depression, as well as many other issues can add to the problem. Both
insomnia and irritable bowel are common complaints in otherwise healthy
individuals, but they also appear in more severe disease states. However,
it is worth a medical evaluation if that bothers you and adds on to your

common causes of diarrhea is
lactose deficiency (high in asian), esp after drinking milk.
IBS, the bowel is sensitive to what you eat, sometimes it is associated with
stress, try to relax.
IBD, including crhon's and UC, esp when there is blood in your stool and
abdomminal pain. steroid may help.
Infection is another common reason, some kinds of GI infection are
specifically related to Chinese food, like fried rice, but in that case, no
bloody stool. |

腹痛大概是从去年年初开始的。一般发生在非周末傍晚5-7点吃晚饭前，疼痛时间持续
半到一个小时。去年上半年痛过4，5次，然后一直到昨天，安然无恙的一年过后，又发
生了一次。
每次疼痛无一例外都是以下症状：
首先全身搔痒，搔痒从毛发多的部位比如腿根，腋下，还有手心。然后会发展到头皮耳
朵甚至到面部，象是过敏，面部会红肿，然后身体其他部位比如胸部背部会有一些小的
疙瘩。
瘙痒渐渐转弱散去的时候腹部不适的感觉就会隐隐传来。先是想要上大号的感觉，每次
还真有。我想这应该是由后面即将来临的剧痛引起的bowel movement吧。
接下来，就是腹痛，象是宫缩的疼痛，痛到浑身无力，灵魂都要出壳了。此刻，手脚冰
凉，满头大汗，只能蜷缩床上。这样的疼痛一般会持续半个小时到一个小时，然后会自
行消失。
疼痛消失后下床走动，每次都会有少量bleeding，象经期第一天和最后一天的量，还有
脱落的内膜可见。难道这个疼痛是宫颈发炎或者子宫疫病所引起的？
去年上半年疼痛期曾经做过全面检查，包括妇科，一切正常。跟医生描述以上症状他也
难以断定是哪里的问题，只是被告知疼痛发生的时候再去就医。可是这个疼痛 |

should use this medicine with caution because it does not treat the cause of
diarrhea. It is helpful if irritable bowel is the cause, but may prolong
illness if you have a gastroenteritis (infection). |

一直没吃过什么药，现在终于撑不住了，已经影响到我正常社交了。
看了医生后开了药 loperamide，好像还挺管用的。这个药能不能长期服用啊？
IBS不能根治的话我可不可以就后半辈子吃这个药啊？有没有什么副作用？

care or ER immediately.
Otherwise, bed rest & take some ibuprofen or naproxen to control pain and
swelling. Can also try ice compress for the first 24-48 hours.
If no obvious improvement occurs after 3 days, see a doctor. On longer
terms, physical therapy may help. |

乙肝5项检查结果是国内俗称小三阳的一种（1,3,5项阳），转氨酶ALT,AST轻微偏高，
提示可能有轻微炎症的发生。
是否需要进行治疗，还要做一项检查，乙肝病毒定量HBV DNA，看是否超标。

US medical centers have technologies, expertise, or trials that may improve
survival. For anything stage 1 or stage 2, I don't think treating in China
vs in US makes a lot of difference.
2. Just the rectal cancer inpatient surgery alone (no radiation therapy no
chemo) medical bill from top Bay Area hospital is $135K. If you decide to
stay here and treat, you might want to send him to a cheaper cancer center.

以下是孩子的情况，因为时间长，做的检查也多，在这里尽量以log的形式简短点说，

D-1 Feb 17周二。白天低烧，没胃口，晚上开始肚子痛睡不着，去医院急诊，当天住院。
D-2 Feb 18周三。开始呕吐，一直吐到肚子没东西。医生用了口服抗生素某种盘尼西林
。因为之前有长时间的咳嗽病史，医生照了X光，但胸部clear。继续没胃口。心跳持续
140以上，最高达到160多。身上开始出现rash。
D-3 Feb 19周四。呕吐停止，开始拉肚子，大便呈褐色，有腥味，护士说拿去实验室化
验，目前仍未得到结果。没胃口，没有排尿，停用抗生素
D-4 Feb 20周五。做了腹部B超，说腹部有积液，医生确认肠部感染（Bowels
Effection），开始输抗生素。医生说血检显示肝功能不太正常，怀疑是肝病毒，做了
专门的血检但是到现在还没拿到结果。用多种抗生素，转入加护病房，24小时监护，心

