difficulty in switching jobs (after all, what you described is pretty
much happening in every field:( it sucks to be a newly minted PhD too),
their huge debt exacerbate the issue dramatically, esp. for ppl w/o
the security of GC/citizenship. That makes a lot of sense.
Maybe the time for postdoc has finally come:)

1. 好好享受最后的日子，多陪陪家人。
2. 好好放松加好好提高EQ:)
3. Intership开始以后你就知道睡觉不够是最大的问题。现在睡足可以至少扛一个

5. 练习英语， 说 第一印象很重要。
6. 然后练习 CASE PRESENTATION。
7. 复习一下CS，以前是为了应付考试，现在是真的要上阵了。

1。如果糖尿病人肾功能不好而用胰岛素，就要格外注意低血糖的可能。不仅仅胰岛素
的用量要相应减少，而且紧急给药时的间隔也要相应延长；另外，就是 sliding scale
要相应调整。常规的sliding scale，血糖每升高50，要多加2 unit 短效胰岛素，而肾
衰病人，就要从1 unit开始，再相应调整；另外不要从BG>120就开始给胰岛素，而是从
BG>150开始。总之血糖宜高不宜低。
2。用长效胰岛素 (Lantus, levemir) QHS的病人，如果第二天有procedure（譬如手术
）而需要凌晨空腹，甚至连午餐、晚餐也免了，一定记得减量或D/C头天晚上的长效胰
岛素，
否则就可能因为第二天不进食而低血糖。如果麻醉的情况下低血糖了更麻烦，因为病人
没有临床反应，所以可能不被发现而造成脑损伤。当然，好的procedure操作者会监测
血糖，并相应静脉补糖，但是也有可能会疏漏而出事。
3。如果用静脉碘造影剂，一定注意肾功能。如果肾功能不好，就要仔细了：如果需要
诊断PE, 就用V/Q scan 取代CT-angio; 如果做腹部CT，那么就不用静脉造影剂，只用

以有较大不同。最近有几位同学对这个问题比较关心，我说说个人体会，算是抛砖引玉
，也给今年面试的同学做个参考。
1. Patient population
贫困区的病人往往年轻些，20-50岁居多。因为大多没有保险，重病人多是因为平时得
不到适当的医疗所导致。AIDS/吸毒病人比较多，另外也经常见到Asthma exacerbation
, Sickle cell crisis, 严重感染等。Resident的自主性比较强，procedure比较hands
on一些。气管插管，腰穿等等很快就certify了。不过抽血等scutwork往往也偏多。这
里的病人往往不太按时随诊，相应而讲纵向观察病情演变的机会略少。
富人区的医院病人往往年龄偏大，70-80很常见。病人除了入院的主要病情, 既往史往
往一大列，HTN/DM/CAD/CKD...单个病人花费的时间长一些。这里的scutwork相对比较

国临床系统经历过的同学还是比较难以把握。应澜静医生之邀，我大致介绍一下我们医
院大内科和专科的日常工作，给正在面试的同学做个参考。我的经验主要来自我受训的
医院，大同应该不差，小异再所难免。其他同学和前辈如果觉得有以偏概全或者误导的
地方，还请及时指出，让我和其他同学共同受益。
中美医疗体系之间存在诸多不同，包括医学教育，保险，住院医系统，以及医生和医院
的关系等等。
就内科而言，一个很明显的方面是美国的医院里有一个general medical team，既内科
住院医的训练平台，而中国没有。国内医院相对而言是由各个层级化了的专科系统组成
的。例如对于COPD exacerbation而言，国内的病人挂呼吸科，收治呼吸科病房，甚至
呼吸机也在呼吸科上。美国的病人一般则由PMD收普内病房(General medical floor)，
请个pulmonologist会诊。由于护理的关系，如果需要介入性呼吸机通气要到MICU管理

发帖数: 1200 | 25 神人讲座啊，270多的高分+如鱼得水的面试。总结如下，多谢多谢。
1 考试：东西理解，要熟，没捷径。具各人情况，看待别人的经验。目标不同，成绩要
求不同。至少要有一门考得非常好。
什么时候考试，要看准备take 多大risk。时间短，波动性大，分数不稳定。
2 申请：7/1 开始，可以上交到ERAS，之前应完成推荐信，OB之类的。
（1）做OB可以增加interview数目，同时会增加interview的信心。
抓住机会做OB或ext.，同学，同事，朋友，买。可增加对美国临床的熟悉。要表现的谦
虚，肯学，勤快，不多事。找好的attending.
（2）推荐信，最好有想申请科的医生的，实在没有也可以。推荐信要好，没有好话，
不能用，就是牛人的，如果没有好话，一定不能用。实在没有的话，找中国人写的也行
，但一定要确定是好话。
推荐信重点 （由重到轻） 1 好话 2 牛 3 最好在美国的医生的，没有的同学写的也
行，但是话要好 总之，谁的话好用谁的，不论这个人来自哪，干什么，只要是好话，
就首选。争取可以看到推荐信的内容。遵守eras规定，但是要灵活掌握，一定要知道是
非常强的推荐。

