biggest healthcare system/company in NY with MLTC and PACE program . bi-week
paycheck, salary range 80-90K depend on your experience, knows about OASIS-
C,SAAM, CHHA or LTHHCP 2 -5 yrs expriences preferred,RN office Case
manager experience highly preferred. You need to finish monthly budget
documents, contact hospital, ST Rehab, long term facility, facilitate the in
and out hospitalization process, updated the treatment plan, contact PCP or
hospital for updated medication information, make sure every patient is
happy with current SN, Physical Therapy, Social worker, day care center
service. We do have union but since its a manager position you will not be
in union, 7000 sign on bonus only for visiting nurse, however, you will have
chance to sit for CCM certificate exam after one year, education dept. will
arrange the exam, your salary will be 100k at that time. You will
allowed to do overwork after regular hours(9-5), initial eval visit is $100
per visit per patient. NYS RN licence required. This is a telecommute
position. After 3-6 months, you will be work from Home. You will get a brand
new laptop and and new Verizon touch screen Blackberry. I just got mine and
email comes in at the same time as in computer, you can sit on the
beach to work if you want to. We do not have regular meeting yet so its a
perfect job for young mother. If you have your own laptop or ipad, IT will
install the system for you, we use CITRIX when we work at home. please do

have high caseload right now around 150 but most Chinese patients are quite
stable usually with DM2 or HTN and family member are friendly. I personally
have few clients are more than 100 years old and we provide live-in aide for
them. We do not simply cut down hours for these pt who really need it.
overall its a very relax company and its a happy department. We have
birthday club each months, Yoga class if you interested. Director is a nice
lady. You will pair with a few visiting nurses and a clinical manager in
each
group, there are about thirty managers in our department, the goal is to get
forty-five this year. This year is extremely busy because business tripled
