有其他genetic lesions作为tumor initiating factors，而
这些我们还没找到？

有快20篇了。其中有cell, neuron, Journal of neuroscience, PNAS 好多篇。而且他
1995年就回国了，那个时候国内的环境有多差不用说了吧。这也快20年了。比饶益2007
年才全职回去多了12年。比起培养的中国学生也多多了，算国内贡献肯定不会小。如果
