两页的话，就是最流行的国内公证处现在的4页
也就是你的2，3加上4中文5英文（certified by）
不需要1其实我觉着连4都不需要，只需要一页英文的下面的内容即可
当然国内开的4页证明是一一对应的，格式规整好看，官老爷最喜欢
你的情况按照uscis找个第三方签字就是certified by
注意第三方不能跟你有很近的关系，所以找个朋友或者同事即可
uscis官网解释的很清楚了
