发帖数: 2763 | 2 看见这里经常有关于甲状腺的问题，因为是我的专业，所以想总结一下，方便后来的JM
们查找。
首先，科普一下几个lab
ＴＳＨ， thyroid stimulating hormone，是垂体释放的促甲状腺激素，作用是通过
甲状腺受体，刺激甲状腺合成甲状腺素，绝大部分是T4,小部分是T3，然后在外周组织
T4转化为T3，T3是真正起作用的活性激素。TSH目前的检查非常灵敏，可以检查到早期
的甲状腺素变化，这是一个feedback，也就是说，当体内甲状腺素过多时（甲亢），垂
体感受到了，TSh就会下降，反过来，甲状腺素低了（甲减），TSH就会升高。
在怀孕早期，母亲是胎儿甲状腺素的唯一来源，从第4-6周起，母体的甲状腺素需要量
就开始上升，特别是在用fertility treatment的妇女中。如果母亲怀孕前就有甲状腺
降低在服用甲状腺素的，应该在正式准备怀孕的过程中就和医生联系，开始加量，以维
持胎儿的需要。
目前最争议最棘手的，是原本甲状腺正常的妇女在怀孕中对甲状腺疾病的筛查。
1。什么时候查。因为TSh是一个很便宜很简单的测试，而且如果能够及早发现纠正甲减

thyroid. 0.35按我ob的标准，属于正常范围之内。孕妇在怀孕初期，由于hcg高的缘故
，是会轻微甲亢的。 |

发帖数: 980 | 5 3.36不需要吃药.我第一次3.6,医生也说不太好,但是也不算甲减,身体也没有任何不适.
第2次再查就1.5了,而且确实是要查ft3,ft4得,是叫thyroid full panel. 再说这个药
的用量也不是ob能控制的,要看内分泌专科. 我以前甲状腺就有问题,所以知道的多一些.
我觉得这种有点什么事情就
让你去er得非常不负责任,我第一个ob clinic也是这样,很差,后来我自己把他们dump了
,现在这个是这里最好的,说如果有什么紧急情况,先打电话给office, 来office如果
office不开门也是24小时有人可以联系的.
不要你了更好,赶紧找个好地把. |

发帖数: 288 | 8 9周1天的时候第一次见OB，血检尿检结果出来了，其它都正常，只有TSH(thyroid
stimulating hormone促甲状腺激素)低于正常值，只有0.22。正常值是0.3-3.3。护士
转告OB让我下一次见她时再重做测试，说可能和怀孕有关。忘了问有多严重，有什么注
意事项改进方法。
回去查了以往的年检报告，TSH都很正常，10年1.59，08年1.3。JMS有过类似经验吗？

状腺炎，没有做任何治疗，就是每1-2个月去查一次血，我上周刚做的lab test，今天
说是一切正常 (现在快产后9个月）。我6个月左右也是体重掉得厉害。现在体重又涨回
来了，我自已估摸着应该没事了，果然检查就没事了 ：）
看mm的描述应该和我挺像的，估计就是产后甲状腺炎，英文应该是post partum
thyroiditis。没有什么大问题，奶照常喂，药没有吃～ |

所以严重缺碘与甲低有关.
不太严重缺碘, 与甲亢有关.

thyroid stimulating hormone 0.61 uiu/ml reference (0.34-5.66)
thyroxine, free 1.16 ng/dl reference (0.52-1.21)
ob说我的thyroxine是normal，但是在border上，所以继续抽血double check一下，如果
有问题，我需要看医生。
这个thyroxine太高，是甲亢么？有点担心，所以先问问。有知道的mm们么？ |

If your TSH is normal, your thyroid function is NORMAL for sure!.
FREE T3, T4 is affected by lots of other factors, including pregnancy. So
TSH is the golden standard.
btw, TSH less than 4 is not bad at all. It wouldn't change dramatically in a
short time, that's why the doctor usually asked you to test that after a
little while and see if it's within normal range. |

道，她说一般一周，然后问了我什么保险，说：哦，你明天就能知道。
还有就是看查的是什么。我看fertility医生的时候，hcg和孕酮都是当天就通知结果。
但prolactin或者thyroid什么的，她们实验室每周只run一次。 |

发帖数: 388 | 24 今天见了OB，OB建议我吃药，她说吃不吃药对甲状腺的恢复是independent的，就是说
甲状腺不会对药有依赖，吃药是为了让身体不难受，对甲状腺自身的变化没有作用，甲
状腺该恢复就恢复，到时停药，不恢复就只能终生吃药。
另外还有些担心宝宝，因为孕期没有检查过甲状腺的状况，不知道那时有没有问题。但
是儿医说 宝宝的两次newborn screening 中thyroid都是normal的，所以应该不用担心
。宝宝现在5个多月了，长得很高很壮，还很爱笑。 |

减的话，可以完全没有症状的。我年检时跟PCP说了我准备怀孕，她就帮我在验血单里
加了TSH和T4，结果发现TSH偏高但T4正常，进一步验血发现抗甲状腺的抗体，确证为
hashimoto thyroiditis，开始吃synthroid，但是我一点症状都没有。 |

好吧，我标题党了。事情是这样滴。
本人孕中期开始有轻度的大细胞型贫血，血检结果是缺B12－－血液B12含量比正常的最
低值少一咪咪。于是就买了瓶B12开始嗑药。这嗑啊嗑啊，就嗑到了宝宝快四个月。这
几个月里我发现如果停吃B12几天，奶量一定会减少。大概少1／6，从30 oz 到25 oz
的样子吧。
那么我为什么会缺B12呢？这个我的医生也不知道。肉里B12丰富，所以一般美国缺B12
的多是素食主义者。可我是肉食动物啊。所以无解。不过如果google之网上和我相似的
例子还是不少滴。
那么磕B12对其他各位缺奶的宝妈有用么？我发唠叨这篇文是因为今天在网上闲逛看到
下面这段话。

法是一样的，就是吃synthroid而已，除非已经严重到有甲状腺组织坏死，那就得手术

The medication you mentioned in US is very common used in US. It is weird

a endogcrinologist. 你 out of range了，应该至少还测一下T3 Free and T4 Free

话，需要将到2.5以下，孕期也要监控。另外我的抗体值也很高，所以备孕一年多一直
在吃药，只是药量很少。lz可以去内分泌医生(reproductive endocrinologist)，找专
门治疗thyroid的医生，通常会给开药的。 |

ptu for 8 months but still cannot the thyroid function back to normal), and
became hypothyroidism and need to take replacement every day, but I feel so
much better than I was hyperthyroidism.
if you want to have another baby, i think it's better to be hypo than hyper
because the ptu has far more side effects on the mother and the baby. |

老婆最近被诊断为thyroid cancer, 需要手术+碘131放疗+endocrinologist
我们已经约好肿瘤外科医生准备12月手术
我过去的几年里一直使用学校的student insurance, 老婆来美国以后学校要求她也加
入我的insurance plan, 所以我老婆也在我的insurance plan里面.
但是最近收到保险公司的信, 说需要证明我们过去的12个月也一直under insurance
coverage, 否则的话,这次的claim就要算作pre-existing condition 然后以后所有
subsequent claims都要被deny.
我担心的是, 我老婆在这个保险公司不够12个月, 保险公司不cover医疗费用. 虽说就
算保险公司deny我的claim, 可能手术可以正常进行. 但是因为所有的这几个treatment
都要在同一个cancer center, 手术之后的放疗会不会因为我的insurance 一直deny我
