however she was super researcher working w/ her boss--Surg Chair, and had
many high tier journal pub, R01 or other awards/fundings, she did super
USMLE, her boss wanted her to do surg, but she (old CMG mom w/ 2-baby!)
wanted rad, so, her boss very strongly spt her and provided 1-yr surg intern for her, now, I thought she might graduate to practice.......

朋友的微博： 今天才知道原来二尖瓣返流还是狭窄引起得房颤不是通过心房得异常排
列得心肌而是因为心房体积得变化引起肺静脉周围电路传导的紊乱，这也是为什么房颤
射频在肺静脉周围一圈操作的原因。（但是我没查文献证实）。我就说嘛，AS 是心房
肥大，AR是心房变薄，用心房肌的多寡解释不了房颤的机制。

Q：为什么menstrual cycle在FSH 增多后雌激素增多，然后就有LH surge 呢？ 为什么
啊？为什么没有FSH surge，是因为产生inhibin吗？
A：排卵前一周estrogen↑，同时FSH↓（E选择性对FSH分泌抑制；颗粒细胞产生的卵泡
抑制素也对FSH分泌抑制。FSH暂时处于低水平，但E反而↑，因为E加强内膜细胞的分化
与生长，使LH受体数↑，加强合成雄激素及转变为雌激素的过程）。排卵前一天，E↑

发帖数: 8 | 4 就这么忐忑不安的开学了。上学期开始了Med-surg 和Peds的实习，每个星期从周一到
周五，每天都是8个小时以上不是上课就是实习。与此同时，我的早孕反应也越来越厉
害，吃不下东西，连水都喝不了，还恶心呕吐，体重开始往下掉。那时在开车去上学上
班的路上，总是会放纵自己使劲哭一下，不是顾影自怜，而是感觉大哭一场难受程度就
稍微减轻了一些。
学习方面，除了每星期16小时的实习，还有med/surg和peds的理论课，实验课，
psychosocial theory, 和一门leadership(这个只有1个credit). 平时的大考小考，加
上论文演讲，再加上实习报告，我感觉体力也渐渐支持不住了。
期中的时候，我的Med/surg理论课两次考试都是C,那是我各方面都觉得顶不住了的阶段
。身体盯不住了了，学习也盯不住了，精神上压力也大得不得了，我觉得自己就要完了

发帖数: 8 | 5 与此同时，我的同学们都纷纷公开表示这学期太累了，受不了了。我觉得很奇怪，也有
一点relieved. 我一直以为只有自己有这种感觉，没想到大家都有，而且比我表达得更
外露，更彻底。
除了med/surg的理论考试外，老实讲，我其他的成绩都还是不错的。带实习的老师们都
对我评价很好，我的care plan 反复被老师当成sample给大家复印，psychosocial 的
大小paper也都获得满意的评语和分数。我后来觉得可能是我学med/surg的方法不太对
头，也不太适应这种考试的题型。
以前学药理的时候，虽然也是选择题，但是每道题都有泾渭分明的对错答案，没有灰色
地带。而学病理生理的时候，我们选择题占很少比重，主要是问答和分析题，所以无论
我写的答案是什么，我总是有机会写下我的rationale来支持我的结论，所以成绩总还
可以。但med/surg这种Nclex的题型，总是问priority, what do you do first, 有时
候我就容易think too much, 反而miss the point.

TELE.从我现有的经验看,我发现自己不喜欢伺候老人,因为耗体力太大.所以我说这两个我
都不选.如果有机会,我会改换一个完全不同的领域.比如MOTHER/INFANT, OB/GYN, PEDS
以前我不喜欢,现在觉得可以试试.NICU太CRITICAL,淘汰.不过我还是喜欢HOSPITAL
SETTING.因为时间比较灵活.这也是我现在还在干MED-SURG的原因.我经常CALL OUT,
因为有孩子要照顾(孩子生病),有时候我自己不舒服.然后我自己决定要不要补回来和
什么时候补回来.从来没人说我,因为科里太缺人了.大家都睁一只眼闭一只眼.象今天本来
是要上班的,女儿病了,老公一大早又有会.我就昨天打电话给STAFFING OFFICE,说我今
天不能去了,给我换到周六吧.人家就给我换到周六了.这是干MED-SURG的一大好处,永
远缺人,就算你们科不缺,总有缺人的科,她们可以把你FLOAT到缺人的地方.我估计我要
是在轻松不缺人的科,早就被赶走了.本来还考虑去CLINIC,后来放弃了, |

发帖数: 367 | 7 很理解LZ MM所说的。也来说说我曾经经历过的学习上的压力吧。记得刚读Nursing I
时，要学很多非常陌生的文化及法律上的东西，课后还要花很多时间练习一大堆的基本
技能，心理上的压力非常大；而且感觉自己的英语太差、对付不了Nursing课程，因而
好几次都想drop，还跟LD及几个同学、朋友提过，但他/她们都鼓励我说：你之前的先
修课都学得这么好，护理课也一定没问题的。在他/她们的鼓励下，我坚持了下来，而
且，竟然在学期总评时拿到了A；我的Med-Surg Tool也被老师作为模本供学生参考--这
已经是2008年春天的事情了，可是在一个半月前，却看到我的那个指导教师与另外一个
老师坐在一起讨论着我的Med-Surg Tool！
要知道，这个Med-Surg Tool也是当初把我逼得差点退学的其中一个原因。十几页纸的
病人Subjective and Objective Assessments啊，还不包括问题分析、病人基本病情的
概要分析报告，以及护理措施等等。要完成这个报告，Assessment 是第一步：除了要

From my point of view, tele is the worst unit in hospital.
some patients go to tele simply because there is no bed available in CCU/ICU.
ICU/CCU nurses only take 1-2 such patients while tele nurses have to take 4-
6 patients at same time.
81% Coded patient expired in my hospital in last 3 mnths. That means sick
patients in med/surg either die or go to other floor before coded. That is
why med/surg nurses are less stressful than tele.

