我们这里是曾经的工业区，美国的制造业走下坡路之后，很多工厂被废弃了，70年代的
时候有不少的污染，听我们这里的attending 讲，这里的白血病的发病率是个black
dot，比全国高几十倍的样子。他刚到这里的时候，第一件事情就是买
water purifier。 即使现在，我们这里的ICU的一个人还说要用某种特定的牌子的
water purifier，比我们常规用的要贵不少那种。。。我所在的城市不小，我住的是郊
区，曾经被评为最适于居住的小区，理论上水管中的水是直接可以拿来喝的。。
在思维发散说点别的。今天我在clinic问attending，问他trump上台后，药物的价格是
否真的有下降的空间。。。attending说在药物下降方面，总统可以做的非常有限，关
键是保险公司。是保险公司和药厂negotiate药价。比如lenalidomide，是治疗

前些天看到Nature上的一个报道，关于 CombinatoRx 公司的 (Nature 2006)。这个公
司的想法很简单，大概意思是：把FDA批准的，过了专利保护期的药，两两组合一下，
找到新的适应症，或更好的疗效。
我的问题是，药物本省过了专利保护期，应该价格不贵，而且生产厂商估计不少。那么
如果CombinatoRx真的找到一个有用的combination，他们怎么从中盈利呢？提高药价？
其它商家生产相同的药，不可以被医生用于CombinatoRx 发现的新适应症么？
一个更简单的例子，Thalidomine 的专利期找就过去了，可是不停地有人发现它的新用
途，比如最近用于治疗MYELOMA。那么发现Thalidomine用于治疗myeloma的公司怎样才
能从这个发现盈利？ |

结构指导的药物设计成功例子不要太多。现在基本所有的抗艾滋的protease inhibitor
都是这么来的。
当年的SARS，要不是被中国靠防疫隔离灭绝了，也要上冠状病毒protease inhibitor特
效药了，也是解了结构才能设计出来的特效药。
还有治疗multiple myeloma 的 proteasome inhibitor,没结构能做出来么？
还有现在很多抗癌的kinase inhibitor，很多都是有结构才有的针对性设计。
你有个靶点蛋白的结构，药厂不要抢疯掉。

国内一亲戚最近在南京军总医院被确诊为系统性淀粉样变AL型(Primary systemic
amyloidosis),现在累及到多个脏器包括心脏，胃肠道，肾脏。肾活检病理及刚果红染
色支持诊断，尚未合并骨髓瘤(myeloma).
他这几个月都已经无法正常进食，人已经很虚弱了，皮包骨头。国内虽然有诊断，但是
却拿不出有效的治疗方案。北京上海大医院都看过了。家人急死了，让我问问美国这种
病有没有好的办法。版上大家如果有这方面的信息，希望告诉我。

alklyzing chemo, melphalan plus prednsione, or even thalidomide plus
decadron,
if patient is candidate for bone marrow transplant then start non-alkylating
chemo for induction then proceed with transplant, sometimes they do double
transplant for better response though it's not curative but do increase
overall survival, |

for myeloma, actually there are some new results, for initial treatment, you
can start with thalidomide plus decadron, lenalidomide is the oral
derivative of thalidomide and has better efficacy and improved toxic profile
, but both thalidomide and lenalidomide are extremely expensive,
the other option would be velcade plus melphalan plus steroid,
either one of the above two options would be much better than the
tradtionally used VAD,

的结果也有很多有疑问的地方，我在这里详细的把我所知道的写出来，望哪位oncology
或血液病专家看看。附件里也放了我妈妈检验的结果。
我妈身体一直都非常健康，就是有糖尿病。快过年的时候，搬家累着了，同时感冒了，
我妈选择吃了十几贴中药。基本上感冒好了，但是咳嗽一直不好。还有就是当时搬家时
被煤气瓶挤了一下，然后后来走路就一直有点瘸。
中国时间2月2号我妈在家人的陪同下去医院看为什么咳嗽一直不好。一照CT，医生马上
说情况很不好，是癌。于是，做了全身的CT scan，bone scan， 以及MRI，情况都非常
不好，显示出在脊椎上有很严重的病变。尤其是在胸椎的上边看上去有长一块东西（好
像医生是说骨头增生），造成我妈最近一两年背越来越驼，人也矮了一些。当时医生的
诊断就是骨髓多发症肿瘤（multiple myeloma, MM）
接下来做了全身各个器官的检查，肝胆脾肺（肠，胃还没做），卵巢子宫都没有问题。
具体的检查报告在附件了。其中提到的淋巴的问题，以及子宫肌瘤的问题，医生都说过

发帖数: 85 | 43 爸爸69岁，被诊断出患有多发性骨髓瘤。现在是high risk smoldering myeloma,
因为骨头还没有受损伤。但是不幸的是细胞遗传学的结果不好，13和17号染色体都有变
异。骨髓穿刺也发现浆细胞比例有40%。其他验血验尿的结果都不太好。
现在进行化疗刚刚一个月，用的药是lenolidomide和Dexamethasone。但是医生担心治
疗效果不会太好，因为染色体的问题。所以以后非常可能会用到例如Bortezomib之类的
更厉害的化疗的药。
但是现在爸爸就已经有了很严重的副作用。他不想再把化疗进行下去了。我们也不知道
该怎么办。我看到在 MD Anderson 好像还有一个免疫疗法，不知道这种方法可行吗？
难道真的只有化疗吗？爸爸现在已经非常痛苦，我简直不敢想象以后的日子怎么过。 |
