Signature of Person Preparing Form 应该填写和签字对吗？
2. 485的表和材料是否也要像I-140一样单面打印？
3. 我的485中带了EVL, LD和小朋友的485中应该放我的EVL呢，还是我需要写affidavit
of support?
4. 485的材料都可以用复印件,对吗？ (EVL, Birth Certificate, marriage
certificate, etc.)

yes, all documents would have to be single paper.
affidavit
employment verification letter is just used for showing the employment
relationship between u and employer. nobody would put your family members in
this letter. You just need to mention in this 485 cover letter that , AAA
is my LD, BBB is my child.
