5. I-131 Application for Travel Document, with copy of passport identity
pages and two photos (Immigration Photo)
6. G-325. Biographic Information.
7. I-693 Form for Medical Examination, with copy of vaccination.
8. Copy of latest Visa in Passport. Summary of beneficiary and spouse¡
¯s nonimmigrant history
9. Copy of I-94 (of last travel) and/or one on I-797 (if you extend H1-B
here)
10. Birth Certificate (Non-availability Cert, 2#s Affidavits OR original DOB
, if you have)
11. Employment Reference Letters (From Companies on Letter Head OR
Affidavits from co-workers)
12. Photos (8 Copies - Specifically taken for Immigration)
13. I-134
14. Marriage Certificate
15. Employment letter - Lawyer will take care of it from your employer.
16. Tax Return & W2 (last 2) and Pay Stubs (last 6) - Only safer of not
getting NOF/RFE on ability to pay | s*******e

1. No separate fee for EAD or AP
2. A total of six photographs for 485/EAD/AP
3. No need for tax returns, w2, or paycheck stubs
