Chides FAA For Not Completely Addressing The Issue
NTSB Chairman Deborah A.P. Hersman (pictured, right), in testimony Wednesday
before the House Aviation Subcommittee, Committee on Transportation and
Infrastructure, discussed the dangers of aircraft flying in icing conditions
and highlighted longstanding Safety Board recommendations that have yet to
be adopted by the FAA to address the issue.
Reducing the dangers of flying in icing conditions has been on the NTSB's

From NTSB Chairman Deborah Hersman:
The target speed for the approach of Asiana Flight 214 was 137 knots, and
the crew can be heard on the cockpit voice recorder acknowledging the speed,
Hersman said.
But the speed was significantly below 137 knots, and "we are not talking
about a few knots," she said.
At about four seconds before the plane crash landed, the pilots received an
"oral and physical" warning inside the cockpit that the plane was on the
verge of an aerodynamic, meaning it was about t...

A federal agency on Tuesday called for a ban on all cellphone use by drivers
— the most far-reaching such recommendation to date — saying its decision
was based on a decade of investigations into distraction-related accidents,
as well as growing concerns that powerful mobile devices are giving drivers
even more reasons to look away from the road.
As part of its recommendation, the National Transportation Safety Board is
urging states to ban drivers from using hands-free devices, including

(Reuters) - The Asiana Airlines Boeing 777 that crashed at San Francisco's
airport on Saturday was traveling "significantly below" its intended speed
and its crew tried to abort the landing just seconds before it hit the
seawall in front of the runway, the U.S. National Transportation Safety

New details in the accident investigation that were revealed Tuesday by
National Transportation Safety Board Chairman Deborah Hersman were not
conclusive about the cause of Saturday’s crash. But they raised potential
areas of focus: Was there a mistake made in setting the automatic speed
control, did it malfunction or were the pilots not fully aware of what the
plane was doing?
One of the most puzzling aspects of the crash has been why the wide-body
Boeing 777 jet came in far too low and slow, c...

July 16 (Bloomberg) -- As the Asiana Airlines Inc. jet neared Los Angeles
International Airport, Captain Vic Hooper told his Korean co-pilot to make a
visual approach, meaning he’d manually fly instead of letting automation
do the work.
The co-pilot froze, leaving them too high and off course, Hooper said about
the incident, which occurred several years ago. Hooper said he had to take
over the controls to get the Boeing Co. 777 back on track.
“I don’t need to know this,”...

Hersman also said the aircraft's stick shaker – a piece of safety equipment
that warns pilots of an impending stall – went off moments before the
crash. The normal response to a stall warning is to increase speed to
recover control.

Associated Press

Airport technology called the Instrument Landing System, or ILS -- which
normally would help pilots correctly approach the runway -- was not
operating at the time, according to a Federal Aviation Administration
bulletin.
"There are a lot of systems that help support pilots" as they fly into busy
airports, Hersman said. Some of these systems alert the pilots.
"A lot of this is not necessarily about the plane telling them" that
something may be wrong, she said at an earlier briefi...

(Deborah

The National Transportation Safety Board, which probes plane, train, ship
and highway crashes, wants to expand its purview to cover commercial
spacecraft mishaps and accidents.
This week, the safety board for the first time asked Congress to explicitly
give it primary investigative authority over accidents involving commercial
space vehicles. As part of the request submitted to the House Transportation
subcommittee on aviation, Deborah Hersman, the board's chairman, also

Flying in icing conditions remains a "most wanted" safety improvement on the
agency's list with a "red" classification indicating an unacceptable FAA
response. NTSB investigated 69 accidents involving U.S.-registered aircraft
and runway or surface icing in 1998-2007.
The National Transportation Safety Board's chairman, Deborah A.P. Hersman,
told Congress Feb. 25 that "we must establish a system that minimizes pilot
fatigue and ensures that flight crews report to work rested and fit for duty
