今天洗澡的时候发现手腕上有个黑色的斑点，大概是一个月前发现的，今天好像比上次
大了点，而且以中间的一块斑点为中心，两边有两个小点的。不知道有没可能是
melanoma。

will be very limited things you can do. Survival rate for advanced stage
melanoma is really low. One good thing is, though, melanoma, is one of only
two (the other is kidney cancer) that does some time will have spontaneus
recovery for unknown reason, maybe due to immune attack on melonama cells.
But the chance for that to happen is of course extremely low. |

得可能有些难听，但我说的是一个事实）。不过肿瘤骨转移需要止痛，这个是医院的常
规，就不说了。
如果病人在美国，还有希望的。可以试ipilimumab，也就是Yervoy的off-label use。
这是抗CTLA-4抗体，FDA批准用于晚期melanoma。它的原理是改变免疫抑制状态以增强
免疫反应，因此没有理由说Yervoy不能用于其他肿瘤。事实是，Yervoy对晚期melanoma
病人15-20%有效。对肾细胞癌（RCC)也有相似的效果。不过这个东西有些可怕，一是副
作用大，二是贵的很。
另外多说两句关于肿瘤治疗的话。看到板上说到肿瘤治疗大都是一片悲观，有回到了至
少二十年前感觉。事实上，肿瘤治疗在近二十年，或者说是近十年，在免疫治疗和
signal blocakde两个方面，可以说是变化巨大（在美国，不是说的中国）。晚期病人
的survival rate可以说是直线上升。具体大家可以自己读。问题也是多多，主要是
