take the results with you to the civil surgeon, who will sign the form. This
way, you'll save money if the civil surgeon doesn't take your insurance.

If you are in the Philadelphia and New Jersey area, then you can go to Dr.
Li Jiang's office in Hillsborough, New Jersey. I had mine done in her office
. It was a very good experience.

说了 cover immunization的，也不行吗？

万一不是network里面的话，那就要么贵，要么根本不cover。

封里和485其他材料一起寄出
