Attendance quiz得0分，也可以得到Attendance的60分。这个老师很强调Attendance。
你看他（她）有句话：
Attendance： Woody Allen said “90% of everything is just showing up.”
他貌似想给Attendance算90%,呵呵。
我估计他有可能给Attendance算80%,就是人去了算60%；交了quiz的卷子算20%,不管你回答是否正确。 |

一些，还是觉得这个人过于Intense？
今天Attending花了一个小时点评（当然多数是批评）我的第一个H&P （这个医院的系
统跟我上个Rotation完全不同），刚好第一天就Oncall, 没有人教我们怎么用这个软件
，所以有些字句还没仔细斟酌就稀里糊涂的贴上去了，有的地方格式也变了，当时不知
道怎么改，后来就忘了这吗事，主要是我上个Rotation的Attending从来不看我的Notes
, 只看formal writeups, 所以也没太当回事，没想到这么快就back fire了。这个
Attending的标准真是高啊，我觉得至今还没有看到过合乎他要求的H&P呢，他对于普通
admission 的H&P要求绝对高于我原来Attending对于Formal Writeup的要求 （除了
Discussion部分），按说我原先那个Attending还是大牛呢。 接下来的几个Oncall肯定
一点都睡不成了，都得花时间考究H&P了。呜呜。。。，不知道该抱怨还 |

较Agressive，又占用了剩下的一半时间，其他时间就是Update各个病人的情况。这次
我的病人又相对简单，所以说得就更少些，讨论别的病人时，想说也很难插上嘴，R2嗓
门大，特自信的那种 （当然人家也确实不错，医学院三年级四年级是全A），Follow那
些病人的同学也不是吃素的，哪容得你讲呢？ 总不能老打断别人吧？前两天mid-
rotation 谈话Attending也说我应该放开点，自己觉得还是比较注意的，结果今天R2又
跟我说像我这样quiet很吃亏的，要我像他学习，大嗓门多嚷嚷，我又何尝不想呢？其
实这个R2对我还是挺好的，挺照顾的，每次writeup他都给我提不少好意见，还在
Attending面前说了我不少好话，说要帮我拿Honor，认为我们拿Honor说明他教的好，
我还是很感激他的。
其实我的知识掌握还是不错的，英文也不错（不过肯定不能跟那些优秀的老美比），很
多时候也觉得可以contribute，可每次Attending都来去匆匆，很多想说的还没说上，
就讨论下一个case呢 |

细的写的话非每天干到半夜才回家不可。我也被attending批过notes写的不像样子。摸
清每个attending的脾气就好了，有的根本不看，就简单快速敷衍一下，有的很挑剔的
，就好好写。也算是triage吧，知道会被骂，但intern实在很难做，事事做的完美，自
己先burn out. |

其实道理大家都是明白的，要是不感激的话我也不来发这个贴，可被逐字逐句点评的时
候心里肯定也是很不爽的，客观来说他的要求实在是高啊，原则上很有道理，实际很难
做得到。 某些意见当然是好的，某些呢，确实是个人偏好，而且有了明确诊断之后与
当时写H&P时想法（就这个病人而言，诊断是出乎所有人意料之外的，尤其是Attending
，当时他根本没觉得这个应该在Differential上）确实差了十万八千里，回头看Notes
不妥之处当然不少. 比如说，他要求PMH按重要性来写 （这个我同意），可这个重要性
只能由当时有限的信息决定。还有这个病人年龄大，History 比较难问，和老伴两人磨
叽半天他们自己也搞不清怎么回事，一些信息是后来他们慢慢想起来告诉我们的，回头
看就会觉得写H&P时疏忽了一些问题。这些都不是努力可以做到的，又因为是Attending
，所以也不好争执，当然据我观察与这种个性的Ａｔｔｅｎｄｉｎｇ争执只能坏事，只
好“怒”在心里。
还有分享一下他说的我觉得最有道理的一点：写ＰＭＨ不能光写当时的Ｄｘ和简要说明
，要写得具体一些（这个很花时间）。比如病人有ＣＡＤ， |

其实他并不是批评你,而是教你.既然不是争输赢,又何须争执.你放心,他既然有心花时间点评,想必看得起你,应该感到高兴呢.
要知道有些人,对你note不满,根本不会对你说的,只会背后看笑话而已,有人当面指出,自己可以少走多少弯路啊.
写note符合要求不是主要,而是其实里面反映了临床思维,即使一开始问病史有不祥尽之处,可以写上某某不清楚,或没有某某病史,起码表明你考虑到了.

round的时候，心内科的医生已经在我的notes后面签了字了。呼吸科的attending愤愤
的说，这是我的notes。按规定一个notes不能两个签字，他想了想在侧面签了自己的意
见。后来又发生了几次这样的事情，我索性把notes揣在口袋了 |

原则上是很Reasonable的事, 可能有些医院做起来并不难，但对我们的Patient
Population很难很难，几乎70% 的病人除了自己现有的症状，就一问三不知，有些人还
到处乱窜，自己都搞不清楚当时在那个医院看了什么病，甚至自己得过什么病也拎不清
楚，能把药瓶带来我们就谢天谢地了。还有些Homeless装病就为了免费吃喝，我还曾经
花了两个多小时打电话FAX要病历，结果就证明了病人确实是说了谎。所以要收集那些
信息绝对工作量是大大的。昨天Oncall，10个病人，3个AMS，2个Syncope，2个
Dementia，还有一个病人昨天去看时说自己有症状X, 今天rounding时Attending问时完
全否认自己的说法，硬说自己症状其实是Y，Attending还很疑惑的看着我们，他看着不
像在扯谎呀，FT, 那难道我们撒谎了？这样的H&P真不是花一般功夫能写好的。我们的
Intern一夜没睡，现在还在写H&P呢，而且我看他的Notes没有一个合乎要求的，Kaka。

前一段时间印象深刻的的一个病人，89岁，开车时晕厥几秒，撞树被吊销执照。 然后
检查， 包括EKG，MRI等等一切正常。 他强烈要求开证明帮他拿回驾照。 fellow当然
不想给他，但好像确实没有什么理由拒绝他的理直气壮的要求。 很为难。 人家吹胡子
说健康几十年，就晕了那么几秒，检查还什么都没有发现异常。 不能判了他的终身监
禁。
后来attending建议加做了一个心电图，大家大喜，就解决这个问题了。老人开始很气
愤，一张心电图纸就判了他的终身监禁？ 后来attending态度很好，给了方案，最后老
人讲道理，自己讲来讲去，绕了回来，不得不自己承认应该感谢医生。好可爱。 |
