我父亲于12年五月份因双侧胸腔积液，心包积液，胸膜，纵隔淋巴转移，积液细胞涂片
检测为腺癌。至今也没有明确的原发病灶。CT推测可能是在左肺中部和淋巴融合一起，
但增强CT中没有发现明显的显影增强信号。
由于开始没有足够的病理组织做基因检测，针对腺癌，先吃英版易瑞沙三个月，在此期
间发现在病发初期取心包积液的针眼处有一肿块在形成，肿块取出，病理化验确认是腺
癌。估计是心包积液引流管带出的癌细胞种植在此。由于服易瑞沙期间该肿块在长，断
定易瑞沙无效，随停止服用易瑞沙。
随后由于黑曲霉感染，服用伏立康唑一个月。去年十月份开始力比泰加卡铂(AUC5) 一
线化疗，由于反应大，从第二次化疗起卡铂降为（AUC3），六个疗程下来，肿大淋巴少
许变小，但由于双侧胸腔积液，且积液中再次发现腺癌细胞，另加CT上显示两肺中模糊
影增多，断定力比泰加卡铂抗药。停止一线化疗。
基因检测除了HER2表达高，其它靶向基因统统野生型。最近上二线化疗赫赛汀加多西他
赛一周一次，两周下来咳嗽急剧加重并伴有大量粘稠的白痰，最近两天开始不定期出现
呼吸困难。请问大师，是否可以认为二线化疗方案已经失败？
肺腺癌HER2表达到病人，较理想的化疗方案是什么？
父亲一向身体很好，至今心脏，肝肾功能很好，非常不舍父亲这样衰弱下去，还请各位
好心大夫帮助提供治疗建议，希望父亲能够陪伴我们更多时间。非常感谢！ |

this is very dangerous. this is most likely interstitial pneumonitis caused
by the targeted agent. do a chest X ray immediately. unless it is clear
other than the lung cancer, immediately stop the targeted agent and chemo
drugs.
EGFR and Her2 targeted agents cause interstitial pneumonitis in more than 5%
patients in east Asian population in a duration of 1-2 weeks. interstitial
pneumonitis carries a very high death rate (30-60%).

discontinue the targeted agent and chemo drugs, and use corticosteroids or
immunosuppressant, but they may worsen the tumor.
(standard chemo drugs can also cause interstitial pneumonitis, but not as
unpredictable as targeted agents in this aspect)
if it's not, then you should look for the cause of large amount of secretion
and difficulty in breathing.

ray；右侧是的 经HERCEPTIN/DOCETAXEL 两次化疗，出现呼吸困难后于5/3/13拍的X

patient, i cannot make a judgment whether interstitial pneumonitis was the
cause for this change.
was oxygenation checked? was arterial blood gas done when he had breathing
difficulty? if you have these results, it may be easier to make a judgment.
i think you should specifically ask his doctor what is the cause for his
worsened symptoms including intermittent breathing difficulty.

X-RAY是在美国做的。医生断定症状是代表两个化疗方案均对我父亲无效下的肿瘤进展
。进展之凶猛我们始料不及，上周六已上呼吸机进ICU。医院现按照肺炎进行治疗，注
射抗生素。
我希望医生送我父亲的组织蜡块重新做基因检测，看看化疗后父亲的肿瘤PROFILE是否
有改变，是否有基因变异，看看靶向分子可不可以抑制肿瘤进展。
我想请教您，我这样做还有意义吗？我也很想听您的建议和见解。多谢！

发帖数: 4124 | 8 increased reticular grids on chest X ray, large amount clear or white phlegm
and rapidly decreasing respiratory functions are among the major features
of interstitial pneumonitis. i cannot say for certain whether he has drug-
induced interstitial pneumonitis without seeing him.
it is not well recognized by some physicians even in the US with one major
reason being that it is quite rare in Caucasians and African Americans. you
might ask his physician whether it was considered. multiple studies in Japan
showed it is very common there (~5%) and anecdotal figures from China seem
to show about the same incidence for Chinese.
the main treatments are mechanical ventilation and sometimes corticosteroids
and/or immunosuppressants. they are not much different from what he
receives now.
i don't think aggressive cancer-directed treatments should be encouraged at
this time. the focus should be on improving respiratory functions.

我父亲在呼吸机上十六天了。我心急如焚！入院前八天按急性肺炎治疗，由于痰，血液
，大小便，支气管镜盥洗液中都没有培养出细菌来，医院更加确认是肿瘤进展，打算放
弃治疗，建议拔管。我和家人坚决不同意，父亲除了肺，其他器官都还健康，也没有远
处转移，究竟是癌症进展，还是化疗毒性太大，引起的一系列的针对肺的毒性反应，难
以确定。最近抽出大量胸腔积液后，我发现父亲的呼吸有所改善，建议医院无论如何让
我父亲脱一次机，不行再上，医院坚持脱机就不能再上机。就这样僵持着，我束手无策
。医院强调他们的目标就是确保父亲舒服。在最近一次SBT 中， 父亲自主呼吸了一个
半小时，他没有什么不良反应，医生偏说他的PCO2 (73) 值太高有危险，强行让我父亲
回到麻醉状态。本打算昨天再试着脱机，结果医生说我父亲又有新的肺炎（强调是插管
的结果），拒绝再试脱机。这样一天一天的耗着，心力交瘁，快崩溃了，请问这种情况

Thank you very much for the suggestion.
My father did receive two weekly-dose of herceptin plus docetaxel. After
this combination of regimen his symptom got worsening...though hard to tell
the exact cause.
How about lapatinib, could you please give your insight and advice. Thanks! |
