重的时候一下子就去ENT，估计也得折腾一阵子，从低剂量抗生素开始。
不过反复发炎拖了一年多，是应该早点看ent啦。

全民医疗以后，估计是30%病人的增长，这些病人是原来不买或买不起保险的，加上原
来的medicaid，这些病人才是NP/PA的主体。 所以hospitalist的工作应该不会受影响。
“但是反倒specialist的数量会相对稳定，因为本来specialist大部分都是在私立医院或
者private agency里面工作，除了薪水可能有差别，工作机会应该不会减少太多的。
所以我觉得如果考虑到还要工作20－30年的情况下，做specialist还是有必要的。”
specialist的需要在NP/PA开始subspecialize后一定会受到打压，有人可能会说，NP/
PA会多consult specialist，这个是肯定的，但你看了我的职业指导I， 你应该明白
capitation的含义，就是一个病人就允许那么多钱，PCP叫了consult，你的份额就会减
少。事实上，我说的已经在发生了。

和自己的一些亲身感受。
对于理工科的MS/PHD来说，这条路的好处主要是经济上负担比较小和风险可控。从
career的发展上来看，大致经历technology specialist, part time/full time law
school，asscociate, partner这么几步。走这条路的话，80-90%的人最后可能是end
up做patent law，当然你也可以有条件转去做别的，如果想转的话,law school那一步
很重要。
先说说怎么起步吧。首先assume你有了理工科的MS/PHD，一般来说，PhD更好，有些专
业性很强的学科，比如生物，化学等等基本上是PhD required。另外，学校越牛越好，
这个我后面会详细说为什么。Research不用特别牛(牛人就去做AP了，谁还折腾这个)，
不过有那么几篇paper还是有不少加分的，postdoc或者工业界工作的经验也有加分。但

改了题目才发现今天的更新是多么难写。
饼饼走了，现在没有任何言语可以形容我的心痛。我们太爱他了太舍不得他了。
昨天开始我们发现他几乎不太走动了，除非是去上厕所。其他时候就呆在一个地方，走
路看起来也是很不稳，摇摇晃晃的。今天早上喂饭的时候，我们突然发现他肚子大起来
了，软软的，跟之前不一样了，顿时有种不详的感觉。给他称了体重，长了不少，就觉
得十有八九是变成wet了。早上喂了饭以后我们在沙发上，酥糖也在，酥饼本来远远地
望着，后来脚步蹒跚的走过来，上了沙发，在我们旁边躺了好一会儿。这十步可能是这
两天他走的最远的一次。
大概昨天开始越来越差，今天是最糟糕的一天，他几乎不动，呼吸困难，我们哭了一下
午，就陪着他，后来他慢慢呼吸越来越不好，肚子越来越大（毫不夸张，几个小时后就
比之前大了好多。。），去了沙盆都是出来就直接趴在沙盆旁边，去喝水也是守着水碗
，把头搁在水碗沿上，胸前的毛全部湿了。他太累了没力气fight了。。
大概傍晚的时候，我们差不多已经做了决定要送他走，不能再让他suffer了，可是真的

现在家里的基本情况是家在新泽西，只有老公一人上班，老婆不上班，有一个不到三岁
的娃。全家都很健康，很少看医生，娃马上要去day care，暂时没有生二娃的打算。所
以这个保险平时可能主要用来给娃看医生了。请大家建议一下应该如何选择，多谢！
看到保险有好多种，HMO，EPO，PPO之类，基本定义是这个样子：
HMO，有primary care physician，有health network，network之外不cover；
EPO，不需要指定primary care physician，有health network，network之外不cover；
PPO，不需要指定primary care physician，有health network但是network之内看病也
要出一些钱，network之外cover一部分，deductible比较高。

包括给我们推荐过去那边买房的AGENT。我们现在这边是租房，所以不存在卖房的问题
。但是我不喜欢他给找的那个AGENT，这个纯是凭我主观感觉了。于是我就自己在网上
找了一个AGENT，好不好我其实也不知道，但好歹交流起来比较顺畅。然后我就给
relocation specialist说我不想用他给推荐的agent，他就不高兴了，说他们是拿
agent的referral什么的，意思就是我断他财路。于是他提出如果我自己找agent可以，
但是要跟我的agent说，必须用他们做referral才可以。后来我的agent也同意了，他们
现在签了合同，大概是要给relocation specialist这边30%的referral.
其实我一直没弄明白这整件事跟relocation specialist有什么关系，公司又不帮我们
付任何买房的closing fee什么的，我就是自己买房自己找agent不行么？为什么非要让

配偶在国内320天左右。结果tax specialist直接用了exclusion，因为配偶虽然
treated as resident alien, 但是是国内全年的税务居民，可以用exclusion.
我对这个不太确定，反复跟tax specialist讨论。这个tax specialist又请了其他高级
的tax specialist过目，没毛病。
我还是不放心，多花了$40买了一个peace of mind.

可能不是所有人都知道easement是什么？我的解释就是土地所有权还是你的，但不幸的

发帖数: 35 | 48 也就是凑个热闹,过了这阵子就算了.大多数人没那个决心放进去15万美刀和3年时间去
读JD,基本都指望着先找tech specialist位置,再指望firm给出学费念part time JD.
这条路,先是firm里tech specialist的位置本来就很少,而且缺的是EE/CS方面的人,这
些专业的人本来收入就不错,转过去收入持平甚至还要降低,所以没什么动力.做bio/
chem的人,想转的最多,但是firm的需求量很小.而且firm很看重communication skills
和writing skills,很多中国学生并没有优势.就算找到了tech specialist的位置,真正
愿意出钱给读part time JD的firm并不多,主要看你跟所里partner的个人关系如何,和
firm的业务偏重如何.不是那么想当然有个理科学位就肯定能tech specialist - part-
time JD - patent attorney的.这个版上的中国律师很多,基本都是JD/LLM出身,很少有
走这条tech specialist路的. |

是。属于CP(Clinical Pathology).
也需要考Board？
是。可申请CP only或AP/CP住院医生。但clinical chemistry 和microbiology很多由
PhD负责，不需考Board.
Clinical Chemistry属于Clinical Pathologist吗？
