15个月以后，停了药和导尿管。没有并发症。第二次，9个病人，一年以后，7个病人能
通过皮肤刺激导尿，但是没有一个能完全控尿，两个病人能下导尿管。其他两个手术带
来的好处一是肠功能，二是膀胱sensation。另，这9个病人中有一个 产生了垂足的后
果。除此外，没有其他的并发症发生。
从这些数据，可以看出，
1，反射弧形成的比例相当高，第一次，2/2, 第二次， 7/9 能自主小便。
2, 不能完全改善： 第一次，2/2停了药和导尿管；第二次，0/9可以完全自主导尿，2/9下了导尿管。
3， 其他好处： 肠功能，膀胱sensation
4， 并发症，或副作用，第一次， 0/2, 第二次： 1/9 垂足。

xray照了几张，没有fluid，心脏偏小是缺水，今天在外面两家医院，呆了11个小时，
根本没饭吃没喝水，没上厕所。粪便很多。
换了诊所，住院医观察时，她居然自己又可以走了。但是有几次，居然就拉了，但是自
己不知道。我觉得这个应该不是问题，她今天没机会poo，bb又太多，应该只是没hold
住而已
今天血检的结果不好，主要是 ：
globulin 4.6 normal 2.3-3.8 (last Wed at the 1st vet was 4.1g/dL, normal
range 2.8-5.1)

席等等遇难呈祥的故事/见证，其实都是关于信心/faith的故事。
说这些故事的人，他们（包括我）是真的这么觉得的。但旁观者是很难体会和认同的，
而这（对旁观者）也是真实和我愿意尊重的。
有一本书：Newberg Andrew. Waldman Mark. 2009. How God changes your brain:
Breakthough findings from a leading Neuroscientist. NY: Ballantine Books.
里面有讲到通过各种等等，当事人大脑里形成了信心（稳定固定的神经元活跃群），那
么这个信心就成了神经元上的真实不虚/neurologically real,和大脑其他的神经元群
（比如电脑，早饭，妈妈）真实是没有本质差别（neurologically）。
我自己也有从“排斥抗拒不以为然不屑”这些所谓见证到“见到又真又活的上帝”这个
心路历程。
不过呢，我比较自私，不爱惹麻烦争议，所以很少和（无相关经历者）分享这些见证。

”“前辈”的都去掉吧。首先在公司也不过junior level，谈不上大牛，其次一提“前

其实讨论soft skill只是为了给自己讨论CRO争取点时间整理思绪，没想到反响那么大

说起CRO(contract research organization)，很多人对这个名字都不陌生，但具体做
什么的而且这两年为什么越来越热，认识就很抽象。说起CRO，不得不说“outsourcing
”。说起outsourcing，大家并不陌生，但也有误区：似乎outsourcing那就是把机会搬
到印度中国等劳动力便宜的地方。说错也不能算全错，因为outsourcing很常见的是IT
以及很多服务部门，比如call center和软工很多都在印度，而General Motor想

最近施一公与Sjors Scheres合作得到了4.5A 分辨率γ secretase的晶体结构， 和

这两个完全不同。
我刚才以为你说的是“鉴定”-identify，比如用ChIP-seq；这个应该叫
identification，而不是prediction吧？我相信MACS这样的软件
open to all species吧？只要你提供Sea Urchin的genome就可以了吧？
但如果你说的是“预测”-prediction，坦白说我觉得很不靠谱；但是也有很多类似的
research，基本逻辑估计也是基于一堆ChIP-seq的“鉴定”出来的TFBS，算出motif (
比如-TAAA-), 这个是training data；然后拿到新的感兴趣的数据里去test？
2. 关于神经系统的genetic variation；其实我只是工作中遇到过，并没有系统去研究
，虽然也觉得有意思。
我做neurology的（但坦白说neurology的人其实不太懂neuroscience，两个不同行当）
，我过去的工作之一就是研究我们这种神经疾病一些GWAS noncoding SNP是否有功能。

