标 题: 485 新的RFE类型

3月10号递交了485,TSC， 然后就是一直焦急地等待中，不料上一周突然收到USCIS通知
，RFE给寄出的，然后更是各种担心，不知是RFE什么东西，有点担心是体检（latern
TB),或者出生证明（封面页寄出了，没有翻译），或者是其他东西。焦急地等待了五天
，今天终于等到了RFE信，内容是：
APPLICANT'S SIGNATURE MISSING (I693体检表）
然后最后一页附上了I693的第一页，在最下面一行用黄色笔给高亮了出来。我仔细看了
一下I693上这一项签名，上面的说明是“Signature- Do not sign or date Form I-
693 until instructed to do so by the civil surgeon",当时体检医生也没提任何签
名的事，所以也就没签名了。

问为什么 I693 Part5 （civil surgeon worksheet）2.B 还有 3.B civil surgeon写
了 “APPLICANT DENIED”
感觉被体检老头坑了 I693是封起来的 自己也看不到
奇怪的是 主副同一个医生同时做的体检。主485顺利通过了。
请问这两个 part B 正确应该填什么？ 这个是需要找civil surgeon 再出一份693么？

发帖数: 136 | 3 PD current,今天收到485的RFE了，说是TB test还是要做，由过去做过的结果证明也不
行，必须在体检医生那里再做，光照X-ray也不行。而且RFE里面说“Take this letter
and your enclosed Form i-693 to the civil surgeon who conducted your
medical examination. Only the civil surgeon should open the sealed envelope.
The civil surgeon must administer the TB skin test and record the results
on the Form-I-693." 可是收到的RFE信里就只有两张纸，没看到退回来的sealed i-
693 form啊，这个表格会单独再给我寄回来吗？有收到过这个RFE的TX给confirm 一下
吗？还是他们给我忘记寄了？只有30天回复，挺急的，谢谢 |

Civil surgeons completing medical examinations between Nov. 1 and Dec. 31,
2011, should use the current form dated 10/11/11. However, USCIS will not
reject the previous version of the form, dated 7/20/10, for this time frame.
Starting Jan. 1, 2012, civil surgeons MUST use the current form, or face
rejection by USCIS, requiring the applicant to return to the civil surgeon
and resubmit the current form. |

surgeon注明。

我自己的经验：
我第一颗智齿是在一个全科的dentist那里
拔的，中国人，折腾了快半个小时，
第二颗发炎的时候去了哈佛校医院，一个台湾
人给拔的，不到10分钟的样子
后来我LP去一个oral surgeon那里拔，上下
4颗一起，一共10分钟搞定。
感觉熟练程度还是有差别。

发帖数: 748 | 13 最近看牙医，说我下面的智齿影响到了边上的一颗牙，而且已经开始蛀了，说如果不把
智齿拔掉，并且把边上那颗补上，将来有很多不好的影响，总的来说就是现在不拔，多
少年后两颗都要拔。
但是他照了X光之后，又说我的那颗智齿离神经很近，如果要拔，可能会伤到神经，也
许短期内能恢复，但也有一定几率这辈子那里的神经就被损坏了，永远好不了！
这个实在听得我太郁闷了，感觉拔也不是，不拔也不是。
大家有没有类似的经历啊？不知道该怎么办了。
By the way,我看得是Dentist,not oral surgeon. 不过他告诉我，其实拔这个智齿，
他和oral surgeon不会有大的差别。是这样吗？如果Oral surgeon基本不会出问题，请

我推荐你考虑吃点云南白药。我之前膝盖大手术后每周见两次理疗师每月见一次
surgeon，开始两个月很有效，腿从一点不能弯到能弯70多度，再后来好几星期停滞不
前，理疗师和surgeon都觉得没戏了，如果再几个星期还没啥效果得再做一次手术清理
scar tissue，我着急了就去中国城买了些云南白药来吃，几个星期后竟然神奇的可以

