像PHMI(Partners Harvard Medical International)等都在中国积极活动。

China’s move from socialized medicine fuels debate
Special care for those who can afford it becomes lightning rod for criticism
By Li Yan and Luo Jieqi
BEIJING ( Caixin Online ) — Neither specialized treatment nor medical
specialists are required when a “special-needs” patient checks into a
hospital in China.
But the patient’s private room ought to be especially comfortable. Ideally
it should be equipped with a sofa, tea table and big-screen TV. Nurses
should be extra-attentive. And the best surgeons in town should be available
around the clock.
After all, for Chinese hospitals the expression “special needs” is a
euphemism for “rich.”
And here’s another requirement: Public hospitals that offer services to
special-needs patients should be prepared to defend this increasingly
controversial, yet highly lucrative health-care business against its many
critics.
Opponents of special-needs services think public hospitals should treat
wealthy people no different than the rest of their patients. That was the
rationale behind a 2009 order issued by the State Council, China’s cabinet,
stipulating that “special-needs services must not exceed 10% of the total
services provided by public hospitals” nationwide.
The cabinet’s directive prompted governments in Beijing, Shanghai, Shenzhen
, Suzhou (a city in the eastern province of Jiangsu), and elsewhere to
impose limits on special-needs medical services.
As a result, services for the rich were eliminated at some public hospitals
and moved to separate facilities that were built inside health-care “
industrial parks.” City governments also encouraged private investors to
finance “special-needs care centers.”
Today, special-needs service providers, including public hospitals, are
still doing a booming business. In Beijing, for example, almost every public
hospital citywide has a special-needs wing. In Guangzhou, hospitals have
been expanding their special-needs treatment facilities.
Political pressure and debates about patient fairness have not discouraged
hospitals from catering to the wealthy for the simple fact that VIP services
make money. A recent survey of Beijing’s top hospitals, for example, found
each has a special-needs wing.
“Hospitals are profiting from special-needs care,” said Zhang Songlun, who
formerly directed the international department at Peking Union Medical
College Hospital. “So they have no motivation to eliminate it.”
No lines
Peking Union was one of the first in the country to build facilities that
cater to the well-off. What’s now called the hospital’s international
department would be labeled a special-needs wing at another medical facility.
Caixin Online
Patients who can afford health care in the international department enjoy
quieter, cleaner and better-equipped facilities than their less fortunate
counterparts on the other side of the hospital. Receptionists are friendly,
and the tea is free for patients seated comfortably in brightly lit waiting
rooms. Moreover, outpatients need not stand in long lines like the common
folk waiting in other parts of the hospital.
Rooms in the international section cost up to 1,500 yuan ($245) a day. The
best ones feature private bathrooms, automatically adjustable beds, sofas,
tea tables, TVs and refrigerators.
“We have independent clinics, emergency rooms and patient rooms,” said an
employee of an international department who asked not to be named. “There’
s no need to wait because all our examination and treatment services can be
booked in advance.”
The international department and an adjacent section for high-level
Communist Party officials have a combined 120 beds. The department handles
some 2,500 patients and 1,000 surgeries annually.
Elsewhere in Beijing, well-to-do patients are well-treated at Fuwai Hospital
. This facility, which has a reputation for excellent cardiovascular care,
has an entire floor of suites dedicated to special-needs patients.
At any one time, Fuwai’s special-needs floor can accommodate no more than
20 patients staying in apartment-like rooms equipped with high-end furniture
and appliances.
“Once you’re checked in, you can enjoy one-stop service,” said a doctor
who works at the Fuwai special-needs facility. “There’s no need to wait
for any of our examinations or treatments. All clinical blood samples,
electrocardiograms and other services can be performed right in the room.”
“We provide 24-hour care services, including surgeries and special
consultations.”
Price advantages
In the early 1990s, a relaxing of hospital service rules by the former
Ministry of Health set the stage for special-needs hospital wings. Health-
care facilities were remodeled and additions built to accommodate rooms for
wealthy patients. Some hospitals even bought and converted old hotels into
special-needs departments.
Since the State Council’s 2009’s directive, however, calls for public
hospitals to focus on the general public’s health-care needs have grown
louder. Critics say special-needs departments represent an improper use of
public funds and encourage unequal patient treatment.
As a result, lately some public hospitals have found it increasingly
difficult to win government permission for plans to expand their special-
needs facilities.

US-china only
