发帖数: 378 | 8 近期病史：4月末出现过半边面瘫，3周后自动完全好了；7月第一周右脚踝微酸；第二
周变为两脚脚踝和膝盖都微酸，两脚脚后跟疼痛；第三周也就是这一周髋部和骶髂位置
开始剧痛，屁股和背部肌肉酸痛。小关节从没痛过。
这一周看医生，开始时怀疑会不会是强直性脊柱炎，查了HLA-B27为阴性，类风湿因子
阴性，C反应蛋白low，但Lyme Ab是high，2.43 （<0.91是negative，>1.09是positive
），Lyme Ab IgM也是high，1.54 （<0.91是negative，>1.09是positive），因此排除
强直性脊柱炎，认为是Lyme Disease引起的关节痛。开了三个星期的青霉素，每天口服
500mg*2，关节或者肌肉痛的时候另加服用motrin。
请教有经验的同学，强直性脊柱炎能完全排除吗？这lyme disease还需要进一步确诊吗
？是不是得培养病原体才能进一步确定？担心误诊然后错过治疗时机。
以前没查过lyme disease，也没印象被什么tick咬过，以前几年也没有过什么关节疼痛

lyme disease 的人,可真的倒了大霉了. 我到了美国好几年后才得的.而且得了也混然

00年夏天去郊外(sugar loaf)过美国国庆,在朋友家呆了三天,其间爬山游水狠狠地运动
了几天.回纽约后大约两周,右腿膝盖后凹进去那一块开始痒痒,接下去三天发烧,痒痒那

有一个构思，说我因为要做infectious disease （HIV）的专科，所以我现在要申请
internal medicine，这样以后才能做infectious disease的fellowship。而且
infectious disease需要很强的internal medicine的底子才能做好。
不知道这样子能不能说的通，还是这个故事根本就没有可行性？
想问问板上有经验的同学，internal medicine的住院到最后有多少做了infectious
disease啊？

对于药物治疗，优点是有一小部分人有可能完全痊愈（40%左右），但缺点是大部分人
都会复发。网上有大量不靠谱的信息，这个药物确实有非常少见的一些人对肝脏和白细
胞有伤害，但是发生的可能性并不高于车祸，其他绝大多数人就像大马路上开车的一样
没有任何副作用，而且这个药物是可以长期甚至终身服用的。
这个病不能通过自己饮食和生活状态自己变好，不治疗任由甲亢发展对身体伤害非常大
，甚至可能发展为危及生命的“甲亢危象”
中医无法治疗该病，中药甚至可能有更大的肝毒性
两三个星期前一天早上忽然头晕心脏疼，赶紧去了urgent care。医生抽血后发现tsh
0.01远低于正常值,t4 2.5高于正常值，判定为甲亢，还有一个抗体指标tsi还没有出来
，然后给我安排了一个专科医生。几天后我去专科，tsi也出来了，高于正常值，专科医
生直接就说从这三个指标看我基本就是graves disease。让我回去就可以吃药治疗了。

因病毒引起的发烧，有可能会导致Reye's Syndrome。Kawasaki disease不是病毒性的

发帖数: 806 | 26 我邻居经常带他的卡特豪拉豹犬去打猎，平时也天天在灌木丛中off leash跑的。昨天
告诉我说兽医做gait analysis时注意到狗关节很僵硬，怀疑是lyme disease。其他症
状还有几个月来energy level比以前低了很多，近两周特别差，还伴有depression。问
题是这个很难诊断，所以他们就直接开始治疗了，好消息是开始治疗之后的第二天就好

disease通常在被tick咬2－5个月才有症状，时间长了对抗生素就没什么反应了，可是
疫苗好像很controversial，Kirk当年打过之后，脸肿得像个大菠萝一样，吓死人了，
马上去看急诊的。他说兽医说frontline基本没用，advantix还好，推荐vectra。我们
的经历是frontline真是piece of shit，当年Kirk身上的flea一样疯长，后来用了K9

标 题: 求助：表姐儿子得了罕见的骨溶解症Gorham disease

我已经帮忙联系了波士顿儿童医院，院方可以对病情进行评估，远程诊疗。现在需要把
一些国内的诊断书和病历翻译成英文。我自己翻译很困难，而且不准确，医学术语太多
。所以想请版上好心人帮忙翻译，家人都很焦虑，如能相助，不胜感激。
另外，不知有没有人知道，还有哪里对Gorham disease有诊疗经验，国内对这个病了解
很少，没有任何诊疗办法。请有了解的人能提供一些信息，非常感谢！

may still be negative at this point, because it takes time for your body to
mount an immune reaction. But it is crucial to start treatment ASAP,
because Lyme disease can lead to devastating consequences. Lyme disease
cannot be treated by cephalosporin.
You need to go to the emergency room and get doxycycline. |

(一种四环素）100 mg 2X/day; or Amoxicillin 500 mg 3X/day; or cefuroxime 500
mg 2X/day. Treatment should last 14-21 days once diagnosis has been
established
So I was wrong - cephalosporin can be used. But I still think you need to
see a doctor since treatment dependent on the correct diagnosis of disease

i agree that 21 days course of antibiotic is needed, furthermore, i think it
should be intravenous rocephin(ceftriaxone) instead of oral doxycycline,
this is my rationale, for primary infection, doxy is good enough, plus it
also covers other tick borne diseases,
while in theory the patient may have recurrent primary lyme disease due to
another tick bite, but with headache, you really cannot exclude lyme
meningitis, so you HAVE to offer a spinal tap, and if i were the patient, i

lyme disease and there are plenty of "bulls eye" pictures that you can

发帖数: 61 | 50 上周血液检查发现 lyme disease 阳性， IGM P２３， P４１ 阳性。 之所以做这个检
查是因为一次血液检查TSH 高， 怀疑甲状腺机能减退( negative for the full test)
，医生复查附加了这项－－在马里兰LYME DISEASE 高发区，同时很多症状符合。
户外不多，不记得有被虫子盯，也没有发现过有大的RASH。 不过好几个月特别累， 头
里面好象灌了什么，很重， 很累。晚上睡得长也不顶用，要命的是，睡了醒来，脖子
很疼－－调整枕头高度没用，腰也痛。白天背很痛。其他：皮肤很干，有不少部位发生
湿诊，脸上也经常痒，眼睑不时发现感染
请教了检查查出这个病以外的医生，发现很难找到更好的办法。先吃药一个月再说。
１００MG TWICE A DAY －－Doxycycline
问题，
如果是LYME，为什么只检查到IGM， 而没有检查到IGG（基于我的症状已经好几个月，
应该不会是急性的感染）。比较害怕是细菌躲到我的血脑屏障之后捣乱。如果已经到感
染比较晚期，医生开的药是不是最合适？有人有亲身经验或者临床经验吗。谢谢！ |
