flute, apparently it's not available in China yet. i asked couple pulmonary

呵呵,有些rehab的医生水平的确有问题,比如昨天值班spinal cord的病人RAT call了
,我去看病人,明显pulmonary edema,而年长的attending却说病人dry需要more fluid
.后来在我的劝导下他才同意给lasix.
关于consult还是要看attending,有的attending喜欢consult各个service.我大部分是
和fellow电话讨论一下,除非是非叫不可的GI bleed啥的。而且一般都是早晨call 他
们,而不是拖到下午。他们最烦的是下午4点半左右给他们打电话。我觉得在大学医院
作为一个intern,要建立好和各个fellow的良好关系。 |

见了实际病例再读比较有效。所以就想把疾病的发音重点练习一下。不是要把它吃透的
。你觉得呢?
那个tarascon pocket pharmacopoeia都是些药物用法,我是肯定不会去记的(也记不
住),想优先练一下analgesics, cardiovascular, pulmonary, diabetes-related,
anti-histamines, anti-tussives, anxiolytics/hynotics常用药物的商品名和发音。
还有哪些类是大内科病房常用的?
另外,在医院里用EMR写大病历的话,是不是ROS和PE只要一项项click就行了,只要写
好HPI和A/P就好了?

Cortisone.
SymbiCort是 Formoterol + PulmiCort, 一个long acting beta agonist + 一个
inhaled steroid。SymbiCort = symbiotic (生物共生) Cortisone,Formoterol 都不
用记了。
和SymbiCort 同类的是 Advair, Advair = salmeterol + fluticasone. Salmeterol
呢,挖其根源,和Albuterol 、Formoterol一样,都是terol 一族, beta-2 agonist
。而fluticasone 呢,看似又一个难记的,但是慢着,它的brand name 是 什么?是
Flonase 啊!Flonase 作为inhaled steroid,最早是用来治疗过敏性鼻炎,也就是---
Flow nose。所以Advair = 一个long acting beta agonist + 一 |

坐飞机一下睡了7个小时, DVT, 大面积PE,这样的病例不会上NEJM阿,古怪是在
retrieve oocyte to have posthumurous offspring上?取oocyte时候发现肿瘤了?卵
巢癌(ovarian cancer)、绒癌(choriocarcinoma) 啥的,一是可能直接造成
pulmonary tumor embolism,二是可能造成hypercoagulability,easier to form DVT
and PE from DVT.

PE, no matter how severe it is. So it is not hypoxia that causes tachypnea.
Second, I think the chest pain and panic is the major reason for tachypnea.
Third, tachypnea leads to respiratory alkalosis.
To sum up, the occlusion of pulmonary artery is generally not a problem for

Highly competitive: Cardiology, Heme/Onc
Less Competitive:Pulmonary, Critical Care, Rheumatology (Becoming more
competitive), General Medicine (Research based)
Not Competitive: ID, Endo, Nephrology, Geriatrics
I have seen good people not get GI and Cardiology spots. H/O is very
competitive, but there are many spots out there.
