and subsequently inflamation-induced damage, anything could happen dependent
on where the damage is. Based on ur early description, I think there is
at least a component of infection, which may be the sole cause or at least
the inducer if the girl does have a genetic lesion. For now, to prevent the
brain damage is more important than finding the cause. But I am not sure

然感染后机体清除病毒的过程中起关键作用，而不依赖于抗体的产生（时间上也来不及
）。
慢性感染者体内当然可以检测到HBV specific CTL，但是由于肝脏整体上的免疫抑制环
境以及病毒诱导的免疫耐受效应，使得存在的CTL本身不同程度的impaired/exhausted
，结果就是既不能完全耐受又无法彻底清除病毒从而陷入了拉锯战，使得肝脏长期处在
一种inflamation的状态下，这是目前慢性乙肝致癌的最主要机制之一。
核苷类药物确实可以部分恢复患者的细胞免疫，但是由于短期内无法显著降低HBsAg载
量，也就无法彻底解决免疫耐受的问题，本质上也就无法彻底恢复T细胞功能，自然就
不能达到清除病毒治愈HBV的理想终点。但是，长期的核苷药物治疗（有效控制病毒复
制6年以上）还是有一定比例的治愈率，HBsAg-HBsAb血清学转换的比例大约有12%左右
，当然前提是不能出现耐药，而拉米夫定连续治疗5年80%以后都会出现耐药的。

cirrhosis - HCC 可以算是HBV感染导致肝癌的标准模型，只不过这个过程很漫长，往
往要20-30年，在乙肝患者体内，免疫系统对被感染肝细胞的不完全杀伤始终伴随着肝
细胞的“增殖再生-重新感染-继续杀伤”这样的循环，经年日久，最终发展为肝硬化乃
至肝癌。
理论上，如果掐断了新生肝细胞被重新感染这一环节，reservoir逐步消除，cccDNA在
有限的细胞分裂增殖中迅速稀释衰减直至被清除，就有可能实现慢性乙肝的“治愈”，
这种治疗策略，相比较慢性乙肝患者本身，并不会提高肝癌的发病率。
当然，对于已经存在纤维化/硬化的患者，进一步激活肝细胞再生，自然有可能火上浇
油，所谓个体化医疗，用处就体现在这儿了。 |

when I read the HESI book about diverticulitis.
Diverticulitis is the inflamation of diverticulosis. Usual symp involves abd
pain and GI bleeding. Complication is perforation. I had one pt who had the
dx of diverticulitis, and he's really c/o pain while I was his SN. I can
tell the pt was in moderate to severe pain, even tho all he did was to lie

levels of liver funtion reflected as varying levels of ALT/AST etc. which
is also likely to be accompanied by some inflammatory markers. Under anti-
inflamation drugs, the side effect especailly in the initial response of
drug in such patients might not reach a statistical level due to the large
variance of individuls. As such, the drug toxical effect can be masked, and
that is why it might be a smart/sneeky trial.

看我的文章没有。
Rodimus 根本拿不出任何版规，还说我的文章他不屑一看，居然发帖说我是受虐狂。
我发帖反驳说他动不动冒充全版不欢迎新人，还无根据威胁封人，是虐待狂。
Rodimus恼羞成怒，把我的帖子和后面更贴的对话全删了。
闹了半天是个不看贴就摆架子，威胁人，骂人，删帖的版主。混mit这么多年，还真遇

they still have their gallbladder, but clearly it was taken out , and some
