New purchase or Refi?
Points?
Who pays closing cost?
Loan amount?
They all affect the rate a little.
VA have higher closing cost compare to MD, bigger loan is easy to get better
rate in no cost refi.

I was able to get less than 4.0 a while back (I pay the closing cost)

我觉得tax和cost应该没关系吧。不过不确定是不是refinance还要交tax。
