1. you don't necessarily need ap. if you are still on h1, you can still come
back on h1, and does not affect 485. however, if you are already aos, you
will have to leave after ap is approved, otherwise your 485 is forfeited.
2. If your 485 is approved while your are outside of the country: dunno...

the major applicant and in H1B status. Now I am wondering if she will be
rejected for 485 when current. Or can she come back with H4 visa to avoid
the problem?

As long as you hold non-immigration visa, you can leave US without AP, and,
if you are already aos (using AP for your previous trip), you can still
apply for H-1 visa and enter with H-1 visa.

So to answer OP's question, if you have H visa, you will be fine travel to China without AP (which also doesn't affect your AP approval), but you will need to apply for H visa at embassy if your visa expired. If your green card is approved while you are outside of the country, that is fine as well. (I know someone mail the green card back and still able to enter, but I don't think that is recommended).
