药房买， 其他要医生处方。 看报告crp和esr有些高，显示可能有活动。控制病情的药
DMARD要看病人的情况来决定2联还是加生物制剂。 病人不在美国，医生不会给开药的
，因为免疫抑制剂有非常大的副作用。
另外，类风湿关节炎现在强调早期用DMARD。如果不上，2年内手指erosion的机会非常
大。 建议在国内治疗。因为国内很多医院有风湿科，可以看类风湿关节炎， 比如上海
仁济，北京协和。 |

国内朋友两岁的孩子不幸得了这个病，非常着急，希望能得到些建议。
孩子9个月大时就出现了关节肿胀，由于没意识到问题严重，11个月时才开始入院，现
在一年多了。
目前的症状是多个关节出现肿大，不发病时可以走路，但情况非常依赖药物（Naproxen
），不吃药的话睡觉都会蜷成一团，十分可怜；总吃药的话，孩子很容易感冒发烧，有
时高烧不退，非常危险。
早些时候用过抗风湿药副以少量激素， 效果差，副作用大，才改用生物单抗类药的。
想问问这种病可以长期用药吗？有没有比较好的治疗手段？国外尤其在美国市场有没有
好点的药，能不能直接买到？

Match的结果终于尘埃落定，周1等电邮的时候非常紧张，因为成败就在此了，虽然一直
告诉自己，今年只是热热身，成败都可以接受，但到真的揭晓的时候，还是很紧张，毕
竟一旦卷入其中就身不由己，何况自己也还是有所付出。到周4等match program的时候
虽然也有点紧张，但程度已经大大减少，1个原因是毕竟match上才是头等大事，去哪里
尚在其次，还有1个原因是心理还是有所准备了，虽然希望留在home school，但直觉告
诉我恐怕是非常困难了，在我rank list上第二位的program应该就是我要去的地方，果
不其然。经过几天的等待，发现果然是要搬家了。

如果是arthritis，要想一下双方父母家族病史里是否有arthritis的先例。另外有些特
定的基因marker会导致arthritis，一般医生可以order blood test来查是否携带特殊
的基因marker。
如果是因为细菌感染引起的关节发炎，的确就是用抗生素来治疗了。但为什么细菌会感

风湿性关节炎，类风湿性关节炎。百度通通翻译成rheumatoid arthritis

类风湿给的解释似乎是真正的rheumatoid arthritis,而风湿性关节炎虽然也
叫rheumatoid arthritis, 但是居然撤上了中医理念，还联系上了风湿热
（rheumatic fever),会造成心脏瓣膜的损伤。这可是比rheumatoid
arthritis还要罕见的病啊，至少在美国是这样。
这个问题的根源在于将rheumatic这个词翻译成了风湿。西医里面的rheumatic
指的是免疫类疾病，和中文字面上的风湿没有任何关系。
其实在我看，国内大多数人的理解是只要是慢性关节炎，尤其是阴雨天更严重的关节
痛，都是风湿性关节炎。这个是严重的误导。关节病的病因不下几十种，多数人都有
阴雨天更严重的感受，于是风湿性关节炎成了最常见的关节炎了。其实大多数人所谓

回顾从step1自己闭门造车考了只带一个9的分数， step2开始参加小组学习，做了所有
能做的题只为拿个99，虽胸有成竹且考试感觉很好，最终但又得到一个9的分数。虽然
这次成绩不高，依然很高兴，毕竟不用发愁如果没过的话住院医开始后没有时间复习该
咋办。过去的3年中得到太多人的帮助 (此处略去人名20个)，绝大多数甚至从未谋面。
今天发贴也是为了回报我们这个社区吧。
自我介绍：毕业>15年，220+/220+/1st/210+；
复习时间：6月，头4个月每天约2-3小时，周末由于陪孩子们玩（前两年欠的太多，你
懂得）及做点家务（再不做但心被老婆炒掉），反而复习不了多少；最后2个月每天4小
时，鼓励自己考了这个2年内再不用受这个罪。

if by 类风湿 you mean rheumatoid arthritis, sorry to be cynical but you have
to make sure the doctors get the right diagnosis, for instance, if you
check rheumatoid factor, it's probably positive in lots of females, and
though rheumatoid arthritis could happen in feet, but it's not very typical,
if she indeed does have rheumatoid arthritis, it is a chronic disease that
cannot be cured, and she would be at risk for significant functional
impairment and loss of independence, as patient with RA has s |

different in patient with and without Sjogren's syndrome. But it is clear
that Sjogren's patient has a high risk for rheumatoid arthritis. My
understanding is that the treatment will be the same on the arthritis part
regardless of Sjogren. |

章很麻烦，耽误了2个多月，材料才得以从学校寄出。个人觉得申请算是比较顺利。把
我当时准备的情况跟大家说一下也许对将要申请的人有一点帮助。
我个人从这里寄出去的材料包括：
1.国内护士执照的公正，在这里网上找的公证处，在纽约，叫最佳公正，电话
7183212810
2.指纹卡，就在我住的地方的警察局办的，free
3.handbook上的申请表，贴一张照片在右下角（handbook链接：http://www.rn.ca.gov/pdfs/applicants/exam-app.pdf）
这一页有一项是填社会安全号，我当时没有就空着的，没有关系。
4.两张支票，一张办指纹的($51)，一张申请的($75)
5.写了一封信，表示感谢，一张纸，很客气的几句话
从学校寄出的材料：
1.中英文成绩单（学校有，我来之前到学校教务处办好的）
