还是不正常。

Total iron binding capacity: Low 36umol/L
Alanine Aminotransferase: High 87U/L
Immunoglobulin G, Serum: High 17.7
ANA: Positive
Speckled pattern 1:160
Anti Smooth Muscle Abs: Negative
Hepatitis A, B and C: Negative
Lupus Anticoagulant: Normal
其他常规项目正常。Ultra sound显示a little bit fat liver.
怀疑hemochromatosis，转往hematologist,要求检查肝脏MRI，看是否含铁。

transferrin rules out most of the hemochromatosis type except 4A which is
also called ferroportin disease. It is very rare and I highly suspect you
have it.
I agree with A++, you should see a liver specialist. NASH which is also
called Steatohepatitis can cause elevation of ferritin. If you are a guy and

1.ana 如果是homogenous pattern,对于诊断lupus来说比较specific,speckled
pattern是最常见的,也是对不specific的,很多其他结绨组织疾病都可能会有阳性表现.
2.ferritin不仅代表体内铁,也是常见的感染蛋白,如果体内有炎症,不管是infection
or inflammation,都会出现ferritin增高.
3.楼主贫血吗?其他检查都正常?CBCD? |

a liver specialist. She just ordered a bunch of tests that she thought to
be helpful for the liver specialist. So when the liver specialist saw my

ferritin is also a marker for chronic inflammation. But with chronic
inflammation, you will see other abnormalities, like anemia, low albumin, high
platelet count, high ESR, CRP, and LDH. With a normal albumin and PLT, it's
unlikely you have chronic inflammation going on. |

用sirna敲除后，发现tfr增加，ferritin减少。
这个可以解释fes减少导致irp1基因aconitase活性降低
irp1 变成ire binding模式，结合ferritin mrna 5‘ire
抑制翻译，或者结合tfr的3’，稳定mrna。
但是我overexpress这个蛋白，发现tfr减少，ferritin也减少，
这个怎么解释？这个蛋白上有个假定的铁结合位点，难道是铁被它搞定了？ |

症状描述：12月2号开始感觉身体不适，发低烧（37度1-2），喉咙痛，胸腔难受，全身
乏力，12月底出去游玩了一趟，受了凉，开始鼻塞，症状有所加重。后来鼻塞症状消失
，但低烧持续不退，体温略有上升，胸腔难受，疲劳乏力。3个月以来，一直在发低烧
（37度3-4），皮肤过敏发红，胸腔难受，心跳过快。
就医过程：12月初开始看家庭医生，她认为不算发烧不开药。12月中耳鼻喉科专科医生
认为有鼻窦炎，但只开了泰诺和nasal spray。1月底，发烧持续，而且上升到37度3-4
，血检中的一项：类风湿因子RHEUMATOID FACTOR过高，家庭医生推荐到了风湿免疫科
的专科医生处。这位专科医生的诊断结果是：未知原因弥漫性结缔组织疾病Unknown
Diffuse Connective Tissue Disease。并说，从症状来看，怀疑是红斑狼疮。她开了

看化验报告，虽然Total iron-binding capacity 较低，但我的铁蛋白（Ferritin）超
标的高，可能是吃prenatal维他命的关系。Transferrin saturation 也高过正常。
总体上好像不缺铁。
下次见ob还有两周多，现在非常confuse，到底缺不缺铁，怕吃铁片或者继续吃维他命
也只会继续提高Ferritin。
问一下有经验的mm，缺铁到底是指哪个指标？ |

地磁场。室温下一个铁原子的磁取向需要大约100T=1,000,000 G 的磁场， 而地磁不到
1G. 如果MagR 20个铁原子的取向互相独体(顺磁)，磁化就很难。如果这20个铁原子取
向一致（永磁），看作是永久磁铁，取向磁场也得要5T. 真实的情况只能在这两种极端
情况之间。所以我怀疑他们看到的~50%取向可能和样品制备有关。另外他们的数据分析
其实应该基于相对地磁场取向角度分布。这样的话直接可以和热力学预言的分布比较。
上面这些分析和他们观察到的MagR的晶体沿磁场方向取向不矛盾。他们的晶体实验实在
光学显微镜下作的，所以是个微米尺度的物质。里面可以包含很多MagR蛋白，所以磁化
可以强几个量级。从文章里不清楚这个取向实验中磁场强度多大。另外，MagR晶体能随
磁场转动其实也不证明MagR是永磁体。只要磁化是各项异的，就能转动。不过他们的磁
化测量表明MagR/Cry晶体具有铁磁性。虽然机制不清楚，但是结果很有趣。这个实验证

nothing about the iron.
So I think the iron, TIBC, %saturation, ferritin should be normal.
But later Goljan said the alpha/belta thalassemia have normal iron, High
TIBC, high %saturation, high ferritin.
This really makes me feel confusing.
Can anybody explain it, please?

是蛋白质，所以你如果只是在硅片什么的上面做少量的管子可以；大量的就贵了去了。

容易吸收的是血中的蒲林铁。
铁在体内也可以回收利用，蛋白质ferritin是存铁用的。
如果这个ferritin的生成利用有问题，吃再多的蒲林也没有用。
氧多了动中毒。现在不是提倡antioxidants么。用不完的氧自由基，
破坏能力很厉害。
简单化学成分永远不是问题关键。 |

心内膜炎，我爸他自己翻医书对照症状，怀疑过。但是，说给医生听，医生不相信，他
们的观点是，如果是心内膜炎，会让人很不舒服，尤其是会让人产生“高热”，我爸的
症状都算比较轻微，只是全身不舒服而已。而且也做过“普通心超”，医生诊断不是心
内膜炎。
请问“Ferritin（铁蛋白）”如何检测呢？国内一般的县城医院有条件做这样的检测吗
？检测这个为了证明是属于什么样的病呢？能不能请你解释的更详细点？这样我好让我
爸去针对性的去做检测。

can get MRI and bone scan, and if concern enough, get bone biopsy to
establish diagnosis osteomyelitis,
reason i thought of endocarditis is because of after dental procedure and
fever, TEE is not that difficultof test to do, all cultures are negative is
probably because it's partially treated,
reason i said to check ferritin level is quite far-reached, since he
mentioned some joint problems, rash(if i understand it correctly), sore
throat, fever, yet all cultures so far negative, so i'm reachin |

phase reactant, like a poor man's sed rate or c-reactive protein, i don't
think it is used as a tumor marker for follow up, doesn't mean cancer is
spread or recurred or anything |

this could be very nonspecific, basically says nothing.

iron profile, ferritin level
我感觉不太像缺铁性贫血。因为
1）从报告来看，是小细胞贫血没错，但RDW正常，一般如果是缺铁性贫血，血细胞大大
小小，RDW会变大
2）血红蛋白13.2， 正常情况下红细胞是13.2x0.3=3.9,但你的是5.8，所以有点像地中
海贫血的样子， 如果你有以前的验血报告可以比较一下。地中海贫血应该从小到大都
是如此。
3）能引起小细胞贫血的，主要有4类，除了缺铁性贫血，地中海贫血，还可以是铅中毒
（小孩子多见)，或慢性疾病引起的
4）如果是女性，我就会怀疑是月经丢失引起的。但像你这样的成年男性，如果是缺铁
性贫血，除了摄入不足（一般正常饮食是不会缺乏的，除非你是vegan, 有十二指肠的
问题，那里是vit C帮助吸收铁的地方，或其他小肠的问题，慢性腹泻，比如celiac
sprue），那就是丢失过多，比如从消化道，或尿道（如结石）。 可以查个尿常规，看
看有没有血细胞，做个大便潜血试验。
