evidence) after the filing of the AOS asking for additional proof of birth
documents thus we have asked all applicants to obtain (2) birth affidavits
to support their birth certificate for this filing. ”
有谁办过吗？ 这个affidavit去不是必须去公证处盖章? 实在不想麻烦爸爸妈妈。

Thanks for the reply. She did provide one English sample. My question is
that has anyone ever gotten this affidavit notarized in China? Do I need
prepare a Chinese copy and what else documents I should provide?
