consider thymectomy if this has not been done,
anticholinesterase drugs such as neostigamine or pyridostigomine is only
symptomatic relief and if you overdose them the symptoms would get worse,
plus those drugs wouldn't affect the couse of the disease,
if he already had his thymus gland removed then maybe steroid and IVIG and
plasmapheresis,
the new drug would be cellcept, not sure how effective that would be though, |

hypersensitivity, but Th cells can be involved in activating b-cells.
Thymoma is often associated with myasthenia gravis.
It is often treated with acholinesterase inhibitor or immune modulating
drugs. Plasmapheresis & thymectomy are other options. It is certainly a
devastating disease, but still a disease that can be managed to a large
extent. |

发帖数: 658 | 7 一般用胆碱酯酶抑制剂，如果比较严重要用激素，免疫抑制剂如硫唑嘌呤，环孢酶素，
环磷酰胺等。有没有做CT,如果有胸腺瘤,切除后病情会有很大好转。 另外还有用免疫
球蛋白和血浆置换。 google myasthenia gravis.
上述的药都要医生处方的。 |

是只是前期，后来引发悲剧的是一块桔子。某天他在家里吃桔子，不小心被一块桔子卡
住喉咙，但是因为他有MS,没有办法自行把桔子咳出来，结果被活活窒息到脑损伤。更

好像是关节外面的韧带. 查了下医学手册，有这么几种:

贝：十号发了那是个周一，周三我就给朱令他爸爸打电话说，我看到提问里还有说是铊

贝志城：很多医生很激烈地发表意见，说一定要作这个化验，因为她的症状太像铊中
毒了。
朱令父亲：贝志城给我打了个电话，说有人觉得是不是就是铊中毒。

Amyotrophic Lateral Sclerosis等等。 很多神经方面的病都很难确诊，因为不是靠验
血啥的，都是based on symptoms and rule out other possible causes.
如果是人的话，肯定会拍脑部ct/mri，看是不是脑部有tumor，压迫到神经；再看有没
有中枢神经系统烦人感染；再排除中风啥的。就不知道对于猫，有没有那么完善的检查
手段，有没有那么完善的理论和治疗方案。
送专科，赶紧的，neurologist, 普通医生根本没办法。 |

of chest was reported as unremarkable.
PMH: Asthma, seeing a pulmonologist who reviewed CT and agreed with
radiologist's report.
Soical. Smoker 1 ppd. No ETOH/Drug.
FMH: neg for CA.
ROS: Not remarkable, no diplopia, dysphagia, no fever, no joint pain, no

他的疲劳感不代表他真的无力， 实际上他的检查真可谓正常。 我只是发现他的右手握
力有些弱。所有的检查， 包括myasthenia Gravis都是正常。
我决定做brain CT，我相信他的症状是真实的， 我的检查是正确的， 结果发现脑肿瘤
。 我当时惊呆了。
我第一反应就是叫放射科，让他们把肺部CT重新读一下， 结果还是single
lymphadenopathy。 我让他们biopsy后， 证实lung small cell CA.
他得的是Eaton-lambert syndrome， 是肺小细胞癌paraneoplastic syndrome的一部分。
所以每到他的生日， 他就给我送一张card。 我nurse刚跟我说的。

发帖数: 722 | 25 哦，我说这俩都是食管mobility disorder,功能障碍，而做胃镜要给sedative, 对这俩
病就是一种药物缓解，所以很容易漏诊。swallowing evaluation不知道做没做
Barium swallow study？不过即使Barium swallow study，诊断价值也远不如ｍａｎｏ
ｍｅｔｒｉｃ ｓｔｕｄｙ，因为后者是直接测量esophageal mobility function．
如果楼主是中老年女性，大概还要考虑Scleroderma，不过感觉楼主年轻英挺，所以就
免了吧。:-)
MG (myasthenia gravis?) 首发、单发为吞咽困难的多见么？这我可不敢班门弄斧了。
就是好奇您老这神内大拿为嘛把患者转给Acne?:-)

小结一下：病人表现双眼闭合不全，突发。
分析：病变累及肌肉或及所支配的神经。
可能性：
1，系统性病变致双侧病变：如：神经纤维瘤病NFtype 1,2.type 1可有双侧性神经胶质瘤。
2，感染性或脱髓鞘性病变：如：多发性硬化MS，神经脱髓鞘性变，Lyme disease。
3,肿瘤性病变：如：小细胞肺癌产生的paraneoplastic syndrome-Lambert-Eaton myasthenic syndrom.
4,自身免疫性疾病：myasthenia gravis.
老大，再给点clue吧，好让我们narrow down呀。 |