St. Joseph's Hospital

这个医院有个附属的Barrow Neurological Institute, 全美Neurology & Neurosurgery
排名top 10. 有个主治医跟我说还进过 top 5，我面试前刚刚查的是排在第9 名. 因此，neuropathology 毫无疑问是很强大的, 这方面的样本量超过很多top university
hospitals. 另外，这个医院还有个很牛的transplantation center, 有单独一栋楼，
叫Heart & Lung Institute.
我面试那天只有我一个 applicant. 面试程序和别的地方差不多. 先是见 PD. PD 自己
是个cytopathologist, 介绍了一下program 的情况，详细问了我以前做的科研，并说
支持住院医做科研，有很多科研项目可以参与，当然多集中在神经和移植两个方面. 然
后一个住院医领着hospital tour. 硬件还是很不错的，医院非常漂亮. 之后和几个主治

accepted into the Neurology program, the prelim year is guaranteed in UF.
This is what they wrote in the invitation letter: "Internal Medicine has a
fully accredited residency program and will reserve PGY-1 positions for
qualified Neurology applicants. Unless you have made arrangements to take
your PGY-1 year elsewhere, you should apply to the Internal Medicine program
at UF." |

I graduated in 1991 with bachelor, clinical work in Neurology, Master of
medicine in 1997,1998-2003, PhD in neuroscience from Europe.
Postdoc- now, in USA. Got GC card this year. All research are neurology
related basic research.
Going to take Step 1 soon.
Hope to hear other people's advice too.

我们这里没有neurology service，seizure的病人都是收到medicine，基本上都是药物
水平太低，load一下，monitor一下level, 调整一下剂量，最多的碰到3,4种药物一起
上才控制住的。检查最多也是EEG加MRI, 我从来没有refer过病人去做过VEEG ，
surgery或电极植入。感觉这些是那些非常顽固的病人才需要的吧。不知道疗效有多神
奇，我比较持保留态度。
另外楼主的弟弟大概20多吧，这种年龄段新发的seizure是不是secondary的可能性更大
呢？

materials from ERAS PostOffice. Once the application becomes "complete" the
program will review it (assume it's before the program's deadline).

A "categorical" position is one which offers full residency training
required for board certification in that specialty. A "preliminary" position
, in contrast, is a position offering only 1-2 years of training generally
prior to entry into advanced specialty programs. Many internal medicine and
surgery training programs offer preliminary positions in addition to
categorical positions. Transitional year programs are also considered
preliminary year training programs.

be easier than program B, as there are less floor months and unit months.
As of call schedule, there could be Q3call, Q4call, Q3 is definately busier
than Q4, that means you ahve to be on call every 3 days, you are working on
regular schedule on the two days in between. Also the frequency of long call
matters, that means overnight call, usually happened on weekends. now there

也感谢远方的家人长久的关注和支持。Medi上很多ID给了我很多支持帮助，现在是回报
的时候了。我很早就想分享这个，但是结果没出来，怕误导大家。现在找OB非常难，没
有connection, 难上加难。很多做postdoc或PHD的同学，还有一些做clinical
research的同学，因为工作关系，多少可以通过老板或同事找到connection而去做OB。
可是我们呆在家里的妈妈们整天忙孩子忙考试，没有多少机会接触外界，所以要比别人
更早动手，留心去找connection.我们的connection在哪里？根据我的经验，在local
community. 首先，找个很好的社区居住，这个估计大部分有孩子的中国家庭都能做到
，如果你还在为好区稍贵的房租犹豫不觉，那我劝你这个投资绝对值得，无论对孩子还
是对你自己。如果我当初没有住在现在的地方，估计会是另一个结果了。然后通过三条
渠道，以下详细述说。
1. 与邻居交朋友。不要顾及邻居是印度人还是哪里人，愿意在好区租房的邻居都是
