1. certified birth certificate from your local country's birth/death
registrar office. This office is usually located in your country's
government building and court house where you pay for property tax.
2. your and your ld's driver licenses and passports also if you are not
permanent residents or citizens.also make a copy
3. photo of your baby. get the requirement from the webpage of the
department of states
4. a check of $80 paid to the department of states
5. application form that you can download from the web of the department of
states.
travel document:
seems that you have to travel to washington d c now if you are in the
midwest
1. certified birth certificate
2. marriage license
3. both u and ur ld's passport, visa entering us, other documents, such as i
-20 or i-797, i-94, expired passport
4. a money order for the chinese embassy. forget how much, u had better
check the web of chinese embassy
5. photo of the baby
6. application form that u can download from the web of chinese embassy
7. make a copy of all materials
