零花钱
通常说的medical physics分两大块，主要是radiation therapy 和 diagnostic
imaging，radiation therapy占多数，pay得也比imaging多个三四万，比较popular，
不过我的background在imaging这边。therapy的工资据说在12到18万间，imaging的工
资据说在10-15万间
现在招medical physics都require ABR certify或者eligible for certification，
ABR certify要求你在campep accredit的graduate program修课，在campep acredit的
residency program里训练，然后才能参加ABR的certification考试，说白了就是人为
设置门槛，抬价

经常有家长问：我的孩子快2岁了，还不会说话，怎么办？ 我的建议是联系early
intervention。尽管很多人会说：不要紧，谁谁的孩子3岁才说话，现在不是好好的。
但是，作为父母，你应该意识到语言发展迟缓的原因有多种。别 的孩子语言发展迟缓
的原因不见得是你的孩子发展迟缓的原因。不错，大部分孩子后来都catch up了，可能
和typically developing children没什么差别。但是，你要不要白白得等下去，等孩
子自己catch up。如果你等到了，祝贺你！但万一没有，岂不是浪费了珍贵的发展期。

，不知道该怎么办。之前六个月加辅食的时候，娃还吃得很好的，蔬菜水果，买的自己
做的，都吃得没问题，九个月左右的时候，就能吃一小碗肉汤煮的碎碎的面条了。
然而在十个月多一点的时候，他就开始拒绝各种食物，有颗粒的不吃（stage3的jar
food），能用舌头捋出来。菜泥，水果泥不吃。粥从来都不吃，煮得再烂，他都能呕出
来。面条偶尔吃几根，就再也不张口。各种肉，肉泥，肉末都不吃。我给他做各种食物
（家里买了好几本婴儿食物书），咸的不吃，甜的也不吃。仅有能够入口的yogurt，牛
油果，蒸鸡蛋，牛奶泡的燕麦糊。如果在这其中混一点其他东西，就不吃了。以为他想
吃大人的东西，结果给他一点，他伸出舌头舔一下，也不吃了。但是喝奶很凶，一天能
喝32oz（4次）。当时想给他减奶，但是他哪怕饿着，也不吃辅食，就只好维持这个奶
量了。
一岁体检，身高32.5inches，体重23lb，跟医生说了他吃饭的问题，医生觉得还好，还
有愿意吃的，提醒我们要减少奶量。结果看完医生回来这两个星期，我们逐渐减少了奶

即便有自闭，也应该是轻度的。
从解决问题的角度来出发，作为母亲你必须做点什么。
是不是autism只是一个标签，重要的是你怎样帮助孩子更好的发展。
我尝试给点实际的建议，供参考。
先看你的孩子最明显的问题：
1. language delay.
2. not following directions.
3. lack of social skills.
4. repetitive behavior (stimming).
要解决这些问题，常规的做法无非是找专业人士提供特殊教育或者服务（service）。1
需要speech therapy，2-4需要intervention，一般是ABA therapy。

介入越好，这样没准在娃懂事之前就discharge了。你娃学校才两学生要speech
therapy. 我娃学校很多娃要speech therapy. speech therapist每天有12-14个group
therapy（不是one-on-one的）. 算下来一天差不多有20-30个娃有speech therapy. 不
过我娃学校speech therapist工资固定，不管多少娃都一个工资，可能是因为这个学校
K和1年级很多娃都在speech therapy, 都算在special ed里，越往上越少娃要speech
therapy. |

。硕士的时候做过一个AAV载体preclinical trial的相关小课题，算是刚入过门，不知
道版上有没有专业做这个方向的出来交流一下。
不少人对gene therapy都经历过从一个极端走到另一个极端的认识过程，对也好错也吧
这里不做评论。gene therapy本身也经历过数次潮起潮落跌宕起伏，可以说刺激无比。
故事的开始应该是在大约20年前，1990年9月1FDA批准首例基因治疗临床实验，接受治
疗的是一名4岁女孩，患有先天性重症联合免疫缺陷综合症。结果异常成功（至少在当
时看来），女孩可以走出隔离间而免于因各种感染而死亡。在那个年代这无疑是一针强
力兴奋剂，直接催生了基因治疗的第一波高潮。随后几年中，世界各地有大约400个
trial被实施，超过3000人接受了相关治疗，从现在看来，确实有点不可思议的味道。
这些临床方案主要是单基因遗传病、肿瘤以及感染性疾病以及骨髓移植免疫排斥的消除
等。无论如何，大部分的结果都还是积极的，虽然必然也伴随着一些副作用比如抗自身

就是鼓捣鼓捣那几个病毒 改来改去 了不得发个molecular therapy 6 7分的文章。运
气好了 PNAS。
所以一般的lab都是即做点gene therapy 也带点basic biology做做。做AAV的大牛
mark kay 早就开始做gene therapy以外的东东了

这个行业的出发点是政府强制，有很多经费，和27年“宁可错杀三千，不可放过一个”
有类似。
各种专业人员纷纷盯上了这个钱。少数儿童“心理医生大”打头，一年半年做个评价收
费，然后医院和独立的机构开设这种therapy program赚钱。心理医生赚的小钱，似乎
是掮客的地位，也乐于给个帽子，让后继的therapy赚大钱。确实是个行业。
积极点看，瞎猫碰死耗子，总能碰上帮助少数几个真正的autism.瞎猫比没有猫强点。
消极点看，明明是活耗子，只是不perfect，有时懒得动的小耗子，被心理医生那么短
短的两个小时观察到了，然后错杀三千，就被划归为死耗子，转给后继的therapy
program去了。而且therapy的好的真的巨少。
这个行当有很多坑：
＊ ASD的typical和外围症状有太多的种类，要找的话，除了那种天性外向脾气好的小
孩子，其他小孩子基本上都能或多或少找出来。
＊ 宣传的铺天盖地，父母很容带上放大镜去找，然后评估师根据高标准，1sigma边缘