在MED/SURG FLOOR， 也可能在心血管，肾科，骨科，神经实习，因为MED/SURG包括了
所有成人疾病，不分内外科的。学妇儿精神科，去相应科实习，一边学一边实习（这个
实习既不像国内实习，也不像国内见习，介于两者之间吧），最后会有个capstone，这
个就有点像国内的实习了，但不会是一年，不是天天去。 |

发帖数: 2269 | 11 5月初开始教的，不知不觉已经是12月份了，7个月了，时间过得太快了。
刚开始只教晚班的med-surg，还有每周两次中的一次实习。后来7月份白班的老师缺人
，我就放弃了实习兼带了白班的med-surg and pharmacology，10月份晚班的课结束，
原想可以只带白班了,结果新的晚班又开始了，我挑了skills lab。
orientation：lecture的orientation只有2天，实习也是2天。当时很着急，感觉压力
很大，知识上我到不怕，就是怎么教课，怎么表达。
当时对学生的期望值太高了，第一节课就发现他们学得a+p都忘光了，好些学生连Ph值
都不知道。我的第一章就是最难的一章----fluid and electrolytes。当时花了很多时
间备课。基本是上2小时课，另外pay2小时的prep。但是我几乎需要5小时来prep。所以
刚开始实际上是赔钱。总结的经验是：不能对这些学生期望太高，他们毕竟是高中毕业
或者GED。需要用很多生活中的例子来打比方给他们解释。
等到教白班时，已经是第二遍教med-surg了，就得心应手了，2小时的prep刚好用，稍

发帖数: 1637 | 12 恩，是应该边读nursing边去医院做CNA。光读4年书的BSN不容易一毕业就去医院做RN。
今年我所在的班上有37个BSN，我知道的，有一个男生毕业后找到医院的full time med
-surg floor（他读nursing的时候就没有做CNA而是一直在做一份跟nursing不相关的工
作），2个女生是“空降部队”有在医院connections也直接去医院med-surg。还有一个
女生自己找了大半年才去的医院做PRN med-surg RN.还有3-4个是读nursing的时候一直
在做CNA，毕业后就留在那个科室直接做RN。剩下的学生大多去了rehab centers，
nursing homes， home care。总之，形式很不好啊。
我们这边的医院大多要求新毕业的new graduate BSN要有一年的RN工作经验才有希望进
入医院做RN。真是自相矛盾，new graduate怎么会有一年的RN工作经验呢？说白了就是
有经验的老护士优先考虑。 |

大turbo的surge threshhold是一样的，前提是其他条件都一样，只比较turbo大小的区
别。这里其他条件包括，不仅限于：BOV (or DV) 开启阙值，wastegate关闭阙值，
turbo几何形状。那个工况图描述的是随着turbo rpm的增加compressor一侧的压力增加
，exhaust这一侧相应的就需要更大的压力或者说发动机转速来保证turbo正转而不是反
转，所以surge线就是斜向上的。从这个图怎么能看出大小turbo的差别呢？小turbo就
是大turbo的按比例缩小，两侧的压强差是一样的，wastegate关闭阙值也一样，大小
turbo同时启动。所以我还是前面那个观点，大小turbo转动惯量的差异是起决定性的。 |

我打算transfer 3个。
我做的是自然的，自测LH surge。总共就需要做两次u/s, 一次LH surge前，一次后。
后面那次主要是查lining等等。然后会做个blood test 查progesterone等等to
confirm ovulation did occur. 如果一切正常在LH surge后第五天transfer.
我打算用assisted hatching. |

。可是如果是
IUI或者IVF药物辅助的cycle，在hcg注射以后的24小时内用排卵试纸测的话应该出现什
么结果？
我自己想不太明白。一方面，我觉得hcg注射后24－36小时内排卵，这个提前的时间和
试纸能提前预测的
时间差不多，所以排卵试应该纸显示LH surge；可是另一方面，会不会因为Iui和ivf是
用hcg来促排
卵，所以实际上LH值并不会升高，和正常的cycle不太一样。
有经验的JMS帮忙解答一下吧。多谢了！
PS:我昨天晚上将近12点注射的，刚才9点半用试纸测排卵，没有发现LH surge。不知怎么解读是排了
还是没有。 |

A rebound in Japan’s exports in January failed to keep pace with growth in
imports, leaving a record 1.63 trillion yen ($17.4B) trade deficit for the
month.
The provisional data released Wednesday show exports for the world’s third-
biggest economy rose 6.4 percent to 4.8 trillion yen ($51.2 billion) in
January from a year earlier, the first year-on-year increase in eight months
, while imports jumped 7.3 percent to 6.43 trillion yen ($68.6 billion.)

starting today as the world’s second-largest economy promotes usage of its
currency in global trade and finance.
