most of lacunar infarct is due to small vessel disease with risk factors
such as hypertension, diabetes, hyperlipidemia and such, though very small

话了.去了医院.立刻做了,CT and blood test说没有问题.到了医院其实就没事了. 医
生说是MINI STROKE.
2星期前在国内做过MRI,所以本人不愿做. 就回家了.国内的医院说是脑血管太细.本人
也有很多不好的习惯.抽烟喝酒都有.
医生给了个处方叫加PLAVIX.
请问该怎么办? 不知道在哪可以买更多的plavix.要不要再做MRI.
在国内做过造影,说不能装支架.

You might want to consider using Prevacid along with it.
Plavix is better than aspirin and is gentler to the stomachs than aspirin.
It is going generic soon. You can keep an eye on this med.
However, 20% of Chinese do not respond to Plavix. You need to run a genetic
test to determine whether your mom is a responder or not. |

bleeding后就应该给阿司匹林。
其实外科医生不是太忌讳阿司匹林，一般会在术前几天停掉，但来不及的话不会因为阿
司匹林而耽误手术的时间。他们比较痛恨plavix，如果心梗的病人有可能要去搭桥的就
不要给plavix.

这是一个颇具争议性的问题.
与支架后冠脉内致命性血栓的危险性有关.
而支架后冠脉内致命性血栓的危险,又与支架的个数,长度,等有关,患者有无DM,高血压
等有关.
是否是支架内复发性血栓,也是决定Plavix应用时间的一个重要因素.
我对病人的情况不是很明确,所以不敢给明确的答案. |

阿司匹林和plavix都可以引起皮肤瘀斑， 一般不是什么大问题。 如果他的医生说没问
题， 可以先观察。 Thrombotic Thrombocytopenic Purpora是一严重的副作用， 我从
来没见过一例，血常规可以看得出来。 如果出现贫血或者瘀斑显著加重就需要停药。
plavix和支架的试验是捆绑的， 所以没人知道aspirin或其他药物是否有同样效果。 |

we consider to repeat MRA if pt can tolerate. I think she may also need some
work up to r/o vasculitis or hypercoagulabe state.
as for 2nd prevention of stroke, there is no data suggesting which is
superior among ASA, plavix vs aggrenox. In practice, physician alternatively
choose them. eg, if one fails on asa, physician may change it to plavix or
aggrenox.

98年在国内查出真性红细胞增多症。
一直以来使用口服羟基脲控制，
交替使用过几个疗程的干扰素注射，但是口服药量并未减少。
血象控制较好时，白细胞，血色素，红细胞含量正常，血小板偏高（约40万，单位不是
很确定，大概正常值是30万左右）。
近期变化：
去年入冬以来血小板呈直线上升趋势，最近半个月都在60－70万居高不下。
国内的主治医生建议口服波力维（Plavix，氯吡格雷，clopidogrel），利用其降低血
小板的副作用达到治疗目的，但是服用两周后完全无效。
查询国内治疗血液疾病的北京大学人民医院网页，上面介绍的治疗手段如下，不知可否
尝试更换治疗方案。
“1.静脉放血：每１－３天１次，每次放血量３００－５００毫升，直至细胞胞比积≤
４５％为止。 2.化疗：常用药物有马利兰、苯丁酸氮芥、环磷酰胺、三尖杉酯碱、二
溴甘露醇等，以上药物可单用，也可２种或３种联合应用。 3.３２磷治疗：对６０岁
以上老年患者可选用。 4.其他：潘生丁、别嘌呤醇、心痛定等均可选用，还可用高压

后来有所上升。因 drug eluting stent 需要用antiplatelets 而不用DES, 除非你经
济上有困难或你对它过敏。再说，Plavix 不是马上要成generic? 总之这决定应该是医
生结合你母亲的comorbidity 来做出，经济只是一个因素。

revenue来于Angiomax。其他三个很少。这次Cangrelor没有通过，按说Cangrelor有四
个trials和25000病人的data，申请前应该和Plavix的比较过，data应该是还可以的。