近几个星期有left flank和left back pain。Pain level 2/10。医生怀疑肾结石。做
了腹CT with IV contrast。结果如下：
CT Abdomen+Pelvis wo+w IV Con
There are no renal stones or hydronephrosis. No solid enhancing renal masses
are present.
There is no bowel obstruction. The appendix is normal. There is no free air.
There is no intra-abdominal abscess. There is mild rectosigmoid
diverticulosis without evidence of diverticulitis.
目前症状略有减轻，但每天清晨会醒。感觉有气在左下腹肠子里经过。排气后即无不适
。有时会打嗝。
几星期前大便过硬有微量便血。医生检查后说是肛门有小撕裂，叫我不要担心。

患者44岁，男，6年前因为肝癌做了肝移植 （符合米兰标准），之后无灾无病好几年。
去年2016年初发现轻微贫血，血色素12，因为当时在服小剂量阿司匹林，于是停服，看
看有没好转。托托拉拉到了年底，还是贫血，血色素10.5。医生说做个内镜。于是在胃
内发现一个肿瘤，内镜医生直觉是GIST。随后CT，但CT却没有异常。4月初第二次胃镜
，GIST依然还在，取样活检表明可能肝癌转移或者是非常罕见的HAC。第二次CT 报告大
小46x35mm，说比较第一次32mm变大了。可能第一次看片医生那天喝多了？肿瘤位置在
Fundus，非常接近GOJ。
AFP好久没有查了，因为肝上的情况一直很好，肝移植前AFP 100多，移植后AFP一直小
于2。上一次至少是2，3年前。也是疏忽了。现在AFP 254. 显然胃上这个要么是转移，
要么是HAC。
下周二医生还要安排一次PETCT，周三见外科医生，估计排除转移外就安排切除了。 求
这里的专家教授指点，这个情况如何是好？下面我贴上最近几个报告。

是很紧张，我OB说，不要紧，不过该做的检查还是要做，于是抽血查了torch五项
（感染的指标，wiki一下你就知道了），做了羊穿，结果都是阴性。CF我很早就查过了
，是阴性，不过这个病中国人很少得。22周又复查了一个大b超，复查的时候就正常了

的可能，我作了羊穿，验了血检查是否病毒感染，结果都正常。后来每2周做一次B超追
踪，33周白点消失，宝宝生下来很健康。

宝宝下午拉屎了. 大家的回信很有帮助.上午给儿医打电话时是护士回的电话. 护士也
觉得应该没有问题. 她说有的孩子甚至7,8,天或者10天没有BOWEL MOVEMENT，都算正常
的。 差一点我就带宝宝到医院检查去了。
她还说如果用prune juice 要对一份水， 但是如果大便不是硬的就没有必要喝。

prominent loop of fetal bowel", 可能是暂时的，也可能意味着肠梗阻（intestinal
obstruction）,让我过3到4周再B超复查，看是否会消失，还是继续存在。之后又跟Genetic
Counsellor谈了，她说如果是肠梗阻，可能是基因（比如Down Syndrome)导致，也可能是Cystic
fibrosis导致，还有可能是因为Virus...总之具体原因很难讲。她说做羊穿可以看23对染色体，从
而看看是不是基因引起的。可是我的integrated screening算得我的Down的概率是1/35000，其
他几项也都是几万分之一。这种情况下，我需要做羊穿吗？现在还在犹豫。听从了她的话当天做了
Blood work (for Virus testing)和Cystic fibrosis的test（这个比screening时作的那
个多６０多个pattern据说）．这两个结果要下礼拜才出来．有没有姐妹们遇到过或听说过这种情况
的，都是怎么处理的啊？现在脑子里很乱． |
