A review of your file indicates that you submitted a medical examination [
Form-I-693] completed by a designated civil surgeon. In the "Results"
section of the Vaccination Record, the civil surgeon stated that you may be
eligible for blanket waiver as indicated above. However, information is
missing from the vaccination record to establish that the vaccination
requirements have been met or in the alternative, that you are eligigble for
blanket waiver. Specifically, the civil surgeon failed to indicate whether
the spcific vaccination and/or series of vaccinations are (1) complete (2)
not age appropriate (3) contraindicated or (4) that there is an insufficient
time interval to complete the vaccination series for the following
vaccinations: TD/Tdap, MMR and Varicella.
You do not need to undergo the entire medical examination. Please take the
enclosed Form I-693 with this notice to the civil surgeon so that the
appropriate vaccinations can be administered.. Once this is complete, please
return the amended Form I-693 in an envelop sealed by the civil surgeon.

有人收到同样的RFE吗？我有两个问题问一下：
1. 我是去年6月份做的体检，但我发现我的I-693表格用的是去年1月份过期的，是不是
这个原因引起RFE的
2. 如果是医生用错表，那我这种情况医生还要重新收体检费吗 |
