这样的：
首先去city court 申请改名的court order（sinor），需要宝宝父母同时到场。十天
左右，花费$92。 如果改名不是因为父母离异之类的原因，这个order是必须的。
然后凭court order去当地Vital records office领取有新名字的birth certificate.
（same day service需预约）
之后去SSN office改ssn card上的名字
差不多就是这样，希望对你有参考价值，如果不清楚，可以打电话去咨询一下你们当地
的Court clerk有关court order的事项。 （电话可以在当地的city court的网站里找

1. 先到town里clerk office拿一个表格（这个估计上网也行，但是我是第一次办，就
亲自去问了）
2. 然后填表，给OB填，现在的表格比较严，好像必须有辅助走路的才qualify，我的OB
说上周她们medical group里面开出来的application，被拒了2次
3. 其实我也不是特别需要辅助物，就是有risk，limited activity，OB就把病症还有
risk写了，认为我在平时的生活方面，需要给予一定的帮助。
4. 我就拿着form，再回到clerk office，就办好了，当场就拿了，是临时的（红色的
残趴）
我觉得我们Town里的小秘太nice了，医生给我写的是due date之后的一周（预估一个
recovery period),我的小秘直接给我了6个月的permit，想都不没有想。:)
现在再去上班，或者买菜，就真的不用走的特别长，还是自己能manage的，觉得虽然美
国人看怀孕月子啊，其实比较大度（就是啥都可以，我自己的OB也常说没有那么多讲究

1、医院出生纸
出生住院时有人上门办理，可同时办理社会安全卡（SSN）
办理加急要出示2周内出发的电子机票的纸质材料。
2、爸爸委托配偶办理护照的委托书DS-3053公证
材料：委托书DS-3053、出生纸扫描打印件
地点：公证处或美国驻华大使馆

一直没有考虑出生证三方公证的事情，周一才认为有必要需要公证下出生证，以防回国
办户口需要。周一马上打电话county clerk office咨询apostile。不用邮寄，可以直
接带出生证去office 5分钟搞定，5刀 check,money order,cash随意.接下是州政府
apostile。可以选邮寄到 albany。或者直接walk in: Albany or NYC. 我电话了NYC
的department of state office,说可以直接办理. 然后坐火车直接从我们那农村旮旯
到penn station，出站出 w 33rd ST.& 7th AVe,坐2号线 （downtown方向）15分钟，
直接到出fulton st. 走2分钟到123 william st. 二楼大厅，右拐，看指示就可以了。

clerk签发的，已经认证过的。州务卿的不需要孩子去，邮寄办理就是慢一点。以加州
为例，邮寄办理8天时间，in person的10分钟办完。
还有一点，出生证明每个county发的版本略有区别，有的county的只是一张纸，不是
authentication，这种情况的要先去county做county clerk registration.
我们county比较好，直接就是发的就是州务卿认可的可以直接签字的，可以直接拿去州
务卿办公室签字。 |

一天法官会裁决充许改名。记得我就按时间去，clerk就把盖好大印的文件给我，法官
的面都没见着。
还有都是我自己去的。文件上的父母双方签名好像是要notarized.
等等，好像LD递文件时去了一次。年代久远，记不清了。但绝对没见着法官, 还挺遗憾

我上法学院的朋友说，法学院的毕业生的最高成就是去做US ATTORNEY。 给法官做
CLERK，年薪4，5万，工资只是那些进顶尖律师行的一半，甚者三分之一。所以，怎莫
也不算接近最高成就。 但是做CLERK 有个好处，可以积累人脉和知识，为将来自己出
来开业能积累不少有用的经验。
拿一两个特例，是说明不了问题的。就好比BILL GATES， STEVE JOBS， QUIT 了大学
，成了百万富翁。但是对于大部分人来说，还是念完大学更好。

是也比没有比例要高一些，是不是？你跟Bill Gates一起混没有被他看上，打工不被领
到看上，泡妞不被美女看上，elite社会，比拼的就是人际关系。

其实学理科的一个普遍误区在于：对一个问题的看法非黑即白，一定要找到最优答案。
现实生活中，答案是多样的，社会是多元化的，最优答案也可能是有不确定性的。
所以，数学并非是让一个人在，中国或美国社会上“解题”的最佳利器。

你说反了。。。在美帝，对于大部分 STEM track，数学在大学录取时没啥用。。。因
为 SAT 数学，只要够细心，数学傻子都能考高分。。。过来大学录取的坎后，数学反
而更有用。。。当然我估计你不是 STEM track 的，不理解数学的用处也是自然。。。

2. There is a 72 hours waiting period, so you cannot have the actual wedding
ceremony within 72 hours
3. Invite parents over before the actual wedding ceremony and get married in
a church or in a court house in front of a judge, A.K.A. the actual wedding
ceremony

on your case. Most of time, you will contact his clerk not himself. It
really depends on who is working on your case. In my EB-1A, I wrote all the
recommendation letters and sent them to the lawyers for their professional

Their charge is higher than other firms. However, for the guys need the GC

婚的法官签字，county court的人签字。但是这个在IO那里很可能不算certified copy
，我明确知道有人485交的就是marriage license copy, 收到RFE要求certified copy
。所以最稳妥的做法是去county clerk office order一份certified copy，这个copy
其实就是county clerk office的人在marriage license下面加一行注明是certified
copy，然后再加签字，再加盖章。
别的州是什么情况，我不了解。 |

second time I went back, still the ticket wasn't shown on the computer. So
the clerk told me this ticket would be officially dismissed one year after.

把罚金先交了，支票上写上not guilty,request TBD)。 然后填了表赶紧寄过去就行了
。tbd form上不用多说，这个事情就是看你运气如何，如果开罚单的警察没respond你
得tbd, 你就赢了，罚金退回。
tbd了以后警察回了，那基本你是输的，他们会给你一个出庭日期，你快到出庭日期前
一个月，要求reschedule到更晚的日子，反正有可能的话就尽量拖，然后就是去法庭撞
大运，如果警察哪天没去，恭喜你，你又赢了！啥事都没了，罚金如数退回 。警察去
了，那你乖乖上驾校等等。。

初犯就可能撤销或减半罚款，并不记点；若是强横的法官就没有任何减免，还记点。
在明州，clerk可以让你多交钱换hold；但clerk可以像法官一样减免罚吗？他们的权利
应该不一样吧？