话说那是一个工作日。早上随便吃了几口costco的凉鸡肉，懒得热就匆匆
地去上班了。到了单位，我开始肚子痛，以为只是鸡肉坏了，去卫生间肯
定可以解决。决定忍。但是去了几次发现没有任何异常，症状却也没有好
转。大家跟我说话，我的面色痛苦，态度就很严肃，用词简短而严厉，往
往意识到要吓坏小伙伴们才在扑克脸上加上please。把大家都搞得忐忑不
安。尽可能在上午把事情安顿下。腹痛却无丝毫的好转。虽然很饿，却根
本没有吃午饭的意识和能力。遂决定下午旷工。
开车到了PCP的office要求walk-in。RN不明所以地瞎检查了一通。问了N
多的问题。开始了瞎怀疑。怀疑UTI,验了尿却完全干净。怀疑肾结石，敲
啊敲我却没反应。又去怀疑胃溃疡。在我多次指出痛点是在肚脐下边两英

。然后右手，这回扎到了，血流得手上都是。。。然后我的surgeon来晚了，还得我和
麻醉师等了半天，surgeon最后便衣手里提着内装muffin和apple的塑料带就来了，在腿
上签了initials.后来麻醉师解释说先在IV里放点让人sleepy的药，然后block腿上大神
经，然后全麻。我估计是等的太辛苦了，他刚放那个sleepy的药我就睡去了。我都不知
道怎么被推到手术室的。等醒来的时候都过了3个多小时（本来surgeon告诉我1个半就
搞定)。事后我bf说等半天还不出来以为我挂了，就不断要求联系医生，后来医生的解
释是手术器械没准备好。。。 |

首先，美国的牙医是全世界最好的，日本，欧洲的牙医都服气。OK, 那么DENTAL
SCHOOL 毕业前
10名才有资格申请矫形牙科---又回医院去学习6年，才拿矫形牙科医生执照。
问题是，你应该咨询PLASTIC SURGEON。国内的整形外科刚起步，没有办法和美国比，
就离棒子和
AV国都有距离。国内专家的意见，听听而已，不要较真。
我碰到一个白人女孩，做了面部整形手术(动骨头)，太美了，太成功了。这里有人做过整形手术
的可以冒泡讲
讲。
建议 1：花点时间，约个PLASTIC SURGEON亲自谈谈。要花很多钱的，你要是觉得花那
个钱还不如买部新车开开，那还是算了。
建议 2：背单词，你要是连maxillary, mandibular teeth都不知道，还是先不要急着约

发帖数: 1842 | 29 不是所有的保险公司都不cover，我这个没问题，只要不是给牙美容的都cover。 以前
问过一个在公司的同学，她的就不cover，她老公的（另一家小点儿公司）cover，但比
我的比例要少点。
我是牙打掉后就切找我的牙医了，他给我列了两份清单，一份做bridge，一份做
implant的。保险完后后一份只多花300刀不到，我选了后者。只是后者的保险要分两年
用。反正也要等长好了才做得成下一步晒，所以就慢慢等。但是中间有个小插曲，就是
dental implant是手术，我的牙医不能做，通常切的牙医院的那部分也不能做，要切
dental surgery那部分。护士带我切的时候人家要先收钱，以后保险公司再还给我。我
就不干了。好在平时耍得好的朋友有两个以前都是牙医，跟他们一说，他们帮我找
dental surgeon, 怎么收钱还不看他怎么往电脑头输。结果很巧，以前学校的一个
dental surgeon刚好move过来，他又给我列了一个单子，让我先切跟保险公司谈，保险
公司很爽快，算了一哈我交的钱和用掉的钱，就跟我说只要我的surgeon同意，我可以
只教保险扣过后的那部分，但让我保存好 |

能tenure，没有tenure就没有garantee的life～
但是，每一步都是他自己的选择～
他原本可以去一个没有那么competitive的school做research，一个RO1或许足够他在那
些小学校拿到tenure，他为了自己的梦想，为了成功选择去了一个top school （我猜
的）？我猜，一个RO1还不足以满足他对自己的要求？
我理解的，这个author是个非常anxious的personality，他对自己的要求非常高～ 你
从他的文字可以体会到他无时不刻不在焦虑自己的未来，career和发展。
即使笔者 是个surgeon，他多半也会拼尽全力去当department chair，或者科室的
chief，他就算是个start-up，也会想办法把公司做的更大... 因为，他对自己的high-
standards，他会把自己折腾的很累,笔者太焦虑了...
我相信，任何学校做生物科研的，总有一群faculty每天朝9晚5，依然可以拿到足够的

gastrointestinal surgeon in China. So I choose a PA rotation in surgical
oncology in MD Anderson. I found the surgeons there are great. There are
several progresses that definitely should be applied to practices in China.
It will increase survival rates of colon and rectal cancer patients.

