It is hard to pinpoint at this moment for me w/o knowing any lab values.
This is likely a systemic process with multiple organ system involvement: GI
, vascular and possible renal and heme. I would guess diagnosis is
eventually made by a pivotal biopsy.

我们shareholder开会， 谈到了一个重要的部分， 就是在未来三年内， 如果诊所被发
现任何fraud，所有的bonus医院都要收回， 并追究法律责任。
上个星期刚刚说这个事， 这个礼拜事情就来了：
我们的COO刚刚离职， 原因就是这次收购。结果她刚走， 就有人发现， 她不当使用诊
所的信用卡， 挪用了很多钱。 中午一个vascular surgeon就来找我， 要我跟他一起
采取行动， 给诊所施加压力， 把这个COO绳之以法。 道理很简单， 这个fraud可能会
影响整个收购计划。
我发了个email给president， 他很快给我一个回邮， 他是这么说的：
1. 我们已经注意到这事， COO表示合作， 签了个法律文件，同意归还所用款物。
2. 对于几个医生要求把COO交给警方， 这种做法是值得商榷的。 一个disgruntled的
前雇员什么事都干得出来， 因为她知道的事太多了。

经有聪明的人做过大量研究了，呵呵。利尿剂引起的高尿酸和通风的高尿酸的代谢机制
不同，风险也不同，利尿剂对心脏病有保护作用所以还是值得冒这个风险用的。
用ARB合用利尿剂能够降低尿酸水平，瑞典大个子2001年作的study，2006年咱们中国人
民又山寨了一把，不过只要换成中国人口就是一篇新文章了。

hospital. Last week, we had a STEMI pt with glucose level>550.(pt doesn't
have DM history).
It is common to see pt with DM has all kind of vascular disease.

1990, S/P TAH/BSO who presented with a rib fracture after bicycle accident.
Her chest x ray identified a round 2 cm nodule of right upper lobe

asymptomatic carotid stenosis, although some data support surgery over drug,
the benefit is small and depend on life expectancy and surgical skills.
For this patient, her syncope is not caused by carotid stenosis. I always
question cardiology why order carotid u/s for syncope? Vascular surgeon may
say, r/o bilateral carotid stenosis, give me a break.

involving the left circ. I am not an interventional cardiologist, but in
interventional radiology, one of the contraindications for vascular stents
placement is multifocal stenosis, which doesn't respond well with stents or
angioplasty. With three-vessel CAD, bypass is not a bad option. |

要说简单，Autonomic failure and volume depletion 是两个最主要的原因。尤其夏
天一天热，7个病人可能有4个由于各种原因脱水，出现症状，头晕，甚至急性肾损伤。
治疗，如果是Volume depletion， 楼上高年医生已经说得很好了，要补液，能口服
就口服，不能口服，能不能IV呢？+Vasopressor，最后再加小剂量激素
FLUDROCORTISONE。如果继发，治疗原发疾病。
要让不知轻重的intern往复杂说,所有影响CO（preload，afterload and
contractility），Systemic vascular resistance (vasoconstriction and

说一下CEA吧，做这个手术的医生什么科的都有，美国医生做了一段时间后就有一个手
术并发症的比例，如果超过5%就别做了。 应该承认，医生之间还是有差别的，可以很
大，比方说1%对4%， 后者是4倍，但总的来说还是很低，所以没听说什么医生为此失业
的。美国医生标准化的训练对缩小医生之间的差距很重要。
做手术多水平就高？ 我还真不认同这个观点，我做了这么多年的腰穿，穿不出来还是
穿不出来，每年都有那么几例。
我感觉，一个好的residency training加上1-2年的独立工作，他们的水平就没什么太
大差别了。当然不会做的做再多也没用，当年北京最好的神经外科医院做aneurysm，做
一个死一个，做了那么多例也没个长进，现在估计好多了，美国做aneurysm有几个做死
的？
至于天赋，我想就是1%和4%的差别，做CEA的医生我常用的有两个，一个是certified
vascular surgeon， 另一个是普通外科医生，我感觉后一个并发症更少。
有意思的是我常用一个骨科医生做颈椎手术，他是退役的NBA player， 没球打了就去

复杂，我希望讲清楚，谢谢你的耐心。
今年1月底，乳房剧痛 红肿，然后有肿块。很大。等约到医生做超声波就是2月中了，
但是当时肿块已经没有了。
等了1－2个月好像又有肿块，5月突然我觉得摸到很大一个（5cm 多）肿块，做了超声
波，医生 ""quite vascular, hypoechoic colletion, and more suspicious for
infection" STILL 建议biopsy。
biopsy pathology test 结果是 diagnosis is granulomatous mastitis.
granulation tissue with scattered granulomas that display central
neutrophili aggregates
然后医生让我等他自己好。
等到10月份还是没有消，我就要pcp给我看了specialist
她说很concern 让我做MRI ， MRI 显示enhancement 就做了2个地方的biopsy.

specialty选择还处于犹豫和懵懂状态。。。暂时对cardiology和radiology都有兴趣，
比较想做interventional的东西（喜欢动手但同时也做诊断。。），但是听说这两行的
Interventionalist之间抢生意很严重（还有vascular surgeon也参与竞争。。。）不
知道是不是属实？
目前在学校成绩还成（class ranking 99%)，估计step1也应该能拿个像样的分数。所
以现在在准备夏天的research,目前跟着一个cardiovascular radiologist在上选修课
和做research。不知道各位前辈有什么建议没有？关于如何选择specialty和如何为了
match进心仪的specialty尽早做准备？如有赐教不胜感激！ |

附件是主治大夫的reasoning(email回复，红字)， 他的水平我们还是很佩服的，因为
他已开始猜了个病就和我们宝宝症状非常一致，可惜后来发现不是；而且他不是代谢病
专家。
问题是坚持宝宝不吃长链脂肪酸的奶粉后，他奇迹般地改善了；完全出乎医生们的预料
。我感觉医生列的是常见的代谢病，而我们感觉宝宝吃长链脂肪酸似乎是慢性中毒，大
概需要一个月左右才能体现出来。因为过程很慢，我们怀疑很多方法查不出来。
我们最大的问题是如果是这种病怎么查？不能确认的话他一辈子吃饭都受影响，基本也
就很难出门时间长一点了。尤其是他的肝已经在要换的边缘。医生们现在改口说他的病
程都是偶然，可我们不能同意。为什么每次都是长链吃一段时间就得病，尤其是这次停
了鸡蛋后马上改善。。

attending，4000刀；另一个后来居上的哥们做rehab，400，000刀， 多两个零，呵呵
我查了一下本地医生，vascular surgeon第一名，所有的pain management都比PCP什么
的多几倍， 大家知道该怎么做了吧？ |
