A. Birth certificate (translation and affidavits)
B. Copy all of the passport biographical page and Nonimmigrant Visa pages
C. Two Photos of 2” X 2” on the white background
D. Form I-94
E. Form I-797A Notice of Action, Approval Notice for Change
nonimmigrant Status
F. J1 visa and J1 waiver
G. Affidavit of Support/Employment Letter (For EB-1a, I think it is not
necessary)
Forms and Check:
1. G-325A, Biographic information sheet (for 14-79 years old)
2. G-1145, E-Notification of Application/Petition Acceptance. Use this form
to request a text message and/or email when USCIS accepts your form. (Clip
the completed G-1145 to the front of the application or petition you are
filing. E-Notifications do not include any personal information, including
your name, because email and text message are not secure ways for us to
transmit this information. The notification will list your receipt number
and tell you how to obtain case status information.)
3. Checks: (one or two?) Filing Fee for I-485 ($1,070 for me, $635 for my
son)
4. I-485 forms
5. I-693 form: Report of Medical Examination and Vaccination Record. A
public list of designated civil surgeons is available online through the
MyUSCIS Find a Doctor website. https://my.uscis.gov/findadoctor
Other infromation for EAD and AP card application (later)
I-765 and I-131 form: Employment Authorization and Advance Parole Card. an
Application for Employment Authorization, Form I-765, and an Application for
Travel Document, Form I-131, concurrently with or after filing Form I-485.
If USCIS approves both the Form I-765 and the Form I-131, the applicant may
receive one card that will serve as both an employment authorization
document and an advance parole document. |

the H-1B receipt number. but Pjiang's blog, it should be the red number on
the down right of visa. Who has any information, please come out to tell us. |
