besides online application and Certification of identity, what other
documents we need to send out?
In Certification of identiy, what is our USMLE ID? is that our ECFMG ID? and
what is "State Licensing Authority for which Step 3 is being taken" ?

my USMLE ID is the same one as my ECFMG ID. I did request throught www.fsmb.
org and turn out to be the same one.
I choose Connecticut, as most people do.
