good by to USMLE
Material: MTB3 and UptoDate. Step 1/Step2: 250/249 but those were 3 years
ago. Step 3: 227. Time: on/off for 3 months.
Good luck to everybody.
Not a whole lot of thoughts about this test. if you can memorize MTB3 and UW
, then there should be NO problem to pass it.
here are some thoughts about this CCS (combined mine and others)
Step I: patients: VASCULAR
Vitals/Age/Sex/Chief complaint/Urgent symptoms: N/V, Pain, fever
/Life style: smoking/alcohol/drug, etc/Allergies/Remember PMH: DM/Lipid/
HTN
Step II:
1. ER: ABCEDFGH+PE+BUOPI
2. Non-ER: PE+BUOPUI+ABCDEFGH
--- ABCDEFG to stabilize the emergency pts.
1. A: Airway, Suction of airway, cannula, face mask, oral or nasal
trumpet, CPAP/Bi-PAP, and intubation
2. B: O2, albuterol, steroids, PE (Trachea midline?, breath sounds exist
/symmetric?), SO2, ABG, CXR, and PEFR.
3. C: NPO, fluid( IVA, NSS) and blood (PT/PTT, DIC panel, and Type and
screen, and Transfusion).
4. D: Devices (monitors SO2, EKG, Bp; defibrillator)
5. E: EKG and ASA( if no aortic dissection)
6. F: Finger stick glucose
7. G: Glu, naloxone, and thiamine
8. H: hCG for all child bearing age women.
--- PE: focused if patients are unstable, or have pain, N/V; or
comprehensive
---- BUOPI ABCDEFGH
1. Blood tests:
--- CBCwDiff, ESR and CRP
--- BMP, Mg, P
--- BT, PT and PTT, DIC panels
--- Blood culture
--- Type and screen
2. U: UA, urine culture
3. Others:
1) CSF
2) Thyroid: TSH, fT4, total T3
3) Pulmonary: ABG, sputum smear and culture
4) Cardiology: Troponin, CKMB and BNP
5) GI: LFT, HBV/HCV; Amylase/lipase, and Lactate, stool
6) Adrenal: Ald/renin, 24h cortisol, 1mg Dexadron, ACTH, CTRH
releasing
7) Lipid profile
4. Panels specific for
1) Disease diagnosis
2) Differential diagnosis
3) Etiology diagnosis
4) Severity determination
5) Therapeutic plan and follow up
6) Prognosis
5. Imaging: Ultrasound, CXR， KUB， CT， etc
Step III: ADMIT ---SMTP
1. A: Activity, bed rest, ambulate, etc
2. D: Diet: NPO, normal, low salt, low fat, soft food
3. M: Home Meds to D/C (metformin) or continue
4. I: In (NG) and out (urine output/foley)
5. Transferring: ER, In-patient, ICU or Office
6. Symptoms: Pain, N/V, F, Diarrhea/constipation
7. Monitoring:
a. Interval H/P
b. Vitals
c. Neuro-check
d. Labs: H&H (bleeding) ; CBC (infection); BMP (Electrolyte), ABG;
PT/INR/PTT for heparin; LFT; PFT
e. Drug side effect
8. Therapy: Medications/procedures/Consultations
9. Prophylaxis and precaution:
a. HOB: 30c
b. DVT prevention: T and K; heparin
c. Stress ulcer prevention
d. Seizure precaution, aspiration precaution
e. IS
Step IV: Adult RATED SEX AM or RSV-Evaluation
Peds: RSVE
