发帖数: 352 | 7 医院里的clinical pharmacist每天写十几到二十多个mini soap note根本不象在学校
里写得那么复杂。上学时候那些作业要求太苛刻了，这要扣分，那也要扣分得，一写一
大篇。如果日常工作中也那么写太没效率不说，人家一看那么长就懒得看了。医院里倒
没有 template，关键是要调理清楚，言简意赅，突出重点。
我接触的Ambulatory care clinic的soap note倒是需要比较详细，不过都有
template, 一些S&O会自动填入，你只要写A&P就好了。 |

我觉得我们说得差不多呀
dig当然能用，但我们觉得应该做为last resort
spironolactone先试，如果BB，ACEI和spir都不能很好的控制，再加dig
不过临床中，我倒经常看到连BB和ACEI都没同时上就很早的用dig了。dig在以前用得非
常广泛，用得早的病人就一直这样用下去了
非常同意你说的在写SOAP的时候，plan里加上alternative therapy or future plan
if the recommended therapy fail. Actually, it is required for our SOAP exam
in P2/P3 years.

Detail:
Dove single bar soap is 99 cent each at Safeway and there is a promotion for