是昨天我不小心看到了一篇新闻文章，才惊讶的发现其实最早做出来的，有可能是
Wayne State University的潘卓华。虽然他的投稿时间和Deisseroth的类似，但是因为
他是一个没有什么名气的学者，而且他的文章包装得不对，所以他的文章被百般耽误，
最后比Deisseroth的晚了一年才发表。
在2004年底潘就做出了和Deisseroth几乎一样的文章，除了用的细胞略微不同：他做的
神经细胞是从视网膜分离的，而Deisseroth 做的神经细胞是从大脑分离的。然后他把
文章陆续投向Nature, Nature Neuroscience,Journal of Neuroscience，却统统遭到
了拒稿。然后大半年之后，也就是2005年8月份，Deisseroth 的文章发表在Nature
Neuroscience上。潘得知之后，心都凉了。

1. stage iiB disease has a good prognosis if actively managed. so don't
worry too much.
2. UVA phototherapy is recommended unless it is not available.
3. bexarotene is a newer drug recommended over first generation retinoids.
if you are taking first generation retinoids, you may ask your doctor to
consider switching you to the newer drug. one common adverse effect of

laser therapy.
有牙保险, 但保险一点都不cover laser therapy, 而且这个laser therapy很贵, 一颗
牙155刀.
我知道自己需要做peridontal scaling, 这个保险cover大部分.
但我不清楚到底需不需要做laser therapy,牙医诊所的人说不做laser,等于没做
cleaning,我很怀疑, 怕是看我不懂, 忽悠我.
小弟想请教一下, 这个laser therapy需要做么? 只做deep cleaning(periodontal
scaling)可不可以?
还有湾区有收cigna dhmo的中国牙医么? 去年选牙保险失误了, 没当回事选了hmo, 哎,
以后一定选ppo.

我们州的EI，不负责诊断，只负责评估孩子各方面的能力，包括motor, social-
emotional, communication, adaptive/self help skills, and cognitive skills.
如果有motor delay，fine motor 和gross motor还可以分别打分。除非有特别情况，
每种需要用的therapy是一周一次，一次一小时。如果一个孩子同时有gross motor
delay, fine motor delay和speech delay，他可能得到一周一次，一次一小时的
physical therapy, occupational therapy and speech therapy, 也就是加起来一周
三小时。
如果有医生的诊断，提供每一种therapy的频率可能超过一周一次，一次一小时。 |

你儿子最需要的，是去做个evaluation，然后开始做speech therapy和OT therapy。我
知道你觉得他不是自闭，但是你说的各种情况，都表明他是有语言delay的。别人家的
孩子能同时handle多少不同的语言都与你儿子无关，你儿子的英语需要很多很多的帮助
，一个好的speech therapist，几个星期就会让你儿子的生活好起来。
我女儿3岁半时，中英互换完全没有问题，听说都很好。儿子就完全不同，11个月开始
上daycare，18个月时还只能说几个字，而且说话的意愿不强，不愿主动开口。他那时
也能follow direction，在daycare和老师同学的互动也没什么问题，但儿医给了两个
option：可以等到2岁体检再看，也可以现在就做evaluation。我们选择了马上做
evaluation，幸运地马上排上了speech 和OT。1个月过后，他能说的字就多起来了，2

therapy, speech therapy, physical therapy 的训练，会改善社交能力和生活能力。
看新闻说他节目录制的时候的了肠胃炎，不知道是不是这个原因他姐姐一直扶着他，很
多自闭症的人有motor skill的障碍，不知道他有没有motor skill的问题。李永波说要
帮他，最好是能帮他制订一套occupational therapy and physical therapy 的方法帮
助训练，基本生活自理会没有问题。 |

确实是中药里少有的几个成功的例子，和青蒿素一样是伟大的发现。但是不知道砒霜完
胜的结论是怎么来的，治疗NEWLY DIAGNOSED APL的STANDARD THERAPY不是砒霜，而是
ATRA+anthracycline-based chemotherapy,我猜ATRA就是所谓的全反式维甲酸吧，我只
看到一个中国的随机对照实验显示在这种情况下砒霜优于ATRA，
我还看到有其他数据表明砒霜不如ATRA+anthracycline-based chemotherapy，显然砒
霜更好的证据不足，要不然的的话，砒霜就是STANDARD THERAPY了 。砒霜只是在APL经
standard therapy治疗复发后是最好的选择，所以它是ATRA+anthracycline-based

我也看了这片文章，问了一下我们医院实验室的一个fellow(他已经做完了三年的
residency,是美国top10的MD/PhD)，他的意见是，I don't know how they can
publish it on NEJM with only 1 patient response out of total 3. They must
have very good relationship with the editor.
我的看法，首先，chimeric antigen receptor CAR这个概念一点都不新鲜，
preclinical已经做了很多年。但是几乎所有的实验到了clinical trial都失败了。主
要问题是回输的T cell到了病人体内好像细胞毒功能和增殖都变得很弱。但是这次的实
验是chemotherapy和immunotherapy联用，可能chemotherapy杀死了很多肿瘤细胞，这
些死亡或者损伤的肿瘤细胞变得容易被T cell识别和杀灭。作者在讨论里也提及这种可
