我个人的经历使我认为：
如果你有点野心想发展，如果你想舒舒服服没有下了班还提心吊胆、在冰冻时惊心
肉跳与外科医生打交到的经历，那么，做两个FELLOW绝对是值得的退一步进两步的
投资！具体如何组合哪两个专业亚分科，那得具体到个人情况和条件考虑。到时，
可以来咨询我，其实老刀会上和目前，常有CMG想搞病理的和住院来问我这个问题和
看法。
我今天能舒舒服服工作，没有STRESS，能承担GU、BREAST、BONE/SOFT TISSUE、
THORACIC、
甚至客串头颈和GI病理、冰冻切片、TRANSPLANT等亚专业、能下了班尽兴玩乐，与
我刻苦做了TOP单位的两个FELLOW训练经历有绝对的相关性。
一般而言比较好或实惠的组合是：
1。surgical path + cyto path
2. surgical + any subspecialty (GU,GI, GYN, ST/Bone, Peds, etc)
3. surgi |

for fellowship training. Since we all have strong research background, I
always say that I want to do surgical fellowship. If you want to be a good (
market value)AP/CP pathologist, surgical path is a must. Please correct me

这种有的写明是research/investigator track, 大部分没有写. 没有写的到底是不是也
是以科研为主呢?
3. 假设match 进去之后，有没有可能在同program 不同track 之间换呢? 比如从 AP

your life on. Make sure that is what you truly love. Try to get first hand
experience about what an surgical intern / ent intern'life is. Go to the
morning rounds, didactic, MM, and OR. Witness how they work everyday. What
i can tell u is that, everyone works VERY hard, long hours, no complaints
and beingpositive. Forget about your experience. That wont help. Their goal

就在此环境中，几位既往study partner组了个pathology iv group。宗旨：此群不练
习iv，只用于定期交流各pg面试情况。
Material and Method: skype group, 不定期组会。
Result: 小组共15名成员，8名match path，4名match IM。
Conclusion: 虽然当年麦地求助无门，却发现4～7年前各位前辈热心的面经分享，受益
无穷，给面试增添不少信心。遂鼓动小组全体成员，独乐乐不如众乐乐，为2015申请
path童鞋鼓鼓气。

转眼一年的intern就要结束了，对于病理从0开始，总算是有了初步的认识。写下来跟
大家分享一下。
我是个铁杆病理，当初申请只申请了这个，最主要原因是看中了不要值夜班，工作时间
相对短，事实证明我是对的也是错的。病理工作时间虽然没有内科长，体现在不用那么
早到，在surgical path我的工作时间平均是早8点到晚7点，偶尔到9点。但是需要大量
阅读，不象内科工作可以学到大部分东西，病理大部分的东西是从书本获得的，自己经
手的case实际非常有限。所以每天保证一定时间的阅读非常重要。
那么作为intern你应该做些什么呢？每个program 的schedule都会不同，但大同小异，
我拣主要的说。
Surgical path
Cover frozen之前，把OR的schedule打印出来看一下，估摸哪些手术会送冰冻，最早的
时候我甚至把可疑的case病史都查一遍copy到word里打印个大概。这样你去取冰冻的时
候就知道外科医生大概要看的是什么。后来我觉得这个有点费时，就不看病史了。因为
冰冻无非是：看边缘是不是切干净了，看良恶性，staging，看组织，... 阅读全帖 |

你，什么地方有工作。推荐~~~~~~ 大多数工作是靠推荐人推的，要和attending搞好关
系。Attending介绍，是最牢靠的招人的方法。在住院医的时候，要有特别帮你推荐工
作的attending。拿到好推荐信。
找工作 number1 要素，不是你的knowledge，第一是teamwork，第二personality，第
三是你的knowledge。你能不能跟人搞好关系最重要。 一定要有advocate！ 有10个
attending都关系不错，不如和一两个有力挺的attending关系很好。
22） 为什么美国住院医不热衷申请MD Anderson和 Sloan Cancer Center？
如果你在大program出来的，没必要要去。
如果你program非常小，可以考虑去， 特别是programs 没有 癌症中心的。
因为这两个programs需要cutting。很少的综合医院的fellowship要cutting。像我当年