that amount of money if not illegal. Ortho is unlike GS which leave you less
room to improvise. Real smart ortho surgeons improvise a lot and have a lot
creations during surgery. That is the beauty part of ortho surgery. After
watching one ortho surgery, you will tell how smart the surgeon is. Such
talent has less correlation with test scores though. |

problem with this.
When a surgeon operates on a patient, there is a hospital charge. The
surgeon usually gets a percentage of the hospital charge, because the
patient came to the hospital to be operated by the surgeon. For the same
reason, the radiologist should get a percentage of the hospital charge as
well.

这里有几点我很赞同：
1. “通过关系拿的面试往往不是最容易的面试”—courtesy IV往往给我的错觉很大。
除非超强推荐人，否则还是没戏。
2. “resident是干啥的？就是干活的”---我曾经以为名校有培养业内leader的责任，
在感谢email/卡里还总提“我可能是今年能培养成为scientist-surgeon的最好
candidate，最后会让你们骄傲的”。扯，人家还是首选能尽快上手，不出篓子，能活
络环境的。
3. “Surgeon 都是很nasty的”， 近似于我lab的resident说的“外科attending是最
mean的”
4. “外科口语非常重要”
几点说明：
1. 科研这点对于名校可能有效果（但也不能成为人家拍板决定的主因）；而对社区、
普通大学反倒易招致反感。面试时不要总提科研，否则不搞科研、文章少的中青年面试
官会打低分数，虽然不确定他们打分的决定性。
2. 10个外科IV有点误导大家，有我自身专业的特殊性，如果有申请外科的同学可以私
下与我讨论。

发帖数: 2269 | 40 昨天上班一个病人，after heart cath, 80% left main blockage, 90% prox circ
blockage， 需要开胸手术，回到floor后，cardiologist就把病人转给surgeon了，自
己完全退出了，一般情况下cardiologist还应该是attending,只有做完手术后
attending 会变成surgeon，cardiologist变成consult。而且这个cardiologist也没有
order any home meds，surgeon的PA看了病人，解释了一下手术并写了４行主诉，４行
plan就消失了。可怜我晚上７点开始page oncall PA, 高速她病人的情况，vital
signs， surgical plan, home meds and details of cath. Then most home meds
were resumed and heparin gtt would be started 6 hours post shealth removal.

机会：这样的机会很难得。以前有个女的到我们科临时加班做tech，中午吃饭就跟她聊
了，结果发现她是我们system里的research institute的research coordinator。她最
早以前是tech，后来熟人介绍她去做research coordinator。tech只是她额外的收入。
我跟她说了我的情况，我以前申请了几次，都在hr就被拒了或者说职位被取消了，后来
就放弃了。而且HR的申请上说必须有一年的research 经验，我每次都硬者头皮投，希
望我以前在国内的research经验能算上，但都失败告终。我给她留了我的名字，电话和
work email。她说如果有位置会替我操个心的，还要我email我的resume给她。回家后
想了想，这只是客套话，而且我不想把自己的resume发给一个几乎是素不相识的人。所
以我就没有发。
惊喜：上个星期突然收到她的email说有个位置，看我是否感兴趣，并给我她的manager
的联系方式。我当天立即send resume。

than a quarter century, mostly at Boston, Massachusetts. I am familiar with
medical field in Taiwan and biological field in US, somewhat with medical
and legal fields in US.
We have no idea what you describe yourself is true or not. As they say, "On

数学都不能叫科学， 仅仅是科学的工具而已。 而统计更不是正宗的数学， 应用而已
。 你要说美国的surgeon搞不懂数学更是不知从和谈起。
听说过“只有教不会的老师， 没有教不会的学生”吗？ 如果一个统计学教授认为外科
医生不可教， 没有天才， 你不觉得这个统计学教授有问题吗？
统计没人学不是因为学不会， 而是因为没的学， boring， tedious，and hopeless.

医学不需要有很高的数学能力，mcat甚至不需要考数学。医学主要还是需要记忆能力就
可以了。但是我们做数学题目，记忆能力没有什么用处的。

breast clinic for bilateral fullness and tenderness more on the right than
