Respiratory, Skin) to give you PE & CT (Prosthetic valves, Endocarditis,

时事考点一直是USMLE出题滴侧重 点之一，个人认为今年最热滴时事考点当属最近造成
多人死亡滴methylprednisolone acetate惨案。考题可能在step1 2 3出现，具体细节
可能包括:
1. Aspergillus老重点了，估计今年基本上每个人都能碰到它了。重点复习一下吧。
2. 冲击高分滴同学注意了:Exserohilum rostratum and Cladosporium.不用背下来，
大概能认识就成，当你碰到一个WTF问题滴时候，如果选项你都不太熟，就选一个跟上
面两个相似滴选项吧。

Step3考场壮烈回来。第一天挺累人，最后一个block甚至做到有心无力状，看着题反应
不过来，导致来不及做完。非常没有信心能否通过。所以match之前先考Step3的童鞋，

UW 52 online cases : 基础。最好复习早起找高手一起过一遍，尽快摸熟软件，进入
状态。第二遍找一位水平相近童鞋过一遍，不断总结protocol. 考前迅速做一遍，熟练
运用protocol.
UW 41 offline cases: 我当时和partner每个case仔细做一遍，虽然不像online有反馈
，但对练临床思维，补充protocol非常有帮助。最后考试与41 case有异曲同工之处，
遇到复杂case也不慌神。

的习题非常有用，我没时间看完整本书，但把每道题都做了，考试时遇到过类似的题。
里面的应考技巧也很管用。还有，我觉得Pharmacology最重要的是记住药名的后缀，就
能联想到Action和Side Effects了。

发帖数: 88 | 26 我在这家NH上了整一年了，说实在的，工作真的不难，老人家们基本都还stable，一个
月下来，恐怕也说一两起emergency,不外乎发个烧什么。可是我是越上这个班越是那个
不高兴啊。为什么呢？有几个原因：
1）8个小时的班事儿太多。我还算快的，如果没有admission,emergency or incidents
的话，基本能按时回家。可是3－11 的班admission太多啦。结果呢，要想完成任务就
只能加班啦。这样一来，老板找你谈话了，怎么回事啊？怎么会有这么多overtime 啊
。他们也知道我们的实事求事的回答无懈可击。谁愿意晚回去啊，大半夜的，不能按时
睡觉，特别伤害身体。我们是没有deferential的，fixed rate. 更可气的事，这帮资
本家们玩起了偷的把戏。我们是打卡的，花在工作上的时间赖不掉，可是工资上部份的
overtime时间竟然被调包到regular hour上，这样就少了那个50%。我真是无语啊！NH
的工资本来就低，那个50%他们也要偷！

，抗生素又不起作用，就上empiric anti-fungal，我没仔细查，可能没说对哈
不过你的思路是正确的，是有感染真菌的可能性
LZ说化验一切正常，不知道是否包含白细胞，band之类的也正常

primary site. Lung ca is an evil disease in the way that the metabolic
presentation varies a lot in different cell types. BAC and carcinoid are
well known to be hypo metabolic.

infection， 但长期这个情况可能性可能更复杂。 你可以试试下面的方法：
1. Cranberry juice， 可能对真菌有效
2. 高纤维饮食
3. 到药店买一些治疗腹泻的药物， 这些药物包括了高纤维和anticholinergic成分，
后者可以抑制肠道的过分分泌， 当然也可以导致便秘。
觉得你这种情况跟有些人过分流哈喇子差不多（sialorrhea）， 只是部位不同而已。

sialorrhea现在可以用botox治疗， 如果能够确定某个肛肠腺体的问题， 我想治疗方

I will fast forward a little bit.
All the cultures you can think of are all negative.
His coags were cold normal at the time of presentation and several days
later. His smear showed some spherocytes which is consistent with hemolysis.
There was no schistocytosis.
His white cell were slightly elevated with slight neutrophilia.
His platelet count was normal but by day 3-5 into the admission, it dropped
to 40-50K.
