发帖数: 319 | 7 我的外公94岁了，end-stage renal failure (尿毒症)，脚肿(还没有到脸部和其他身
体部位)，心脏不舒服，无力。平时有高血压，大概是这个原因没有用雪液透析。
现在住在国内(上海)医院，医生提供两种方案：
1，保守疗法，估计最后是体内循环负担增加，心脏病发。老人要求无痛苦，但是心脏
病发的时候肯定是痛苦的。有什么方法可以减轻到最低。另外从心脏功能衰竭到最后，
估计有多久。
2。腹膜透析，但是不知道老人是否能经历手术(恢复期要2-3星期)，加上腹膜透析国内
是要到医院作的，身体能否每星期3-4次的折腾。
老人平时比较谨慎的人，所以怕手术怕痛苦(倒不是怕生命提早结束)，所以偏向1)。但
是1)的最后结果会不会比手术还痛苦呢？ |

了泌尿科专家，建议做MAG-3 Renal Scan with Lasix。我上网查了一下感觉这个测试
挺不舒服的。想问一下有没有人的孩子做过，需要注意什么，如何帮助孩子配合测试。

aminoglycosides, beta-lactams, fluoroquinolone, bactrim, vanco, macrolide,
azoles, some antivirals
digoxin, dabigatran, lovenox
tramadol, gabapentin, pregabalin, morphine
venlafaxine
metoclopramide
以下是肾功能较严重不足就要注意避免的一些药物：
metformin
duloxetine
nitrofurantoin
NSAIDs
K-sparing diuretics
lithium
xarelto
bisphosphonate
CKD的病人如果有高血压或者糖尿病，应该使用ACEI/ARB，不过医生好像常常忽略~ 药
剂师在做chart review的时候不要忘记检查哦。同时还要记得monitor serum
creatinine and K+. |

如果CKD病人的renal function unstable，ACEI/ARB会被停掉，但病人肾功能恢复到

自己觉得还是renal好一点.neurology得看心电图,这样是不是会忙好多?而且RENAL的
节奏比较不那么快,因为病人在医院呆的时间比较长.
还有个问题也想大家出出主意.neurology先给的offer.结果我去面试renal的时候还碰到
neurology的manager,她问我到底接不接受她的offer.我面皮薄,不好意思说没想好,就说
会接受的.结果她当场给renal的经理打了电话,说我已经接受neuro的了,还说很喜欢我,
意思就是让renal的经理不要再面试了.但是最后还是去renal面试了,而且还拿到了
offer.
现在不我知道该怎么办好.我比较喜欢neurology的经理,但是两个unit比还是喜欢renal.
现在的问题是怎么和neurolog的经理解释一下(因为我说过会接受她的offer).更糟糕的
是这两个科室是紧挨着的,3楼一共就这两个科室.以后上班肯定经常碰到她.我想给她写
个email说我不能接受offer了,但是不知道怎么写.该怎么说好呢?请大家给点主意.谢

发帖数: 44 | 40 今天得知了step3的成绩，210+，心里的石头终于落下。终于轮到我给USMLE说再见了。
回顾从step1自己闭门造车考了只带一个9的分数， step2开始参加小组学习，做了所有
能做的题只为拿个99，虽胸有成竹且考试感觉很好，最终但又得到一个9的分数。虽然
这次成绩不高，依然很高兴，毕竟不用发愁如果没过的话住院医开始后没有时间复习该
咋办。过去的3年中得到太多人的帮助 (此处略去人名20个)，绝大多数甚至从未谋面。
今天发贴也是为了回报我们这个社区吧。
自我介绍：毕业>15年，220+/220+/1st/210+；
复习时间：6月，头4个月每天约2-3小时，周末由于陪孩子们玩（前两年欠的太多，你
懂得）及做点家务（再不做但心被老婆炒掉），反而复习不了多少；最后2个月每天4小
时，鼓励自己考了这个2年内再不用受这个罪。
复习资料：随大流，MTB 3, UW CCS, UW step3 Qbank, UW step2 Qbank;
模拟：考前2月，NBME 1 190+, 考前1周，NBME 2 220+;

HOWEVER, the most accurate way to predict renal function is 24 hours urine
output. A snapshot SCr and BUN may not reflect renal function unless we know
the baseline SCr. For example, if the pt's baseline Scr is 0.7. SCr of 1 is
still within the normal range but the pt may experience AKI. An absolute
SCr value usually doesn't give much information in the renal function. That'

, renal calcs, left renal mass and bladder mass. Good tech may see the left
adrenal lesion but most won't. Then you will still order a CT or MR for
further delineation. Ultrasound doesn't have radiation, but not as accurate
. However, you can see the ureteral jets to the bladder on real time to
evaluate obstruction, which u can't see with CTs

level <500 ng/mL is sufficient to exclude PE in patients with a LOW or
MODERATE pretest probability of PE. But a positive D-dimer doesn't mean
anything. In this case, severe liver disease (decreased clearance) and
infection/inflammation could cause the abnormal elevation of D-dimer.

院驗尿無血，照X光，結果說左側腎門有9.9mm的鈣化點，懷疑不確定是結石還是血管瘤
。想聽聽你的意見。下部應該如何檢查確證。因為在這邊全部自費，所以，希望有的放

IMPRESSION: A 9.9 mm calcification in the region of the left renal hilum
suspicious for a renal artery aneurysm. CT would be necessary to distinguish
between aneurysm and renal calculus.

any history of STD? like gonorrhea? male or female? how old?
if there is blood clot, this is not any pre-renal or renal problem (like
glomerulonephritis or hemolysis), most likely this is post-renals, then it
is nephrolithiasis.
PCP should prescibe CBC, UA, with or without urine culture. |

心里想要的答案
诊断的英文是什么，如果是acute kidney injury那么是无可厚非的，如果是renal
failure那算是灰区了，renal failure并没有特别严格的定义，曾经这种情况确实可以
叫acute renal failure，但是现在概念有变化
不论盖什么帽子，诊断都是人为定义的。
医生怎么和家长说的？肾脏衰竭了小孩没救了？我觉得从你描述的故事看医生不太可能
会这样交代病情
好吧，
再讲一次：
1）小孩的病史：
长期的肠胃消化疾病，多次出现腹泻。出现过elevated creatinine, 结果证明是因为
腹泻导致脱水引起的，之后补水之后creatinine 很快恢复正常了。
2）ER经历
最近一次血检发现elevated creatinine,小孩已经腹泻几天。父母送ER观察和复查已经
很快下降了
被ER医生判断为“稳定”之后出院回家休息。
3）与坏医生的经历
在接下来的普通门诊的follow up 竟然被另外一个医生判断为“肾衰竭”，根据已有的

我父亲陪我妈来看黑色素瘤，可能自己也长期劳累过度；上上周二出现无痛全程血尿，
上上周三我们看了社区医院urgent care,np只做了尿检。安排上周三b超和周五见医生
。医生觉得b超和尿检不说明肾上有肿瘤，可是也无意解释全程血尿一般说明是肾上问
题。 urgent care医生约了泌尿科医生，但遥遥无期。也不允许我们做ct 检查。请求
专业人士给予指点。
1）b超中说cyst是囊肿还是肿瘤？“3.6cm cyst is noted in the lower pole. 9mm
nonobstructing calcification is noted in the mid to lower pole.” 3.6cm和钙
化是不是说明时肿瘤？
2）如果有肾肿瘤，b超是否可以发现？还是必须做增强ct？
3）如果是肾癌，这种情况是不是应该立刻回国手术？
家里悲剧一个接着一个，有点举足无措了。谢谢大家。。。
