does the patient has laryngeal stridor? Laryngeal stridor is one of the
mechanism of MSA sudden death. It should be treated either with CPAP or
tracheostomy.
Stridor is different from snoring. If you can not tell the differences
between stridor and snore, search youtube. |

of screening to see baby's neck thickness, then 20 week routine ultrasound.
Most of moms had these three. I had additional one in week 25 for some
condition. I will have 3D/4D ultrasound next week which is not medical
necessary, just to see baby's looking, not covered by insurance.

1. Upper motor neurons alone - PLS
2. Lower motor neurons alone - Progressive muscular atrophy (PMA) and spinal
muscular atrophies (SMAs)
3. Upper and lower motor neurons - ALS
这个孩子才25岁， 好像是无力不是僵直（spasticity），所以我不肯定他得到是ALS甚
至PLS， SMA倒是有可能（adult onset）。 如果有详细的体检和肌电图检查， 诊断应
该很容易。 纠缠这个诊断很重要， 因为有些SMA是良性的， PLS也比ALS好的多，尽管
有些PLS会恶化成ALS。
很多病早期只能说MND， 时间长了会改变，诊断就更明确。
不幸的是治疗大多数只是对症， 药物治疗ALS只能延长3个月的生命。 不过我觉得没有
道理病人不能吃味精， 难道因为MSG是neurotoxic？ 临床上一些药物in vitro是

kidney atrophy是成因极其复杂，目前手段基本不可逆转的器质性病变
从名字就可以知道，只描述了病症，对病因毫无涉及。AIDS都还有个“获得性”
作为定语，而kidney atrophy根本就不知道是基因性疾病还是后天获得。
只有极少数从感染开始造成的肾脏萎缩可以认为是知道部分诱因的，机理
仍然一无所知。
中医也没有太好的办法，对于这种已经发展到器质性病变的阶段，
也一般就说“针石无力”了。中医总是强调治未病。对于这种已经都
器官病变了，现代医学无论中西医都是很没办法的。 |

Collaboration Director Shep Doeleman of the Harvard-Smithsonian Center for
