whole story.
this is a 73 yo 280lb good old mid-westerner living a happy life until one
week ago, he had a stroke and was admit to neuro floor. only significant
history was a lap chole turn open 2 yrs ago. Since he is not able to feed
himself now, GI put a peg tube in yesterday morning. by noon he was found
aspirated the gastric feed in his room. was rushed to the ICU. later in the
afternoon, they found free air in his abdomen so general surgeon was
consulted, an exploratory laparatomy was scheduled. By the time he came to
the OR, I had trouble pre-oxygenate him despite >10min of 100% O2, SaO2
hangs around low 90s. belly very distended already. I decided to proceed
with rapid squeal induction. As soon as the induction agent injected, his
sat start to drift downward. I try first with direct laryngoscope but not
able to intubate him - the NG curls up all around in the pharynx and just
block my view. i don't have time to even pull out the NG because Sat is now
in the 70's. The only curculating nurse ran to get the glidescope and i ask
the scrub tech to break scrub and come bag the balloon. I had to put both
hand on the mask to keep a tight seal and air can be pump in against the
hugh distended belly. Finally SaO2 start to come up after dipping below 50s
, whew. With the help of glidescope, i was able to intubate his guy, but
when the nurse was pulling out the stylet, my hand was very slippery since
switching from suction and masking,- THE FREAKING TUBE CAME OUT AS WELL!
just a nightmare. changed the tube and glove, i got him tube again. surgery
was finally able to start.
what they found in the belly was that his transverse colon completely stuck
to the stomach because of adhesion from the open chole. when they put the
peg in, it went straight through the colon - what a lucky guy.

stroke. 整一个oral board的考题啊。大部分时候，我们就干monkey的活， 这不是bso
引来的是什么。呵呵。
还没完呢， 这个病人刚送回icu, ER又来一个肠穿透的， 70岁的胖老太， 整个腹部CT
就一张切片有一个5mm长的可疑白线， 在右下腹，放射怀疑是鱼骨。 切进去以后大海
捞针似的居然还给找到了，一根细细的钢丝-BBQ刷锅台的钢丝刷掉出来的， 老太太大
概饿昏头连嚼都没嚼就吞进去了...just a freakish day. i didn't finish until 3
this morning.
