昨天，这些人折腾了一天，白天，晚上皮啦啦，也不知道谁把谁杀了，谁把谁带走了。
反正我先生和孩子被绑架了，就不知道是不是已经遇害了。我本来是打算睡觉的，但他
们头上，地下装电磁波攻击器，让我几乎窒息。虽然我没倒下，但其实他们在我身上扎
的毒针已经蔓延，我能感觉到我的呼吸有窒息的感觉。我想我对他们已经非常仁慈了，
但他们不依不饶地攻击我，欲置我于死地，让我非常生气，特别是肆意伤害我家里的人
，绑架我先生和孩子，而且有许许多多无辜的生命毁于他们之手，有必要把他们曝光。
当然我还没分析这里边是那几个人具体攻击我，伤害我的，但他们显然都是一伙的。鉴
于他们伤害我的头部，胸部 和胃部，我就把这几部分的医生都曝光，受害者慢慢去调
查，追查他们吧。

我还有一些问题想请教：
1）emphysema和chronic bronchitis的病因是不是不一样呢？我感觉emphysema源于
endogenous antiprotease的缺乏,导致elastin被elastase分解,进而导致alveoli的弹
性被破坏；而chronic bronchitis是由于支气管壁发炎，出现mucus所导致咳嗽。不知
道我的理解对不对？
2）都说吸烟是导致COPD的主要原因，因为吸烟可以导致：1> oxidants/antioxidants
imbalance;2>apoptosis;3>chronic inflammation;4>protease/antiprotease
imbalance.请问这四者是相互关联,互为因果的吗?一个chronic bronchitis患者很有可
能患emphysema吗?
我是外行,如果问题很幼稚,请见谅.

COPD means emphysema, chronic bronchitis, and asthma, you can keep asthma
out of this since it seems to have different immune mediators with
eosinophils being involved,
for emphysema it is the destruction of small airways, you don't necessarily
have to have deficit endogenous production of alpha 1 anti-trypsin, tobacco
use, for instance, will activate macrophage and neutrophils to release
multiple proteinase |

reverse it.
regardless of the primary cause of emphysema, treatments are available/
required for:
1. treating existing infections, preventing new infections
2. dilation of bronchi
3. treating/preventing allergic reactions.

吃福利最多的是主流人种的低收入的那些，但是这都不是太大的问题，只要经济增长了

with spontaneous pneumo is from some sort of underlying lung disease, today
is sunday so the chance of seeing a CT surgeon today at this hour is not
very good, but i could ask them sometime this week,
from all i could remember, spontaneous pneumothorax in adults is categorized
into primary vs secondary,

as second hand smoking is another very important risk factor,
he has to understand, that once COPD/emphysema/chronic bronchitis is
established, the damages to the lungs are irreversible,
however, there are things he could do to slow down the progression of the
disease, starting from
1. quit smoking,
2. get scheduled vaccinations including pneumovax, flu shots annually,
3. keep up with hydration,
4. keep inhaled bronchod |

pathology, which can be seen on chest x-ray, including lung cancer (#1 cause
of cancer death), emphysema (#4 cause of death overall), esophageal cancer,
head & neck cancer, heart disease. All of may have different clues on a
chest x-ray.
If you are TB positive and you have a lung nodule, you are likely to have TB
and need treatment.
If there is a nodule, you need work up regardless whether you smoke or not. |

PA and lateral views of the chest were obtained. Emphysematous changes
within the lung parenchyma. No focal airspace disease, pleural

I attended an webinar by NCCN on adjuvant therapy for NSCLC pts and asked the case. The presenter is leaning towards using adjuvant chemo therapy. I am still collecting more openion on this. My mom will have first first follow up CT in Aug. I guess (guess only) if obvious disease progression were seen, chemo should be used. I still have the hope that if she can rest well and build up her immuine system, she will be able to keep the cancer in check.

达意，见谅。
不变应万变
对于难题要做到镇静，想想我不会的题目，多数人也未必会，镇静，镇静！镇静下来之
后才能慢慢寻找解题思路，难题的难点在于，题目信息不熟悉，备选答案内容不熟悉，
不要慌，想一想，出题者的思路在哪里，用自己熟悉的知识点去解决不熟悉的信息，这
就是apply的过程，比如genetic的题目。以不变应万变。
对于不会的题目，不要粘题，没有思路，跳过去，回头再想。有时是瞬时block out一
叶障目，后头再看的时候会发现豁然开朗。不值得浪费太多时间在一道题目上面。
关于排除法的应用，首先排除自己熟悉的选项，再排除自己不熟悉的选项，如果自己熟
悉的选项不能排除，怎么能排除自己根本看不懂的选项。解决方案：如果最后自己熟悉
的选项不能排除，还是选择自己熟悉的选项。因为太超范围，太偏太远的东西考的可能
性毕竟比较小，较大点的可能是可能是自己熟悉的，但是自己没有做到很好的apply或

章很麻烦，耽误了2个多月，材料才得以从学校寄出。个人觉得申请算是比较顺利。把
