medicare限制了每项procedure的价格，但是不限制procedure的量。原因是
1. medicare没有budget
2. 老年人多看病多吃药多治疗被认为是好事，虽然过度医疗可能有害
3. medicare变成政治问题, 面对选票和政治捐献，政府没有意愿控制医疗使用量，减
少滥用。
所以medicare的秘密是
1. 医院薄利多销，以量取胜，尤其是大量增加昂贵的procedure。政府渲染medicare是
bad customer, 剥削医院，但是事实上医院接受的medicare逐年上升。说明医院并不吃
亏。
2. 政府的low administrative cost来自于从不说no. 不花资源去阻止滥用.
结论是medicare的低单价，导致了过度医疗，反而拉高了医疗费用，最后全民买单。 | t**x
