narcotics只能缓解一些疼痛，但是还是能感到疼，而且会使人昏昏欲睡。有些人觉得
好是在宫缩间隙可以睡一会儿，有些人觉得不好是因为被宫缩疼醒会更加难受，还不如
清醒的时候用呼吸法。
好像在各个stage都能用，但是不能在快生的时候用，因为它会通过血液到胎儿，如果
快生了再用，胎儿会有些麻醉的效应，对呼吸有影响
最常见的当然是epidual了，不过听说还有通过IV输入的narcotics? 不知道narcotics
是必须输的吗？在哪个labor stage输呢？还是说如果选用了epidual,就不需要输
narcotics了？就要生了，心里对到时候会用什么药还很没底。哪个姐妹能科普一下，

的也不少)，就是钻FDA 和国会审批监管程序太冗慢的大漏子。
伴随着马里瓦纳合法化的大潮，我老估计再过个二十年，娱乐性的personalized
designer narcotics将会像iphone一样普及。FDA和国会可能一开始要管，后来发现化
合物结构的variation太多太复杂根本管不过来，每年光打官司在化合物结构上扯皮就
把预算花干净了，估计就懒得管了。

吃福利最多的是主流人种的低收入的那些，但是这都不是太大的问题，只要经济增长了
，吃福利的人就会下降。关键是经济增长问题。
把眼睛老是往某些少数民族盯，真是没出息。
但是美国政府开支中最大问题是medicare，这个是67岁以上医疗全包，很多费用是在临
终前3个月上面花费了巨大金钱，这个和什么种族根本没有关系。美国政府其他开销不
是大问题。所以Obama的Obamacare方向没有问题，就是timing有问题。

你才吃那么点，根本不算overdose的
医生下手也挺重，最多norco吧，一来就vicodin。。。我还以为你长期用鸦片类制剂止
痛呢
vicodin会直接抑制呼吸中枢的，不是你睡着了就呼吸弱
所以我觉得你算是对narcotics过敏的，以后要记得跟医生说
hyperventilation是医生诊断的吗？呵呵
我只知道呼吸被抑制一阵子体内ion gap会变化，离子平衡会被扰乱，也可能产生麻木
感的 |

有两个例子与大家分享：
1. 椎间盘手术。 已经有足够的统计证明手术与保守治疗效果差不多， 但我看到一个
统计， NYC的手术率比montana低很多。 原因之一是montana病人少，医生不做手术无
法生存。 所以obamacare想限制医生的手术数目。
2。A patient was seen 2 yrs ago in ICU due to overdose. He had been on
Narcotics for back pain since 2000. After the respiratory arrest, he stopped
Narcotics. 2 wks ago, He told me his back pain has been gone since he
stopped opioids.

厌她，因为她每次都要以不信任的态度仔细的count narcotic， 而且还要我们跟着她
。我的preceptor好讨厌她，因为她总是害得大家晚下班。昨天我自己跟着她去count
narcotic，没想到她居然态度非常rude的威胁我说“你是sutdent nurse吧。我一个人
要负责整个floor的4 个unit， 我很忙的。 你以后要在我来前5分钟count好nacotic，
我没有时间“。但是实际上我们都会在班前和班后各count一次的， 是她要每次不厌
其烦的不信任我们的去count。她说上次她当班的时候nacotic的数不对。我跟她说我只
是student，而且我跟我的preceptor一起count的，让她跟我的preceptor说。她却说“
不，我就跟你说，你要在每次下班之前负责好count的事情， 不能出错。否则的话，我
就向你们老师反映了。“可把我给气坏了。 每次count的时候都是对的，她当班的时候
数不对了，那是她的事，跟我有什么关系。这人怎么这么差劲呀。我觉得她是不好 |

提到最短的只有一个星期，但那个人原先就在那里做NA的。后来，我真的只用一个星期
完成了orientation, 但我发现后来来的新护士也是仅用了一个星期的时间。以后的时
间就是边干边学了。我很庆幸我身边有几个非常有经验（很多人都曾经在医院工作过多
年）、也非常professional的护士，她们总是主动地教我一些新的东西，比如taking
order，admitting pt，discharging，填写病人死亡证明，填写各种assessment
sheets,等等;而且，只要我有问题，她们总是非常耐心地回答我的各种问题、或者手把
手地向我示范各种skills。说实话，如果真有离开的话，我内心一定会非常内疚的，所
以，我一直都在是否要离开上犹豫不决。
至于一天的工作流程，我就简要介绍我所在的NURSING HOME的早班流程吧。
1）与晚班护士一起清点你所的drug card的 narcotics。我一般都帮其他护士清点，同

打电话给er之前应该先跟病人谈谈，至少问问他疼痛情况如何。刚从er出来的，啥问题
都可能发生，具体情况具体分析吧。
既然他一直在用suboxone,说明他是主观想戒的。在跟他交谈中一是了解病情，二是给
他提供写其他的pain reliever alternatives，鼓励他坚持戒下去，先用用non-
narcotics试试看，如果不work，再考虑上narcotics
病人健康和身体状况永远第一，高于刻板的所有条款框架
你的出发点是好，但缺乏跟病人之间互动，没有做到药剂师provide patient
education这个重要职责

不过这两个病人的共同特点是opioid dependence， 我想说的是， 我个人对long term
Narcotics的使用极其反感。 我所有的病人，除了palliative care ， 没有一个是使
用scheduled long term narcotics。 因为选择只有一个， 或者是接着看我， 或者是
pain management。 当然我本人从来不refer病人去pain clinic。

教授的想法有他的道理， 不错， 人死了， 一了百了， 同情归同情， 不良的品质是
没有借口的。 怎么评价名人？ 给孩子们一个什么样的榜样应该是一个标准。
这些人其实都是病人， 上个礼拜一个小年轻到我的诊所寻求2nd opinion， 其实是无
数个opinion了。 他因为腰疼一直吃narcotics， 我很简单地说：
1.手术不可能，腰椎好好的；
2.停用narcotics
他站起来就走， thank god.
毒品有它的市场， zero tolerance应该是我们每个CMG必须具备的观念。

的。 我有个病人，只要percocet和norco，要最大剂量的那种。 他出过车祸，有过
stroke，推着walker走， 每走一步都异常痛苦的样子。 我们director第一次看到他，
就告诉我red flag。 后来的经过很多了， arguing and fighting。 最后我们发现他
根本没有用药，怀疑他把药给卖了， 我们帮他算了一下，按street value， 每月他的
卖药收入接近2k， 我最后一次见他的时候明确告诉他，我不能再开任何的narcotic了
， 他问， 痛怎么办？ 我答， 因为你没用narcotic也handle了，就用你平时的方法吧
。 当然，我还是开了NSAIDs给他。之后， 他再也没出现了。 |

只有美国才这样给病人narcotics， 美其名曰不让病人受痛的折磨。 我对terminal
illness给止痛药是坚决支持， 但对那些良性慢性疼痛患者，30粒一个月是我的上限。 |

indications. It is mostly you PCPs who dumped the addicted patients to us. "

"Same for pain management. Other than seeing the chronic pain pts, some will
see acute pain, help us with postoperative pain management. Some will see
cancer pt, help with cancer pain, which is much needed. They also do a lot
diagnostic and therapeutic procedures. Of course, there are some bad pain
docs, some even got arrested, just like every other specialty. But you can't
just simply point fingers at the entire specialty."
