extra cost. An X-ray will determine whether there is any bone fracture.
It sounds more like ligament and tendon injury, which may not need absolute
immobilization. However, a Ace-Band or other supporting device may help to
prevent recurrent over-extension injury while the joint is still weak. 卧床
架高脚, Ice pack may help, but don't expect any magic cure, and the healing
of ligament and tendon takes time. |

儿子3.5岁，周五去Mall里的indoor playgroud 玩儿,从一个不到2 feet的Structure往
下跳，前几次都没事儿，最后一次落地不稳，然后哭了，说右腿疼。地是地毯的，不让
穿鞋进去那种。腿上没青没肿，但是不能站着，不能走。小腿前面脚踝以上1.5英寸的
有压痛。我们觉得有可能轻微骨折骨裂什么的，赶紧打电话给Ped。Ped说不用去ER，回
家冰敷，看第二天表现。结果夜里没睡好，第二天还是不能站和走，不过他自己说好点
儿。周六早上我们赶紧看了一个Oncall的Ped，照了两张X-ray。Wet Read 说没有
Fracture，但是有bruse. 如果周一还没好，再来看。
问题是现在还是不能走，扶着走两步就叫疼，自己说不是脚踝，是脚踝上面小腿正面。
从外表看什么问题也没有，到底是怎么回事儿哪 |

splint，保护手指，没有说指甲的问题，只是说保护好，骨裂可以自己长回。按照医生
推荐，又约了儿童骨科专科医生，建议说要做手术，把指甲跟部放到正确位置；还要在
指端骨头两侧放pin，固定，3-4周后取出。整个手术估价5000元。
这个手术是必须做的么？
如果必须做，从受伤到现在已经6天了，做手术的效果没有当时就做好吧？如果必须做
，这个估价能不能降低呢？

找了骨科医生，医生没有做任何“徒手正位”就让护士给打了石膏，然后看X光片，建
议做手术，打钉子定形。
我们谨慎的把X光片发回国内的专家朋友，请教后，朋友告知，只是很轻微的错位，根
本不用做手术，只要自然愈合就可以了。而且两个在美国医疗系统工作的朋友也建议不
要全然采纳医生建议，因为这里很小很小的伤医生都会建议做手术。
我妈妈第二次去复查（两个星期后），告诉医生不打算做手术，医生警告手可能以后的
灵活性会打折扣。基于他的描述对妈妈来说可以接受，和国内医生的建议，我们坚持不
做手术。
五个星期后去拆石膏，医生告知骨头愈合，我们都很高兴，但紧接着发现妈妈手臂错位
，两只手不对称，而且受伤的手由于骨头错位根本不能扭转。
医生坚持是因为我们不做手术导致的结果，但我们询问国内医生，国内医生认为是美国
医生没有在上石膏前就“徒手正位”导致骨头长歪了。本来是很小的一个问题，因为缺
少最基本的步骤，现在却成了大问题。
我们原本就觉得很奇怪，上石膏前，医生什么都没做，甚至连碰都没碰一下妈妈的手，

X光，只是开了膏药和腰托。妈妈一直戴腰托，并减少了活动，但没有卧床。三周后疼
痛加重，去拍了X光，并做了骨密度检查，诊断为骨质疏松引起的腰椎第十一节压缩性
骨折。之后卧床两个月，但恢复得不好，至今已六个月，疼痛依旧，且直不起腰，很影
响日常生活，挺痛苦的。前几天听一个朋友说她的亲戚同样的情况，马上做了骨髓泥注
入手术，撑开被压缩的骨头，恢复得很好。妈妈便打电话咨询做手术的医院，像她这样
摔了已经六个月，最近又一直在吃治疗骨质疏松的药，骨头可能已经长硬了，还能手术
吗？主刀医生说可以，一年之内都可以，不过压缩的骨头是撑不开了，但可以缓解甚至
消除疼痛，价格￥24000。妈妈又打电话给另一家医院，被告知时间太久了，不能做了
，做了也没有。另外这个手术有风险，有小比例的病人因为骨髓泥渗入脊柱神经系统瘫

3岁娃，偶然发现一只脚的外侧踝关节下方有个pea sized 硬硬的突起。我自己觉得按
压是不怎么动的（就是没有attached to the skin)。没有受伤史，也不排除娃偶尔扭
了没告诉我们。
带去看了儿医，儿医说是推的动，问没有食欲变差也没有weight loss，也不痛，摸了
淋巴结都没异常。送去拍了x-ray，说骨头没问题。儿医说要么自己观察一阵子，要么
带去看 pediatric orthopedic specialist。
有儿科/骨科经验的知道有些什么可能原因么？还有什么检查手段可以确诊呢？比如B超
可以检测是否是cyst？

过了两天 感觉小孩已经啥都不疼了， 虽然手臂上有石膏绷带，但是已经基本上不妨碍
玩了
感觉没啥问题了 但是今天复诊 做了xray 说是 posterior fat pad sign ， fracture