一下大部分文字是考前两月写下的自己的感悟，由于考前高压，可能会条理不清，词不
达意，见谅。
不变应万变
对于难题要做到镇静，想想我不会的题目，多数人也未必会，镇静，镇静！镇静下来之
后才能慢慢寻找解题思路，难题的难点在于，题目信息不熟悉，备选答案内容不熟悉，
不要慌，想一想，出题者的思路在哪里，用自己熟悉的知识点去解决不熟悉的信息，这
就是apply的过程，比如genetic的题目。以不变应万变。
对于不会的题目，不要粘题，没有思路，跳过去，回头再想。有时是瞬时block out一
叶障目，后头再看的时候会发现豁然开朗。不值得浪费太多时间在一道题目上面。
关于排除法的应用，首先排除自己熟悉的选项，再排除自己不熟悉的选项，如果自己熟
悉的选项不能排除，怎么能排除自己根本看不懂的选项。解决方案：如果最后自己熟悉
的选项不能排除，还是选择自己熟悉的选项。因为太超范围，太偏太远的东西考的可能
性毕竟比较小，较大点的可能是可能是自己熟悉的，但是自己没有做到很好的apply或

the family. update the family daily (efficient)
Update the team about the patient’s progress (team coordination)
consult— call in the morning, as soon as possible
Finish to do list before going home
Pony: 讲了很多精彩的故事。 nurse— 满足reasonable request 的同时不能损害病人
的利益。早晚班跟护士update 一下。
搞清楚rotation, 在rotation开始前跟chief, resident自我介绍。

主要提到了如何提高工作效率，take the orders，不出错，建议大家在开始之前掌握
好以下常见病的处理，不要临时去翻书：
asthma attack, COPD exacerbation, pneumonia, CHF, chest pain, ARF, drug
overdose, stroke, GI bleeding, DKA.
还有就是高钾低钾等电解质紊乱的处理常规。
also watch the videos of PIV, ABG and pap smear.
另外对于内科要求的3个操作，建议多看几遍NEJM的视频。

NYHQ做OB还有一些问题，所以希望写下自己的经历，对后来人有所帮助。
首先非常感谢ACAP的帮助，让我们在NYHQ能一直有机会。我来到NYHQ医院做了一个
月的OB. 虽然知道这个机会来之不易而且还有很多XDJM在报名排队，我在来之前还是有
些犹豫。因为可以在我所在的城市里的教学医院做OB，为什么要离开家人和工作，来到
一个社区医院做呢？其实来到第一天的见闻就打消了我所有的顾虑，一个月确实挺值的
，只是没有机会再做长一些时间。
NYHQ位于法拉盛区（Flushing），医院大厅标注着英文、中文、韩文、俄文等等，
足可以看出医院的多元化（diversity）。第一天先参加orientation和infectious
control training， 然后去街对面GME办公室递交application form等paperwork，再
回到医院办理ID卡，到五楼找chief resident报道。一批observer一般有四人， 两周

成绩出来了，涉险挤入了９９队伍。总的来说，准备的不是很充份，详见以前贴的＂我
的step ３经历＂，主要是在吃CK的老本；其他乏善可陈，成绩单里唯一的一个星号是
给了CCS，这里就说一说CCS好了。
以前总把CCS和CS混淆，其实这两个有一个共同点就是open choices和uncertainty。在
这总场合，最重要的可能就是要有一个严格的protocol，这样可以尽量避免遗漏。考过
CS的同学都知道LIQOR AAA和PAMHUGSFOSS的重要性。CCS也一样。网上比较流行的是
Neeraj的protocol，用google一搜就能找到。非常有用。我把他的protocol稍微整理了
一下，自以为是更加简洁有条理一些。给几个朋友用了用，有的朋友觉的没有时间应用
，有的朋友用了觉的还成，让我贴出来，最后的决定是如果考出来还行就贴，要不就不
出来吓人了。
象CS一样这个protocol充满了mnemonics，开始考试之前第一件事就是把所有的