果我就疏忽没有去保险公司确认。
医生在外面的surgical center给我做了两次处置,很简单，10分钟就结束了。昨天接到
账单，surgical center要收35000一次，医生要收12000，总共47000, 两次就是94000
，可是保险公司只付8000，因为是out-of-network！当时整个人都蒙掉了。昨天在网上
查了一下，这种处置一般的费用是1500，最贵的不过3000一次，而且是包括医生和手术
中心的全部价格，这个医生和手术中心charge了差不多三十倍的费用。感觉自己就是中
了一个大的圈套。现在我能想到的是请一个health advocate帮我review和去negotiate。
请问各位医生，这种情况我还有救么？就只能任凭他们宰割么？现在来了这个打击身体
更加不好，状况非常糟糕，现在我是单身一个人，也没有能帮忙的亲友。衷心恳请大家
帮忙看一下应该怎么去处理这件事情。 |

医生的staff和我说他是in network的医生，处置费也会被保险cover，当时急着看病没
想到那么多，就相信了。这个surgical center就在医生的诊所的同一栋小楼里，我根
本不懂这些细节，以为就是医生自己的处置室。这个center和医生有经济关系的，我记
得签了一个表专门指出understand doctor XXX has financial interest in the
surgical center。当时还有些奇怪，心想这不是医生的处置室么，为什么还要强调这
个，但是因为疾病困扰心情低落没有想太多。 |

我在MEDICAL FLOOR作了一年半，刚去SURGICAL UNIT(GYNE-ONCOLOGY),病人普篇
INDEPENDENT，体力劳动没那磨重，但这个UNIT护士绝大部分是白人，有白人慢悠悠做
事的作分，有时稍范错误她们马上填上INCIDENT REPORT，这是我不喜欢的。另外，昨
天我范了一错误，我不知此错误是否大到足以令我辞职。
昨天一病人术后来我们UNIT，血压很低，我报告CHARGE NURSE， RESIDENT来了，医嘱
LACTATED RINGER 和NS，分别RUN BY GRAVITY， 病人有两个PERIPHERAL LINE，其中一
个LINE，我忘了PRIME （因SURGICAL UNIT 的TUBE与MEDICALFLOOR的不一样）我CLAMP
了此LINE BY ACCIDENT，NS不能流动，夜班护士发现报告了MANAGER。MANAGER找我谈话
了，说真话，我很尴尬的。
我知道是我错了，但人人都会范错误。我想问的是我这个错误（忘了PRIME PERIPHERAL
LINE）是否大到令我辞职。我不愿意在 |

发帖数: 1049 | 38 我也觉得科室和医院比较重要.我的俩家医院护患比一个是1:4一个是1:5-6.相比较还是
1:4的累.因为要answer call bell,CNA也懒.另外一家,是surgical floor,分工明确,
虽然病人多,但是有秘书answer call bell,然后她们根据情况找CNA或是护士.还有
vocera帮我找人很方便,挂脖子上摁一下按钮就可以和别人对话.分工很明确,手术后
病人上来,CAN不用我说就主动check VS q 1hr X 4.写在纸上并留在屋里.所有的drain
包括surgical drain都由她们来empty.有时候病人家属出来找我帮忙病人喂饭上厕所
什么的,我就让她回去ring the call bell.这样secretary就会叫CNA去干.还有urine,
stool for lab,只要告诉她们就行了.这在第一家医院是根本办不到的.我觉得主要
还是看医院,规定不严就没办法.
我是属于不爱讨好人的,让我哄着CNA我是办不到的,只能告诉自己不要期望太高.

是BSN吗？BSN没有听说拿不到面试的，我们班上稍微申请一下就可以拿八九个面试的。
毕业前80%的人都至少有一个offer。
1. 去医院去一家一家的问，这方法过时了。我试过，没有用。都被告知要在网络申请
。HR初筛，然后才能拿到面试。
2. “是不是一毕业就在residential unit工作的经历是否会影响到找surgical的工作
,以后想找surgical的就更难找？” --- NO. 我就见过在nursing home工作了几个月后
，跳槽去医院ICU的。
你要说说你是不是BSN, GPA, 还有你们班上同期毕业生就业情况；我们才知道怎么跟你
出主意。
比如，大医院初筛，先找BSN，GPA 3.5以上的；然后再3.0以上的，然后ASN。这样来挑
人面试。 |
