标 题: PCOS和胰岛素level

我被我的fertility大夫搞得很糊涂。我大龄了(37)，一年前起意要孩子，年初不幸有
一次blighted ovum。最近又开始试了有三个周期，有些烦有些急，加上大龄，就去看
大夫。她做了下B超，很easy地跟我讲有PCOS tendency。然后测了几次血，不太正常的
有两项：
Free Testosterone 实测0.2 正常范围0.2-2.6
Insulin空腹 实测<2 正常范围 6-27
她还是很肯定地告诉我是PCOS tendency。建议我用Metformin或Clomid。我并不
against用药，不过我很有疑惑为什么她一定认为是PCOS倾向。因为，从我能在网上找
到的资料看，PCOS的一个主要表现是有insulin resistance。表现为身体对insulin不
sensitive，导致胰脏分泌更多的insulin，extra insulin 会刺激体内的一些组织，比
如卵巢。卵巢被刺激后会产生extra testosterone。所以，如果PCOS, 合理的应该是
Free testosterone 和 insulin 都偏高，而不是像我这样偏低。
我跟她咨询后，她讲，大多数人可能会遵循这个理论，不过也可能有少部分人会像我这
样disorder, 可能趋势不同。原因可能很复杂，还没有解释。我也不知道她说的是否有
道理呢？
不过至少Metformin是从上述理论来治insulin resistance从而治疗PCOS。因为至今它
还officially只是降糖药。我挺怀疑这对我来说是否真对症。jms有没有有这方面了解

,她放弃治疗的原因,说起来,很奇怪, 她说"治疗好啦,来例假怎么办".). 你的B超医生
也比较怪,B超下:有PCOS,就是有, 没有就是没有.怎么还给PCOS tendency.既然没有确
诊, 为什么就要病人口服Metformin或Clomid. 这些药既没有预防作用,治疗作用也有限
.除非体内同时伴有insulin resistance.
我个人总结关于降糖药治疗PCOS的经验:1 体内真有insulin resistance. 2 病人有典
型的PCOS肥胖体征者, 有一定的疗效.

发帖数: 1211 | 3 First of all, neither testosterone level, nor insulin level are diagnostic
criteria for PCOS.
PCOS is diagnosed by 1) infrequent or no menstrual cycle; 2) clinical
evidence of
high androgen or biochemical evidence of high androgen (note: not
necessarily
testosterone); 3) lack of other cause for the above two. Insulin resistance
is
not always seen in PCOS, and ultrasound evidence is not required for
diagnosis.
If you desire to have children, PCOS will significantly decrease your chance
of
pregnancy. At your current age, you really should not waste any time,
because the
chance of success drop precipitously after age 35 with PCOS.
The combined use of metformin and Clomid is the most effective treatment we
have
based on recent study. I think this is why your OB doctor is recommending
that
although by definition you may not strictly have PCOS.
If you don't trust your OB doctor, you should get a second opinion. But if
I were
you, I would not waste anytime on unproven treatment.

发帖数: 402 | 4 Metformin can not be used in her case. She has no insulin resistance.
She can still use Clomid. However, Clomid will cause dry cervical mucus. At
one side, Clomid increases her chance of ovulation but at the other side,
clomid decreases her chances of implantation.
So try clomid with baby aspirin (if she opposes using asiprin, she can use
plain robitussin, i.e. Guaifenesin plus eats lots of carrots, drinks lots of
carrot juice). If no luck, try IVF immediately considering her age.

我的case可能真不是PCOS，问了这个问题几天后，居然查出怀孕了，目前都还好，有16
周了。我的fertility大夫挺让人失望的，不过还好没听她的话开始折腾呢。希望这次
顺利吧。 |