。
但是问题是 小孩感觉根本就不疼了 基本都能正常用左手了啊
但后重新搭上绷带 要两周复诊。 要尽量说的好活动 不要再受伤
请问这个问题严重吗？ 我听着这名词 一点也不懂啊。
小孩非常调皮 整天都在动 本身她都不感觉疼了 怎么可能不让她动胳膊啊

我没有他们那么乐观 还是自己去拍了个xray 片子给health services的医生看 说我没
事没有骨折 再绑一段时间就好了
我不信 自己去医院要来xray disc 有一根骨头（ulna）都快穿皮肤而出了不过倒是真
的看不出fracture 上网放狗一搜 有个dislocation的病例片子跟我的简直一模一样
最后自己去了医院找了orthopedic surgeon 看了片子 诊断为Closed dislocation of
distal radioulnar (joint)
打了一个over-elbow cast for 6 weeks 就是把手腕强拧过去按在那里
现在拆掉石膏已经2 weeks 左手略能活动 医生让做occupational therapy
但是左右forearm 手腕关节明显不对称 旋转角度也非常有限 时有疼痛

了开颅手术，发现左前和左后有两处fracture。医生说由于出血太多，手术效果不是很
理想，现在需要观察24-48小时，但他觉得情况很不乐观。而且他估计老妈是被人袭击
的，自己摔倒不会伤地这么严重，而且老妈身体一直都很好，从来没出现过晕倒的情况
。事情发生的太突然了，家里还有个小孩，真不知道该怎么办了，老爸现在也在从国内
赶来的路上，还不知道怎么跟他交代。现在能想到的几个问题：
1. 医院这边除了陪在旁边，跟她讲话，我们还能做些什么？
2. 医生说48小时内如果没能醒过来，就会成植物人，到时候需要我们做出决定。过了
48小时真的就没有希望了吗？
3. 警方那边除了提供知道的情况，我们还能做些什么协助他们的调查？
4. 老妈在这边是短期的visit，没有保险，后面肯定是巨额的账单。各位大侠，有没有
买完后可以立即生效的保险，可以cover后面的费用？

发帖数: 94 | 41 本人30多岁，直偏瘦，平时锻炼也不算多，但是也不是完全不动，以快走，慢跑，踢踢
腿，拉拉身体为主。
最近半年右侧hip和髋处疼，平时不疼，但是跑起步来疼的厉害，根本跑不了，快走20-
30分钟也会疼，我一开始以为是不是跑步的时候扭住了，卡住了，或者踢腿的时候踢很
了，要不就是没有warm up拉伤啥的。一开始也没在意，想就是停止跑步踢腿等，平时
就走走。结果半年多，还是没好好，跑两步右侧的髋骨处里面（就是右腿和身体链接处
）就疼的厉害，而且右腿感觉不得劲，感觉总有东西拽着。
于是就约专科医生看，一开始说怀疑有肌肉或者髋骨连接处撕裂，说做个outpatient的
缝合手术就好了，结果MRI做下来，说我要做个 femoral acetabular osteotomy的大手
术，就是把右腿股头截断，然后把链接的骨头和髋骨重新position,再把右腿钉上去，
说是因为右腿和髋骨链接不好是最终原因。
然后这个老头医生觉得我很可怜，不停的说很抱歉你这么惨，没有其他治疗办法了，我
差点再医生办公室哭下来。我出了医院立刻去做MRI的地方要了我的MRI结果，能否请版

胸廓对称，气管纵隔居中。
two pleural cavities in symmetry, separated by mediastinum and trachea.
胸腔对不对称是有无病变的一句。
双肺纹理增强，双肺未见明显增大。
both lungs showed increased lung markings, but no apparent enlargement.
纹理增强不是什么好事，但原因可能是病理的，也可能是生理的，如无其它病症，一般
认为正常。
心影大小形态未见明显异常。
The heart is normal in size and shape.
双侧隔肌光滑，肋膈角锐利。
Diaphragm appears smooth. Costophrenic angles are sharp.
肋隔角不尖锐会提示有问题
所见诸肋骨骨质连续。
ribs are normal.
这个大概是查有没有bone fracture.

在美国这种情况医生一般会有倾向性交代
获益和风险应该由医生进行。比如版上有人推荐手术，病人的当地医生没有把握你敢让
他们开吗？
以及不做手术患者长期卧床会有较高的血栓以及其他严重并发症风险。如果患者仅仅是
高龄+骨折而不考虑其他基础病，手术目前是比较明确利大于弊的。但是考虑病人其他
基础病和骨折前的功能状态就需要骨科和麻醉科医生具体问题具体分析了 |

't right and they used vaccum. Anyway, we was sent to see a physical
therapist after we were discharged. He didn't need anything to fix the
broken bone, but we did need to watch for his sleeping position, and void
using left arm a lot(his left collar bone broke).
My 2 cents :talk to your pedi, and ask for a physical therapist. Then start

）。 Ped 说没事， 会自己愈合。 说是比较 difficult 的生产经常会有这种事。
可是我的生产过程实在算不上困难。 现在心里很不舒服。 其间 OB 似乎很着急，边
上看的LG 和我都认为他使劲把婴儿拔出来了，具体怎么做也没看清。
